Six-month pivotal results of tack optimized balloon angioplasty using the Tack Endovascular System in below-the-knee arteries.

OBJECTIVE: No vascular implant is commercially available in the United States to treat post-angioplasty dissections in below-the-knee (BTK) arteries. The Tack Endovascular System (Intact Vascular, Wayne, Pa) is purpose-built to repair postpercutaneous transluminal angioplasty (PTA) BTK dissections. A trial was conducted to investigate the safety and efficacy of the first-of-a-kind implantable BTK device to treat post-PTA dissections in the setting of critical limb ischemia. METHODS: The present prospective, single-arm, multicenter study evaluated the Tack Endovascular System for treating post

Salvage of anterolateral thigh (ALT) flap with indocyanine green assessment of a flap pedicle and subsequent perforator-to-perforator anastomosis.

Indocyanine green (ICG) fluorescence angiography has emerged as an intraoperative method to accurately assess real-time tissue vascularity, perfusion and anastomotic patency in flap surgery. We illustrate a complex case of elbow reconstruction in an elderly patient with a free anterolateral thigh flap, which relied on intraoperative ICG to evaluate the flap pedicle and map the site of arterial occlusion. Supermicrosurgical instrumentation was employed to perform complex perforator-to-perforator anastomosis following resection of the vascular site of the lesion. These unique applications in a p

Penetrating thermal vascular injury in a child: a case report.

Penetrating vascular trauma in children is most commonly seen in wartime settings or more recently as a result of attempts at invasive imaging. Treatment of these patients can be more complicated than treatment of trauma in adults and must include maintenance of symmetric limb growth, compensatory growth of the vascular graft and its anastomoses, and the need for very long-term graft patency. This report describes the care of a 5-year-old child who was impaled by a hot metal pipe and sustained a penetrating thermal injury to the distal external iliac artery and vein. Issues such as conduit cho

Endovascular stenting of superficial femoral artery stenosis and occlusions: results and risk factor analysis.

PURPOSE: To determine the early and midterm results of femoro-popliteal angioplasty with adjunctive stenting and to identify factors affecting early and continuing success. METHODS: Sixty-nine consecutive balloon angioplasty procedures on the superficial femoral artery (SFA) were performed in 60 limbs and 55 patients in the operating theater. Fifty-two percent of lesions were occlusions, and 87% involved the distal half or the whole segment of the SFA. Immediate endovascular stenting was used in all procedures, involving the placement of 105 stents, with a mean stented length of 13.8 cm. Twen

Brachiocephalic jump graft fistula: an alternative for dialysis use of elbow crease veins.

Elbow crease fistula can be an alternative for autologous vascular access. Either brachiocephalic or brachiobasilic fistulas could be chosen according to the venous anatomy at the elbow crease. When a median antecubital vein is not present, the cephalic vein is usually too far away from the brachial artery. Thus, a end-to-side fistula must usually be performed after an extensive dissection of the distal part of the vein. In this way, only the proximal cephalic vein can be used for dialysis. To overcome this drawback, a brachiocephalic jump graft fistula was designed. A short segment of polytet

Do routinely registered preoperative data provide prognostic information on the short-term outcome of distal bypass surgery?

One hundred and thirty and patients were operated on with unilateral distal bypass procedures for critical ischemia. Fifty-one patients (39%) ended up with graft occlusion or required major amputation within six months after reconstruction. Two-year graft patency was 38%. These rather disappointing results may be partly ascribed to inadequate preoperative selection of patients. Logistic regression model was used to assess whether the independent variables age, gender, diabetes mellitus, smoking, ankle-brachial pressure index (ABI), type of graft, coronary heart disease (CHD), previous vascular

Influence of hormone replacement therapy on graft patency after femoropopliteal bypass grafting.

OBJECTIVE: Thromboembolic events are more frequent in women with established cardiovascular disease who are receiving hormone replacement therapy (HRT). The effect of HRT on the outcome of women undergoing infrainguinal bypass grafting is unknown. The purpose of this study was to estimate the influence of risk factors, in particular HRT, on the outcome of women undergoing femoropopliteal bypass grafting. METHODS: During a 5-year period (between 1993 and 1998), 131 femoropopliteal bypass graft procedures were performed in 106 women. The criteria prepared by the Ad Hoc Committee on Reporting St

[Endoscopic harvest of great saphenous vein for infrainguinal arterial bypass: summary of our initial experience].

INTRODUCTION: This study was undertaken to determine the feasibility of endoscopic vein harvest (EVH) for infrainguinal arterial bypass surgery. We describe our initial experience and early results of bypasses done using this minimally invasive approach. METHOD: From April 2012 to March 2015, 16 patients underwent 16 femoropopliteal bypass operations with great saphenous vein (GSV) being harvested by endoscopic technique. The indication for intervention was critical limb ischemia (Rutherford category "5") in 7 patients (43.7%) and severe intermittent claudication (Rutherford category "3") in

Application of intravascular lithotripsy in a case of severely calcified arteriovenous fistula stenosis: A case report and technical note.

