Costimulatory blockade depletes T peripheral helper, late-activated naïve, and DN2 B cells in rheumatoid arthritis.

UNLABELLED: Rheumatoid arthritis (RA) is a chronic inflammatory autoimmune disease that causes joint destruction along with extra-articular morbidity and early mortality. Abatacept (CTLA-4 Ig), a blocker of lymphocyte co-stimulation, has become a well-accepted biologic treatment with proven efficacy in established-RA and for preventing disease onset in predisposed individuals. To investigate the immunologic implications of abatacept treatment, we conducted a prospective, open-label trial with multi-omic single-cell analyses of lymphocytes and BCR repertoire profiling at predefined intervals. Expression of the NF-kappaB targets BCL2 and BIRC5/Survivin characterizes small B-cell and aggressive B-cell lymphomas, respectively.

Nuclear factor kappa B (NF-kappaB) activation has been proposed as a cardinal feature of tumourigenesis, although the precise mechanism, frequency, relevance, and extent of NF-kappaB activation in lymphomas remain to be fully elucidated. In this study, expression profiling and tissue microarray studies of 209 and 323 non-Hodgkin's lymphomas (NHLs) respectively, including the most frequent sub-types of NHL, were employed to generate a hypothesis concerning the most common NF-kappaB targets in NHL. These analyses showed that NF-kappaB activation is a common phenomenon in NHL, resulting in the ex

Biphenotypic bigenotypic lymphoma with simultaneous expression of PAX5/BSAP and B- and T-cell markers.

Lymphomas are currently categorized according to their origin from a B or T lymphocyte. Immature and less commonly mature (peripheral) lymphomas may harbor rearrangements of both the B- and T-cell antigen receptor genes (dual genotype or bigenotype). Rarely, cells in lymphoma with a single genotype simultaneously express both B- and T-cell markers (biphenotypic lymphomas). We discuss the diagnostic and clinical implications in the case of a 42-yr-old female with a peripheral CD30+ lymphoma that displayed both characteristic B- and T-cell surface antigens and clonal rearrangement of B- and T-ce

Adoptive B cell therapy for chronic viral infection.

T cell-based therapies have been widely explored for the treatment of cancer and chronic infection, but B cell-based therapies have remained largely unexplored. To study the effect of B cell therapy, we adoptively transferred virus-specific B cells into mice that were chronically infected with lymphocytic choriomeningitis virus (LCMV). Adoptive transfer of virus-specific B cells resulted in increase in antibody titers and reduction of viral loads. Importantly, the efficacy of B cell therapy was partly dependent on antibody effector functions, and was improved by co-transferring virus-specific

[Sjögren's syndrome and non-Hodgkin's lymphoma: from pathophysiology to treatment].

Although infrequent, the development of non-Hodgkin's lymphoma (NHL) during the course of Sjögren's syndrome represents a major complication. Nethertheless, most NHL developing in patients with Sjögren's syndrome are B cell lymphomas involving the marginal zone, localized to extranodal sites. Predictive factors include lymphadenopathy, splenomegaly, neutropenia, cryoglobulinemia and low C4 levels. The treatment of B cell lymphomas depends on its nature and localization. Biotherapies targeting the B lymphocyte, such as rituximab, a chimeric monoclonal anti-CD20 antibody, or more recently target

Epigenetics in B-CLL.

B-cell chronic lymphocytic leukemia (B-CLL) is the most common hematological malignancy in adults. Its clinical course is heterogeneous, ranging from indolent forms with slow progression to aggressive variants refractory to conventional treatment. In recent years, it has been shown that epigenetic alterations, such as DNA methylation, histone modifications, and regulation by noncoding RNAs, especially microRNAs (miRNAs), play a central role in the prognosis of this disease. For this reason, the analysis of epigenetic mechanisms has become an essential approach both to understand the progressio

[Chinese expert consensus on the diagnosis and management of primary mediastinal large B-cell lymphoma (2024)].

