Glaucoma in retinopathy of prematurity: A review.

Glaucoma in retinopathy of prematurity (ROP) is a challenge. It is typically secondary, though it may be associated with primary congenital glaucoma. Secondary causes are multifactorial, with mechanisms and severity potentially linked to the stage of ROP, time of presentation, or interventions for its treatment. Anecdotal reports of glaucoma following anti-vascular endothelial growth factor injection exist, but conclusive evidence is lacking. The most important cause of glaucoma in ROP is secondary angle closure in patients with stage 5 ROP. Clinical features such as corneal clouding and incre

Perioperative management of transcatheter aortic valve implantation in acquired von Willebrand syndrome secondary to monoclonal gammopathy: a case report.

BACKGROUND: Acquired von Willebrand syndrome (AvWS) is a rare bleeding disorder associated with conditions like monoclonal gammopathy of und … ormalized FVIII and vWF levels. TAVI was performed without specific haemostatic measures other than vascular closure devices. There were no bleeding complications, and the patient was discharged without antiplatelet therapy. At one-year follow-up, he had no thrombotic or haemorrhagic events. DISCUSSION: We report the management of a patient affected by AvWS secondary to …

New approaches to achieving hemostasis after venous access in cardiovascular patients.

Recent decades have seen a series of advances in percutaneous transvenous procedures for cardiac arrhythmias, including the implantation of … ase patient discomfort, and address the risk of access site complications. These approaches include vascular closure devices and subcutaneous suture techniques including figureof- eight and purse-string sutures. This article reviews the new approaches applied to achieve venous access site hemostasis in patients undergoing transvenous procedures for cardiac arrhythmias.

Antegrade common femoral artery closure device use is associated with decreased complications.

OBJECTIVE: Antegrade femoral artery access is often used for ipsilateral infrainguinal peripheral vascular intervention. However, the use of closure devices (CD) for antegrade access (AA) is still considered outside the instructions for use for most devices. We hypothesized that CD use for antegrade femoral access would not be associated with an increased odds of access site complications. METHODS: The Vascular Quality Initiative was queried from 2010 to 2019 for infrainguinal peripheral vascular interventions performed via femoral AA. Patients who had a cutdown or multiple access sites were

Safety of very early sheath removal in patients treated with REG1 for acute coronary syndromes: insights from the RADAR trial.

BACKGROUND: RADAR compared REG1 (25%, 50%, 75%, 100% reversal) with unfractionated heparin (UFH) in 640 acute coronary syndrome (ACS) patients (479 REG1 patients, 161 UFH patients) undergoing an invasive management strategy. We sought to determine whether the REG1 anticoagulation system allows for safer early arterial sheath removal following cardiac catheterization. METHODS: REG1 patients had arterial sheath removal immediately post catheterization. We measured arterial sheath management outcomes and vascular access complications in patients who had sheath removal without vascular closure de

A "modified crossover technique" for vascular access management in high-risk patients undergoing transfemoral transcatheter aortic valve implantation.

OBJECTIVES: To describe results from our "modified crossover technique" for vascular access management during transcatheter aortic valve implantation (TAVI). BACKGROUND: Vascular access management remains a major cause of complications following TAVI due to the large bore sheaths required. METHODS: All suitable patients undergoing TAVI in our center, between June and August 2011, underwent our "modified crossover technique," which enables the passage of a balloon through left radial access and inflation in the proximal iliac to allow percutaneous closure in a clean field. RESULTS: In total,

Rare Case of Paradoxical Embolism Through Anomalous Systemic Vein-to-Pulmonary Vein Connection: When Zebras Are Zebras.

We report a unique case of a congenital anomalous systemic vein-to-pulmonary vein connection with bidirectional flow causing paradoxical embolic stroke. A 64-year-old woman with no significant medical history presented with a multiterritorial embolic stroke after ankle fracture surgery. Computed tomography (CT) revealed occlusion of the left middle cerebral artery (MCA) and ischemia in the right MCA territory. Successful endovascular clot retrieval was performed, targeting the left MCA occlusion. Extensive investigation identified a rare connection between the right lower pulmonary vein and su

Percutaneous closure of a large unligated vertical vein using the Amplatzer Vascular Plug II after supracardiac total anomalous pulmonary venous connection (TAPVC) repair.

It is well known that the vertical vein (VV) may have to be left open after repair of a total anomalous pulmonary venous connection (TAPVC) in children with preoperative obstruction, whose left heart chambers are small (Cope et al. in Ann Thorac Surg 64:23-29, 3). An unligated VV has been found to reduce pulmonary arterial pressure, decrease perioperative pulmonary hypertensive crisis, and provide better hemodynamics postoperatively (Chowdhry et al. in J Thorac Cardiovasc Surg 133:1286-1294, 2). Although these VVs are expected to close later, they may remain patent in about half of these child

Preclosure technique versus arterial cutdown after percutaneous cannulation for venoarterial extracorporeal membrane oxygenation.

