Sickle Cell Trait-Related Exertional Deaths: Observations at Autopsy and Review of the Literature.

Sickle cell trait-related exertional deaths, although rare, are well-accepted in the field of forensic pathology; however, the increased risk of sudden unexpected deaths in persons with sickle cell trait undergoing strenuous physical activity may be an underappreciated acute phenomenon in the clinical realm. Herein, we report two cases of sickle cell trait-related exertional deaths of active duty military members, with a review of the literature including the pathophysiology of sickle cell trait-related deaths and current military screening guidelines.

A unique cause of intestinal and splenic infarction in a sickle cell trait patient.

Sickle-cell trait is a common genetic abnormality in the African American population. A sickle-cell crisis in a patient with sickle-cell trait is uncommon at best. Abdominal painful crises are typical of patients with sickle cell anemia. The treatment for an abdominal painful crisis is usually medical and rarely surgical. We present the case of a cocaine-induced sickle-cell crisis in a sickle-cell trait patient that resulted in splenic, intestinal, and cerebral infarctions and multisystem organ failure necessitating a splenectomy, subtotal colectomy, and small bowel resection. This case highli

Cardiac rehabilitation participant with sickle cell trait and statin-related hepatotoxicity: a case report.

PURPOSE: Exercise recommendations for patients with sickle cell trait (SCT) and myocardial infarction (MI) are limited. This case report describes such a patient, who subsequently participated in cardiac rehabilitation (CR). CASE: Patient experienced sudden intense chest pressure and shortness of breath while singing in church. Electrocardiogram and cardiac enzymes confirmed acute ST elevation MI. Laboratory tests indicated hyperlipidemia and elevated liver enzyme levels >4 times the upper normal limit (UNL). Day 40 post-MI, the patient returned to the hospital complaining of daily chest disc…

Sickle cell trait-associated deaths: a case series with a review of the literature.

This study presents a series of 16 carriers of hemoglobin S (HbS) who died during various circumstances. Many of the cases were associated with mild to moderate exertion. The onset and/or duration of symptoms varied from a few minutes to several hours with many displaying a prolonged lucid interval with stable vital signs. Despite seeking medical treatment, sickle cell trait-related micro-occlusive crisis was never considered in the differential diagnosis. Several cases were associated with sudden death. In those deaths which were delayed, high anion gap and uncompensated metabolic acidosis we…

Pressurized flight immediately after splenic infarction in two patients with the sickle cell trait.

Splenic infarction in individuals harboring the sickle cell trait can occur in the setting of exposure to low oxygen tension at high altitudes. While this is a concern in unpressurized aircraft flight, it has not been well documented in pressurized flight. What has not been addressed is whether this relative safety of pressurized flight extends to the postinfarction period and whether or not pressurized flight in the immediate post-infarction period, especially air evacuation, would change the patient's outcome. We present two cases of splenic infarction suffered during climbing Mt. Fuji (12,3…

Effects of strenuous exercise on autonomic nervous system activity in sickle cell trait carriers.

This study compared the nocturnal autonomic nervous system (ANS) activity in seven sickle cell trait (SCT) carriers and six subjects with normal hemoglobin in response to exercise Sympathetic and parasympathetic indices of nocturnal ANS were measured in the two groups before and 24 and 48 h after a strenuous exercise consisting of the repetition of three maximal exercise bouts. Global ANS activity decreased 24 h after exercise in both groups and was lower before as well as after exercise in SCT carriers. Indices of parasympathetic activity were lower in SCT carriers at all times, indicating a …

Prolonged survival of a patient with sickle cell trait and metastatic renal medullary carcinoma.

PURPOSE: The treatment and outcome of a patient with sickle cell trait and metastatic renal medullary carcinoma is described. PATIENT AND METHODS: A 12-year-old boy with sickle cell trait had metastatic renal medullary carcinoma. After surgical resection of the primary tumor, he received chemotherapy with methotrexate, vinblastine, doxorubicin, and cisplatin. The carcinoma progressed after a 6-month period of stable disease. At that time, he received chemotherapy including ifosfamide, etoposide, carboplatin, and topotecan. RESULTS: The patient died of progressive disease 15 months from diagn…

[Phospholipid composition and content of the erythrocyte membrane in carriers of sickle cell trait].