Primary mediastinal large B-cell lymphoma (PMBL) is an aggressive B-cell lymphoma that is thought to arise from thymic (medullary) B cells and has unique clinicopathologic and molecular features. In recent years, the understanding of the pathogenesis and treatment of PMBL has been updated to varying degrees, particularly in the area of new drug therapy. In order to improve the diagnosis and treatment of PMBL in China, the Lymphocyte Disease Group of the Chinese Medical Association (CMA) and the Anti-Lymphoma Alliance of the Chinese Society of Clinical Oncology (CSCO) commissioned a group of ex

Bruton's tyrosine kinase inhibitors and their clinical potential in the treatment of B-cell malignancies: focus on ibrutinib.

Aberrant signaling of the B-cell receptor pathway has been linked to the development and maintenance of B-cell malignancies. Bruton's tyrosine kinase (BTK), a protein early in this pathway, has emerged as a new therapeutic target in a variety of such malignancies. Ibrutinib, the most clinically advanced small molecule inhibitor of BTK, has demonstrated impressive tolerability and activity in a range of B-cell lymphomas which led to its recent approval for relapsed mantle cell lymphoma and chronic lymphocytic leukemia. This review focuses on the preclinical and clinical development of ibrutinib

The reactivation of occult HBV infection emerging with the case of acute hepatitis B in the wife of a subject treated with rituximab-based chemotherapy.

It is still a matter of debate whether nucleoside/nucleotide analogues should be administered as prophylaxis to hepatitis B surface antigen (HBsAg)-negative/antibodies against hepatitis B core antigen (anti-HBc)-positive patients with immunosuppression or whether these patients should be closely monitored and early treatment administered to those who become HBsAg-positive. We describe a reactivation of occult HBV infection emerging with the case of acute hepatitis B in the wife of an HBsAg-negative/anti-HBc-positive subject treated with rituximab-based chemotherapy for chronic lymphocytic leuk

Mutations of the p53 gene in B-cell lymphoma.

Mutations of p53 gene have been recognized to be the most common genetic changes in human cancers. Recently, p53 gene mutations have been found in some patients with common subtypes of B-cell lymphoma (9/48:18.8%), Burkitt lymphoma (9/27:33.3%) and chronic lymphocytic leukemia (6/40:15%). Evidences to suggest that p53 gene mutations are associated with the disease progression in B-cell lymphoma have emerged. Functions of wild-type p53 and its mutant's probable role in B-cell lymphomagenesis are described in this review.

Further observations on the role of Staphylococcus aureus exotoxins and IgE in the pathogenesis of nasal polyposis.

OBJECTIVES/HYPOTHESIS: Recent studies have suggested that Staphylococcus aureus secrete exotoxins that may act as superantigens and upregulate the variable beta region of lymphocytes in chronic hyperplasticsinusitis with nasal polyposis (CHSwNP). The aim of this study was to add further information for correlating the presence of staphylococcal species and the upregulation of the V(β) region of both nasal polyp lymphocytes and peripheral blood lymphocytes. Furthermore, IgE-mediated hypersensitivity directed against these exotoxins produces an additional independent immunologic mechanism in upr

CAG repeat mutation in the mouse IL-2 gene enables concurrent assessment of T- and B-cell-specific gene expression by northern analysis.

The unique presence of CAG triplet repeats in the mouse IL-2 gene, lending the IL-2 cDNA a second specificity to MHC class II invariant chain (Ii) mRNA, coincides with another unusual genetic feature of the mouse, the lack of Ii gene upregulation in stimulated T cells. While the former anomaly allows simultaneous measurement of IL-2 and Ii mRNAs by Northern analysis, the latter condition ensures that Ii mRNA remains a T-cell-independent, specific marker of Ii positive, primarily B cell function. Thus, Northern analysis using CAG repeat containing IL-2 cDNA enables concurrent assessment of T an

[Immunoglobulin and T-cell receptor gene rearrangements in patients with autoimmune diseases].

Immunoglobulin-(Ig-) and T-cell-receptor-(TcR-)gene rearrangements were investigated in peripheral blood lymphocytes (PBL) of patients with various autoimmune disorders. In patients with SLE there was no predominant Ig- or TcR-gene rearrangement. This was also true in patients with a long disease duration and with excessive hypergammaglobulinemia. These results lead us to suggest that B cells are activated polyclonally in these patients. In those cases, where predominantly rearranged Ig- or TcR-genes were found, the autoimmune disorder was associated with a low-grade non-Hodgkin lymphoma (NHL)

B cell differentiation factor production in systemic lupus erythematosus.