BACKGROUND: Arteriotomy repair through the preclosure technique during elective arterial access procedures is well documented. Outcomes associated with application of this technique to the removal of arterial access cannulas in patients undergoing urgent venoarterial extracorporeal membrane oxygenation (VA-ECMO) have not previously been reported. METHODS: We reviewed the records of consecutive patients who required VA-ECMO for cardiogenic shock. Patients were compared by use of the preclosure device (Perclose ProGlide Suture-Mediated Closure System; Abbott Vascular, Abbott Park, Ill) at time

Surgical Treatment of Mediastinitis with Omentoplasty in Ventricular Assist Device Patients: Report of Referral Center Experience.

Mediastinitis is more frequent in patients with implantable ventricular assist devices (VADs) than in other cardiac surgery patients and carries significant mortality. We report our experience with a stepwise approach including aggressive debridement, jet lavage, vacuum assisted closure dressing, and finally coverage with well-vascularized and immune-active omental flaps in VAD patients with infective mediastinitis. We retrospectively collected and analyzed data of patients with continuous flow VAD who underwent plasty with the omental flap because of mediastinitis at Deutsches Herzzentrum Ber

AIM: This study aims to describe the feasibility and safety of direct occluded vessel puncture as a new access site for complex peripheral artery occlusive disease. METHODS: Eleven consecutive patients with symptomatic peripheral artery disease underwent endovascular therapy using the direct occluded vessel puncture technique. The occluded vessel was punctured using a dedicated 20 G needle and the Hi-Torque Command 18 ST guidewire under duplex echo or fluoroscopic guidance, and a 6 Fr sheath was then inserted. Hemostasis was achieved with the Exoseal® Vascular Closure Device. RESULTS: Direct

Reduced plasma level of basic fibroblast growth factor is associated with incomplete device endothelialization at six months following left atrial appendage closure.

OBJECTIVES: To investigate whether inflammatory and growth factors (IGFs) were associated with incomplete device endothelialization (IDE) at 6 months after successful left atrial appendage closure (LAAC). BACKGROUND: IDE after LAAC is correlated with device-related thrombus (DRT) formation and subsequent thromboembolic events. However, biomarkers for early detection of IDE remain lacking. METHODS: Plasma levels of IGFs including basic fibroblast growth factor (bFGF), platelet derived growth factor (PDGF), stromal cell derived factor (SDF)-1a, transforming growth factor (TGF)-β1, vascular gro

Corticosteroid Therapy and Vascular Complications in Patients Undergoing Transcatheter Aortic Valve Replacement: A Meta-analysis With Meta-regression.

BACKGROUND: Corticosteroid use is associated with vascular fragility, prolonged wound healing, and infections. Therefore, we sought to compare outcomes between patients with aortic stenosis undergoing transcatheter aortic valve replacement who were using corticosteroids versus those who were not. METHODS: This is a study-level meta-analysis and meta-regression of observational studies. The primary end points of this study were rates of vascular complication (both major and minor), life-threatening bleeding, and 30-day mortality. Secondary end points included acute kidney injury rates, annular

Percutaneous Endovascular Management of MANTA-Related Vascular Occlusion.

BACKGROUND: Vascular closure devices (VCDs) are the standard alternative to surgical cutdown for large-bore arterial access (LBAA) but can lead to complications like bleeding and common femoral artery (CFA) occlusion that require intervention. This manuscript outlines our approach using percutaneous endovascular intervention (PEI) to manage femoral artery occlusion caused by the MANTA (Teleflex, Morrisville, SC), the only collagen plug-based VCD indicated for LBAA. METHODS: Consecutive patients who experienced occlusive complications following MANTA deployment at the conclusion of transcutane…

Lateral Axillary Exposure for Antegrade Access during Endovascular Repair of Complex Abdominal Aortic and Thoracoabdominal Aneurysms.

BACKGROUND: During endovascular treatment of pararenal aortic aneurysms (PAA) and thoracoabdominal aortic aneurysms (TAAA), our antegrade vascular access of choice is a lateral axillary exposure (LAE). We directly access the axillary artery with multiple sheaths followed by primary closure of the axillary artery at case completion. The aim of this study is to describe our technique and to report our results with this approach. METHODS: This study is a single-institution, retrospective review of 53 patients who were treated with parallel grafts for endovascular repair of PAA and TAAA from 2006