By its frequency, but also by its morbidity and mortality, sickle cell disease is a real problem of public health in Senegal. It is an inherited disease of the red blood cell, characterized by vaso-occlusive manifestations in several organs, and chronic hemolytic anemia. Out of this work carried upon subjects presenting sickle cell trait (AS), we especially studied the membrane phospholipids. The aims of this study was to identify and to measure the major phospholipids of the red blood cell membrane (lecithin, lysolecithin, cephalin, lysocephalin and sphingomyelin). We noted a significant decr…

Rhabdomyolysis and hemolysis associated with sickle cell trait and glucose-6-phosphate dehydrogenase deficiency.

We report a case of severe oxidative hemolysis and rhabdomyolysis in a patient with sickle cell trait and glucose-6-phosphate dehydrogenase (G6PD) deficiency. The patient was a 34-year-old black man admitted 24 hours after vigorous exercise with myalgias, malaise, myoglobinuria, anemia, low haptoglobin, and a peripheral blood smear with bite cells consistent with oxidative hemolysis. He had two similar episodes within 21 months of the initial admission. Subsequent evaluation resulted in the diagnosis of sickle cell trait and G6PD deficiency; muscle enzyme levels were normal. G6PD deficiency an…

Effects of physical stress on peroxide scavengers in normal and sickle cell trait erythrocytes.

Sickle cell trait subjects are potentially at risk with microvascular complications frequently associated with high altitude and extreme exertion. Studies conducted in our laboratory suggest that exposure of sickle cell trait subjects to treadmill exercise resulted in an increase not only in the susceptibility of red blood cells to oxidation but also in the density of RBC membrane. The trend of increased susceptibility of red cells to oxidation as a consequence of exercise was associated with relatively excessive production of H2O2, inefficient breakdown of H2O2 by GSH-Px- and catalase-catalyz…

Cardiopulmonary function in men with sickle cell trait who reside at moderately high altitude.

Sickle cell trait has been associated with an unexplained increased risk of sudden death during military basic training. Previous studies of cardiopulmonary function in persons with sickle cell trait at sea level or after brief exposure to moderately high altitude have not shown significant abnormalities. To determine whether cardiopulmonary function is impaired in men with sickle cell trait who chronically reside at moderately high altitude, we prospectively studied 17 men with sickle cell trait and 25 men with normal hemoglobin. All subjects had resided at > or = 1609 m above sea level for a…

Maternal sickle cell trait is not a cause of low birthweight in Nigerian neonates.

As a result of reports of reduced birthweight in neonates of mothers with hemoglobin AS (Hb-AS) in other populations, we compared birthweights in 57 Hb-AS primigravidas in Nigeria with 60 controls who were sickle negative. There was no statistically significant difference in mean birthweight or placental weight between the groups. This finding may be due to development of resistance against malaria infection by Hb-AS women in this population.

Malignant fibrous histiocytoma in bone infarct. Association with sickle cell trait and alcohol abuse.

A 42-year-old black man with sickle cell trait and a history of alcohol abuse had a malignant fibrous histiocytoma of the distal femur. The tumor developed in a bone infarct that had been present for at least 54 months. This is the first reported association of malignant fibrous histiocytoma and bone infarct in which the infarct was related to sickle cell trait and alcohol abuse. A detailed map of the sections from the resected femur disclosed a spectrum of cytologic changes in the sarcoma that ranged from bland fibroblasts in the center of the tumor to high-grade sarcoma at the periphery. Awa…

Autosomal dominant cone-rod dystrophy associated with sickle-cell trait in a Sicilian family.

Three Sicilian siblings had a typical cone-rod dystrophy. The same ocular anomaly was probably present in their mother and in the maternal grandmother. The three probands were also carriers of Hb S, but this last anomaly was also present in an unaffected brother. It is concluded that the cone-rod dystrophy of this family is inherited as an autosomal dominant character and that its association with sickle-cell trait is coincidental.