Spontaneous and mitogen stimulated B cell differentiation factor (BCDF) production by peripheral blood lymphocytes (PBL) from patients with systemic lupus erythematosus (SLE) was compared with that of normal control subjects. BCDF production was measured by a cellular interleukin assay in which a BCDF responsive cell line was cocultured with increasing numbers of PBL from the donor populations. It was found that no significant differences occurred in BCDF production by SLE and normal PBL under the conditions used. Indeed there was a trend toward decreased BCDF release from SLE PBL. This would

High sensitivity of chemiluminescent methodology for detection of clonal CDR3 sequences in patients with acute lymphoblastic leukemia.

Detection of minimal residual disease (MRD) in patients with B-cell acute lymphoblastic leukemia (B-ALL) has been achieved using several radioactive labelling methodologies; however, limited information exists about the use of chemiluminescent labelling. Although many malignant disorders are related to cytogenetic alterations, there is not a consistent chromosomal translocation that could serve as a tumour marker for the monitoring of MRD. ALL are derived from B-lymphocytes in 80% of cases. In the early stages of their maturation, the immunoglobulin heavy chain genes (IgH) undergo rearrangemen

FISH analysis for BCL-1 rearrangements and trisomy 12 helps the diagnosis of atypical B cell leukaemias.

We have investigated the diagnostic value of fluorescence in situ hybridisation (FISH) to detect t(11;14) and trisomy 12 in 53 cases with a B cell leukaemia difficult to classify on clinical and laboratory grounds. These cases were initially diagnosed by morphology and immunophenotype and in 33 of them, on tissue histology, as follows: chronic lymphocytic leukaemia (CLL), 20, 18 of them with atypical features; B cell prolymphocytic leukaemia (B-PLL), two; mantle-cell lymphoma (MCL), 15; splenic lymphoma with villous lymphocytes (SLVL), five; lymphoplasmacytic lymphoma, six; follicular lymphoma

Phosphorylation of 2-chlorodeoxyadenosine (CdA) in extracts of peripheral blood mononuclear cells of leukaemic patients.

2-Chlorodeoxyadenosine (CdA) is an antileukaemic agent used in treatment of hairy cell leukaemia (HCL) and chronic lymphocytic leukaemia of B- and T-cell type (B-CLL and T-CLL). The aim of this study was to elucidate the interpatient variability of CdA phosphorylation and its relation to response to CdA treatment. In extracts of peripheral blood mononuclear cells of patients with B-CLL (n = 39), CdA phosphorylation was significantly higher than in HCL (n = 19) when calculated per protein (391 +/- 155 pmol CdA phosphorylated/mg protein/min versus 288 +/- 166 pmol/mg/min, P < 0.001), but was the

Humoral immunity in cancer.

Humoral immunity driven by B cells, plasma cells, and antibodies has emerged as a major determinant of tumor control, immune escape, and therapeutic response. Across solid tumors, mature tertiary lymphoid structures (TLS) with high endothelial venules, follicular dendritic-cell networks, and T follicular helper cell niches support germinal-center-like reactions, B-cell receptor selection, somatic hypermutation, class-switch recombination, antigen presentation, and local humoral memory. However, B-lineage responses are not uniformly protective. Regulatory B cells, IgA- or IgG4-skewed plasma cel

Advances in the treatment of systemic lupus erythematosus.

Systemic lupus erythematous is a clinically and pathogenetically heterogeneous disease that has long challenged researchers and clinicians aiming to improve its treatment. Advances over the past 75 years have revealed a number of key immune mechanisms that drive clinical manifestations, paving the way for the development of therapies that go beyond broad immunosuppression to improve clinical efficacy and reduce side effects. These include approaches aimed at specific immune pathways, and emerging efforts to restore immune homeostasis, such as chimeric antigen receptor T cell therapies to elimi