BACKGROUND: Severe calcification-associated stenosis of arteriovenous fistula (AVF) poses a significant challenge in the maintenance of hemodialysis access. Conventional balloon dilation often faces issues such as incomplete dilation and high complication rates. Intravascular lithotripsy (IVL) has been widely applied in coronary and peripheral artery fields, but its application for AVF calcification remains limited, with only a few technical reports available. METHODS: We present a case of severe circumferential calcification and stenosis of a radiocephalic arteriovenous fistula treated with

Femorofemoral bypass: appropriate application based on factors affecting outcome.

Current surgical options for the treatment of unilateral iliac artery occlusive disease are quite varied and include aortic-based procedures, femorofemoral bypass ileofemoral bypass, and transluminal angioplasty and stenting. Patients with unilateral iliac disease present with a variety of anatomic, physiological and clinical situations. As a result, the surgical options are complementary rather than competitive, and their use needs to be tailored on an individual basis. Femorofemoral bypass, despite its relatively lower patency rate compared with aortic-based inflow procedures, is a useful op

A new operation with inadequate Brescia fistulae for preservation of more proximal veins.

PURPOSE: The purpose of this study was to test the hypothesis that venous outflow of a Brescia fistula that is patent but unusable for one of a variety of reasons can provide adequate drainage to sustain a prosthetic arteriovenous graft based on the brachial artery, thus sparing more proximal veins for future access procedures. METHODS: The operation consists of placement of a prosthetic graft between the brachial artery in the antecubital space and the cephalic vein at the wrist. RESULTS: Between December 1998 and November 1999, 14 patients (eight male and six female; age range, 34 to 73 ye

The Impact of Reintervention on Arteriovenous Fistula Maturation and Functional Patency in the Hemodialysis Fistula Maturation Study.

RATIONALE & OBJECTIVE: Arteriovenous dialysis fistula (AVF) reinterventions are sought to assist with maturation and/or maintain functional patency; however, their ultimate impact on fistula lifespan remains poorly documented. Furthermore, current clinical practice guidelines lack clarity regarding reintervention paradigms to achieve optimal AVF performance. Therefore, the purpose of this study was to document reintervention rates and their association with AVF maturation and functional patency among patients enrolled in the Hemodialysis Fistula Maturation (HFM) study. STUDY DESIGN: Retrospec

The impact of gender on outcome after infrainguinal arterial reconstructions for peripheral occlusive disease.

OBJECTIVE: The role of gender on the outcome of infrainguinal arterial revascularization (IAR) for peripheral arterial occlusive disease remains uncertain. This study analyzed the outcome of IARs performed over 15 years, stratifying the results by sex. METHODS: Details of consecutive patients undergoing primary IAR for peripheral arterial occlusive disease from 1995 to 2009 at our institution were prospectively stored in a vascular registry. Demographics, risk factors, indications for surgery, inflow sources, outflow target vessels, types of conduit, and adverse outcomes were analyzed. Postop

A comparative evaluation of polytetrafluoroethylene, umbilical vein, and saphenous vein bypass grafts for femoral-popliteal above-knee revascularization: a prospective randomized Department of Veterans Affairs cooperative study.

PURPOSE: Currently, the choice of a vascular prosthesis for a femoral-popliteal above-knee arterial bypass graft is left to the surgeon's preference, because the available information on comparative evaluations is inconclusive. The Department of Veterans Affairs (VA) Cooperative Study 141 was established to identify whether improved patency exists with different bypass graft materials for patients with femoral-popliteal above-knee bypass grafts. METHODS: Between June 1983 and June 1988, 752 patients at 20 VA medical centers were randomized to receive either an externally supported polytetrafl

Clinical Experience with the Gore Hybrid Vascular Graft in Complex Revascularizations Demonstrates Safety and Efficacy.

BACKGROUND: Arterial revascularizations can present significant challenges when vessels are disadvantaged and advances in technology present the surgeon with innovative opportunities. A number of studies have used the GORE® Hybrid Vascular Graft (GHVG), and we have been using this device in arterial revascularizations since it came to market. The aim of this study is therefore to present a large single-center experience using the GHVG. This series presents patients with complex revascularizations in multiple vascular beds. METHODS: We retrospectively analyzed a single-center series of 43 pati

Tibial fractures with infrapopliteal arterial injuries.

Tibial fractures with associated infrapopliteal arterial injuries have been inadequately documented in the literature. Eighteen patients with these injuries were admitted to Boston City Hospital during a 10-year period. Three patients required below-knee amputation, and two of these had ischemic intervals of greater than 8 h before vascular treatment. Six patients had delayed diagnoses of arterial injuries, but none required amputation. There were 15 open and three closed tibial fractures. Nine fractures were badly comminuted. Eight of the 14 viable limbs had delayed unions, but only one was a
