Quadriceps inhibition following arthroscopy in patients with anterior knee pain.

OBJECTIVES: This study was aimed at investigating muscle strength and quadriceps inhibition in patients with unilateral anterior knee pain syndrome. DESIGN: Functional assessments were made before an arthroscopic knee surgery and 6 weeks and 6 months following the surgical intervention. BACKGROUND: Traumatic knee injuries have been associated with severe muscle inhibition of the knee extensor muscles. Muscle inhibition is a serious hindrance in the rehabilitation process and prevents full functional recovery of the affected joint. METHODS: 30 patients participated in the study. Isometric quadr

Lower-Limb Muscle-Activation Patterns During Off-Axis Elliptical Compared With Conventional Gluteal-Muscle-Strengthening Exercises.

CONTEXT: Gluteal-muscle strength has been identified as an important component of injury prevention and rehabilitation in several common knee injuries. However, many conventionally prescribed gluteal-strengthening exercises are not performed during dynamic weight-bearing activities, which is when most injuries occur. OBJECTIVES: To compare lower-limb muscle-activation patterns between conventional gluteal-strengthening exercises and off-axis elliptical exercises with motorized foot-plate perturbations designed to activate gluteal muscles during dynamic exercise. EVIDENCE ACQUISITION: Twelve

Blood pressure and heart rate response to isometric exercise: the effect of spinal cord injury in humans.

The systolic (SP) and diastolic (DP) blood pressure and heart rate (HR) responses during a fatiguing isometric contraction of either the handgrip or quadriceps muscles was measured in 139 paraplegic patients divided into five groups (having injuries at T4, T12 and L1-L3 and suffering complete paraplegia and having injuries at T4 but showing incomplete hemiplegia or paraplegia) and 25 controls. While all handgrip exercise was maintained by voluntary effort, only the controls were able to make a voluntary contraction of their quadriceps muscles; the paraplegics had isometric contractions of thei…

Effects of instrument-assisted soft tissue mobilization technique on strength, knee joint passive stiffness, and pain threshold in hamstring shortness.

BACKGROUND: Hamstring shortness is the most common musculoskeletal condition in sports athletes, which often contributes to hamstring strain injury. To effectively mitigate the hamstring shortness, contemporary HR, SCS, and IASTM soft tissue techniques have been used. The best practical evidence about the therapeutic effects and the biomechanical mechanisms underpinning these manual techniques remain unknown. OBJECTIVE: The purpose of this study was to compare the effects of HR, SCS, and IASTM techniques on improving hamstring and quadriceps muscle strength, related ratio, knee joint passive …

Explosive hamstrings-to-quadriceps force ratio of males versus females.

PURPOSE: Females are known to exhibit a greater risk of ACL injury compared to males. Lower explosive hamstrings-to-quadriceps (H/Q) force ratio in the first 150 ms from activation onset could reflect an impaired capacity for knee joint stabilisation and increased risk of ACL injury. However, the explosive H/Q force ratio has not been compared between the sexes. METHODS: The neuromuscular performance of untrained males and females (20 of each) was assessed during a series of isometric knee flexor and extensor contractions, specifically explosive and maximum voluntary contractions of each musc

Disturbed multi-planar muscular support of the knee after anterior cruciate ligament reconstruction.

This study compared lower-limb muscle contributions to multi-planar knee loading between individuals with anterior cruciate ligament reconstruction (ACLR) and healthy controls during sidestep cutting. Eleven ACLR participants with quadrupled hamstring autografts and eleven matched healthy controls performed pre-planned sidestep cutting. Whole-body kinematics, ground reaction forces, and electromyograms (EMG) were recorded. An EMG-informed neuromusculoskeletal modelling approach was used to estimate muscle contributions to anteroposterior force (in bodyweights, BW), varus/valgus moment (BW*Heig

Central somatosensory changes and altered muscle synergies in subjects with anterior cruciate ligament deficiency.

To clarify the mechanisms of compensation in subjects with anterior cruciate ligament deficiency (ACL-D), we examined proprioception, quadriceps strength, somatosensory evoked potentials (SEPs) and muscle responses during gait in varied combinations of speed and incline. Seventeen subjects with ACL-D were grouped by functional level and report of giving way. Seven subjects without ACL-D served as a control sample for muscle response measures. ACL-D subjects with quadriceps weakness experienced giving way and could not resume sport activities. Those without weakness fell into one of two groups:

Fractured Connection: Unmasking Primary Hyperparathyroidism through Bilateral Quadriceps Tendon Rupture - A Case Report.

INTRODUCTION: Bilateral spontaneous quadriceps tendon rupture (QTR) is extremely rare. This condition involves the simultaneous tear of the fibrous tissue connecting the quadriceps muscle to the patella bone. The importance of prompt diagnosis is emphasized, as early recognition is crucial for initiating appropriate treatment and achieving optimal outcomes. CASE REPORT: This article presents a case report detailing the management of bilateral QTR alongside concomitant parathyroid adenoma in a 38-year-old male. The patient underwent open repair of both bilateral quadriceps tendons, followed by…

Effect of whole-body vibration on quadriceps spasticity in individuals with spastic hypertonia due to spinal cord injury.

PURPOSE: Individuals with spinal cord injury (SCI) often have involuntary, reflex-evoked muscle activity resulting in spasticity. Vibration may modulate reflex activity thereby decreasing spasticity. This study suggests feasibility of using whole-body vibration (WBV) to decrease quadriceps spasticity in individuals with SCI. METHODS: Participants were individuals (n=16) with spastic quadriceps hypertonia due to chronic SCI (> 1 year). Quadriceps spasticity was measured by gravity-provoked stretch (Pendulum Test) before (initial) and after (final) a 3 day/week, 12-session WBV intervention. In

Higher Unilateral Muscle Imbalance at the Contralateral Knee 6 Months after Anterior Cruciate Ligament Reconstruction.

There are a considerable number of patients who, after anterior cruciate ligament reconstruction (ACL), suffer from relapses or reduced performance. Data collected from isokinetic dynamometry can provide useful information on the condition of the knee during rehabilitation. Seventy-one young sports patients with ACL reconstruction performed concentric (CON) isokinetic dynamometry (CON/CON 90°/s and CON/CON 240°/s) to assess the muscle strength of the quadriceps (Q) and hamstrings (H) in both knees at 6 months after ACL reconstruction. Limb symmetry index (LSI) and the H/Q ratio were calculated

Defects in sarcolemma repair and skeletal muscle function after injury in a mouse model of Niemann-Pick type A/B disease.

BACKGROUND: Niemann-Pick disease type A (NPDA), a disease caused by mutations in acid sphingomyelinase (ASM), involves severe neurodegeneration and early death. Intracellular lipid accumulation and plasma membrane alterations are implicated in the pathology. ASM is also linked to the mechanism of plasma membrane repair, so we investigated the impact of ASM deficiency in skeletal muscle, a tissue that undergoes frequent cycles of injury and repair in vivo. METHODS: Utilizing the NPDA/B mouse model ASM-/- and wild type (WT) littermates, we performed excitation-contraction coupling/Ca2+ mobiliza

Hamstring muscle strain recurrence and strength performance disorders.

We determined the frequency of strength disorders in 26 athletes with a history of hamstring muscle injury and recurrent strains and discomfort. We also assessed the effectiveness of rehabilitation to correct muscle performance. After concentric and eccentric isokinetic assessment, 18 athletes were found to have strength deficits, as determined by statistically selected cutoffs of peak torque, bilateral differences, and the flexors/quadriceps ratio. The discriminating character of the eccentric trial was demonstrated, combining a preferential eccentric peak torque deficit and a significant red
