Freeman-Sheldon syndrome. A case report and review of the literature.

The Authors describe a case of Freeman-Sheldon Syndrome, a rare congenital autosomal dominant disorder (gene mapped on chromosome 11p15.5) characterized by microstomia with crinkled lips, camptodactyly with ulnar deviation of the fingers and equinus-varus-supine clubfoot. The autosomal recessive form, even rarer and difficult to recognize, has a more severe clinical manifestation. The symptomatology is worsened by breathing and swallowing disorders due to the small orifices of the mouth and nose, which sometimes require tracheotomy to avoid obstruction of the airways.

Surgical importance of highly located innominate artery in neck surgery.

Massive hemorrhage is an unusual complication of tracheotomy, and the most common causes are injury to the anterior jugular veins and the thyroid isthmus, or unrecognized variations of the vascular structures, such as the arteria thyroidea ima. We present a case of high-running innominate artery that ascends until the third tracheal ring and courses horizontally anterior to the trachea in a patient with laryngeal carcinoma. If not noticed during tracheolaryngeal surgery, trauma to this vessel and subsequent hemorrhage may be fatal. In this report, the case is presented and the surgical signifi…

Familial association of camptodactyly, mental retardation, whistling face and Pierre Robin sequence.

Two sibs are reported with severe developmental retardation combined with the clinical triad of camptodactyly, whistling face and Pierre Robin sequence as clinical signs of fetal hypokinesia. In spite of tracheotomy, the first child of the family died 10 hours after birth. A sister of this child was born 1 year later. During pregnancy prenatal diagnosis of hydrocephaly was made by ultrasonographic examination. MRI scan showed holoprosencephaly. At 15 months of age psychomotor development is severely impaired, birth and length are delayed.

Malformation and stenosis of the cricoid cartilage in association with Larsen's syndrome.

Three patients with the typical features of Larsen's syndrome are described. All three developed severe respiratory symptoms caused by a congenital subglottic stenosis. Tracheotomy and treatment of the stenosis by means of laryngotracheoplasty resulted in complete collapse of the cricoid cartilage and the proximal tracheal skeleton. Lack of rigidity of the laryngeal and tracheal cartilages in patients with Larsen's syndrome could well be responsible for this failure. Surgical treatment consisted of resection of the stenotic and collapsed areas and end-to-end anastomosis. This therapy was event…

Lingual tonsil hypertrophy producing obstructive sleep apnea.

The hypertrophy of the lingual tonsil is a rare occurrence in adults. This disorder may cause obstructive sleep apnea (OSA) and also may lead to a life-threatening obstruction of the upper airway. Diagnosis of lingual tonsil hypertrophy in patients with OSA requires a careful examination by advanced technologic methods. The sleep apnea is successfully treated by lingual tonsillectomy, with or without tracheotomy. In this study the case of an adult with OSA caused by lingual tonsil hypertrophy which was treated with lingual tonsillectomy is presented, and the diagnosis and the treatment procedu…

Upper airway function in Ondine's curse.

Congenital central alveolar hypoventilation (Ondine's curse) is a rare disorder with absence of automatic control of ventilation but preservation of voluntary breathing. Phrenic pacing, used to treat this condition, is usually not successful without a tracheotomy. We performed fiberoptic videoendoscopy on an affected infant airway. During wakefulness and ventilation by a negative pressure ventilator, the airway was normal, but with sleep onset, passive inspiration produced phasic epiglottic collapse. This case demonstrates the importance of central control in maintaining upper airway patency.

Sinus histiocytosis with massive lymphadenopathy in a Japanese male with symptoms of dyspnea and visual disturbance.

A case study of sinus histiocytosis with massive lymphadenopathy is reported. A Japanese male developed lymphadenopathy in the nasal cavity 10 years ago. Biopsy specimens demonstrated massive histiocytosis with phagocytosis of lymphocytes, plasmacytes, and erythrocytes. During the course of his illness, the patient experienced an episode of massive nasal bleeding, and a tracheotomy was performed because of dyspnea. He had also an operation by Killian's approach to relieve a visual disturbance. The patient has been treated with cyclophosphamide and prednisolone for 6 months.

Iatrogenic retropharyngeal emphysema with impending airway obstruction.

The causes, diagnosis, sequelae, and treatment of interstitial emphysema of the face, neck, and thorax have been previously described in the medical and dental literature and are reviewed here. We present a unique case of a patient who, following dental amalgam restoration, developed severe retropharyngeal emphysema with impending airway obstruction, cervicofacial subcutaneous emphysema, and pneumomediastinum. Management consisted of urgent tracheotomy, broad-spectrum prophylactic antibiotic coverage, close observation, and reassurance. The likely mechanisms of precipitation and extension of t…

Management of subglottic stenosis in infancy and childhood. Review of a consecutive series of cases managed by surgical reconstruction.

The management of mature subglottic stenosis secondary to endotracheal intubation in infancy and childhood remains a controversial issue. If treated similarly to congenital subglottic stenosis by tracheotomy and a "wait-and-see" period then there is a considerably higher mortality and morbidity for the acquired disease compared with the congenital. Eighteen children with severe acquired subglottic stenosis managed by surgical reconstruction are presented; 17 have been successfully extubated. The longest follow-up is 5 1/2 years. No evidence of interference with laryngeal growth is evident.

Retrognathia and sleep apnea. A life-threatening condition masquerading as narcolepsy.

The association of sleep apnea with daytime hypersomnolence without obesity, and its potentially lethal cardiopulmonary sequelae, make it crucial that this condition be distinguished from narcolepsy. A patient with retrognathia who had been diagnosed as a narcoleptic for 15 years had the primary complaint of excessive daytime sleepiness. Sleep laboratory evaluation showed severe hypoxemia and a mean of 366 upper airway obstructions per night. The patient was treated with a tracheotomy; this resulted in relief of the sleep-related upper airway obstructions, hypoxemia, and hypersomnolence.

[The laryngeal cyst of the new born (author's transl)].

Only 30 cases of cysts of the ventricular cord in the new born baby have been published. Because of the respiratory stridor one third of the cases died soon or later. The other 2/3 were treated by endolaryngeal procedures. With a case of a new born suffering from a large cyst of the ventricular cord the author demonstrates the possible danger and inefficiency of different endolaryngeal procedures. Only the external operation through the thyrohyoid membrane with complete removal of the cyst - without tracheotomy--resulted in permanent cure.

Increased Neck Visual Fixation in Children With Tracheostomies: An Eye-Tracking Study.

OBJECTIVE: The aim of this study is to objectively quantify the attentional distraction effect of a tracheostomy on visual attention and gaze using eye-tracking technology. METHODS: This was a cross-sectional observational study. An eye-tracking device was used to capture eye movements of participants over 5 years of age who were shown 20 images of children with and without tracheostomies. The primary outcome was the number and duration of fixations on different mapped regions of the head and neck, including the central triangle (CT) and peripheral triangle (PT) of the face, and the anterior
