Undersized angioplasty and stenting of symptomatic intracranial tight stenosis with Enterprise: Evaluation of clinical and vascular outcome.

BACKGROUND: Severe intracranial arterial stenosis results in more than 10% incidence of stroke and transient ischemic attack. Using undersized angioplasty with off-label closed-cell Enterprise stent may be a feasible alternative option for treating patients with intracranial atherosclerotic disease who fail dual-antiplatelet medical therapy. The results of the authors' study are presented in this paper. MATERIALS AND METHODS: Between January 2013 and July 2014, 24 symptomatic patients with a total of 30 intracranial arterial stenotic lesions refractory to medical therapy, who underwent unders

Prevalence of unruptured intracranial aneurysms in healthy asymptomatic Japanese adults: differences in gender and age.

BACKGROUND: We aimed to assess the prevalence of unruptured intracranial aneurysms (UIAs) in healthy asymptomatic adults, and investigate the differences in incidence due to gender and age in Japan. METHODS: Magnetic resonance angiography (MRA) results of healthy asymptomatic adults who underwent the procedure for examination of the brain, from April 2010 to March 2012, were retrospectively examined. Patients with a history of ruptured aneurysm and UIAs were excluded. UIAs greater than 2.0 mm in size were counted. In accordance with these criteria, 8,696 people with a mean age of 52.2±9.5 yea

Thrombolysis in patients with lacunar stroke is safe: an observational study.

Research suggests that the etiology of lacunar stroke is different from that of other stroke subtypes. This could imply an altered response to thrombolysis, but data concerning the efficacy of rt-PA in lacunar stroke is limited and inconsistent. From our prospectively collected stroke database, we identified patients with an MRI-confirmed purely lacunar stroke that were treated in our Stroke Unit between 2004 and 2011. We compared both the clinical course (NIHSS, deterioration, mRS at 3 months) and the MRI findings between patients who either received or did not receive rt-PA. In comparison to

Pial arteriovenous fistula resulting from ventriculostomy.

OBJECTIVE: Ventriculostomy complications are well documented in the literature. We report the first known example of an arteriovenous fistula created during passage of a ventriculostomy catheter for the treatment of hydrocephalus. METHODS: A 47-year-old female patient initially presented with a subarachnoid hemorrhage and an anterior communicating artery aneurysm. The patient underwent coil embolization followed by a ventriculostomy catheter for hydrocephalus. After recovery, a follow-up angiogram demonstrated a new arteriovenous fistula at the site of the ventriculostomy. A craniotomy was pe

Endovascular coil embolization of anterior choroidal artery aneurysms. Clinical article.

OBJECT: Anterior choroidal artery (AChA) aneurysms are difficult to treat, and the clinical outcome of patients is occasionally compromised by ischemic complications after clipping operations. The purpose of this study was to document the outcome and follow-up results of endovascular coil embolization in patients with AChA aneurysms. METHODS: Between July 1999 and March 2008, 88 patients with 90 AChA aneurysms (31 ruptured and 59 unruptured aneurysms) were treated with endovascular coil embolization in 91 sessions. There were 87 small aneurysms (< 10 mm) and 3 large aneurysms, with a mean ane

Delayed lumbar plexus palsy due to giant psoas hematoma associated with vertebral compression fracture and direct oral anticoagulants: a case report.

BACKGROUND: Osteoporotic vertebral compression fractures (VCFs) are commonly observed in elderly people and can be treated by conservatively with minimal risk of complications in most cases. However, utilization of direct oral anticoagulants (DOACs) increases the risks of secondary hematoma even after insignificant trauma. The use of DOACs increased over the past decade because of their approval and recommendation for both stroke prevention in non-valvular atrial fibrillation and treatment of venous thromboembolism. It is well known that DOACs are safer anticoagulants than warfarin in terms of

Development of contrast-induced nephropathy in subarachnoid hemorrhage: a single center perspective.

BACKGROUND AND PURPOSE: The use of iodinated contrast-enhanced imaging studies is increasing in acute cerebrovascular diseases, especially in subarachnoid hemorrhage (SAH). In SAH, such studies are essential for both diagnosis and treatment of the cause and sequela of hemorrhage. These patients are often subjected to multiple contrast studies such as computed tomographic angiography, computed tomographic perfusion, and cerebral angiography. They are also predisposed to intravascular volume depletion as a part of the disease process from cerebral salt wasting (CSW) and as a result of multiple c…

Carotid Stump Syndrome: A Case That Highlights the Necessity of Digital Subtraction Angiography for the Prompt Management of the Syndrome.

Background and Clinical Significance: Carotid stump syndrome (CSS) is a rare and unexpected cause of recurrent ischemic ipsilateral events in the carotid vascular territory despite the demonstrated occlusion of the internal carotid artery (ICA). It is believed to be caused by microemboli due to turbulent blood flow in the patent stump of the occluded ICA that passes through anastomotic channels and retrograde flow into the middle cerebral artery circulation. Case Presentation: We describe the case of a 65-year-old male patient who suffered multiple concurrent transient ischemic attacks (TIAs)

Hepatic necrosis and dysfunction following angioembolization in a trauma-related mortality.

BACKGROUND: Angiographic embolization is a key tool in managing hemorrhage from blunt traumatic injuries, particularly hepatic trauma, but it carries risks such as hepatic necrosis. The incidence of clinically significant hepatic necrosis post-embolization is around 6%, with non-selective embolization increasing this risk compared to selective techniques. OBJECTIVE: To report a case of a patient with high-grade blunt hepatic injury who developed hepatic necrosis and dysfunction leading to death following non-selective angiographic embolization. METHODS: A 46-year-old male involved in a high-sp

A "Mismatched" connection: a rare case of indirect or dural carotid-cavernous fistula: a case based review.

INTRODUCTION AND IMPORTANCE: A carotid-cavernous sinus fistula is an abnormal connection between the internal or external carotid artery and the venous system of the cavernous sinus. It represents a rare entity, and it is often misdiagnosed due to its overlapping symptoms with other conditions such as cavernous sinus thrombosis or orbital inflammation. Cerebral angiography continues to be the gold standard for diagnosis and surgical planning in patients with CCF, and the endovascular trans-venous approach still represents the primary line of treatment. Trans-arterial technique has become an ex…

Collateral circulation status-guided mechanical thrombectomy in pediatric stroke with an extended ghost infarct core: illustrative case.

BACKGROUND: Mechanical thrombectomy (MT) has been proved to be a highly effective therapy to treat acute ischemic stroke due to large vessel occlusion. Often, the ischemic core extent on baseline imaging is an important determinant for endovascular treatment eligibility. However, computed tomography (CT) perfusion (CTP) or diffusion-weighted imaging may overestimate the infarct core on admission and, consequently, smaller infarct lesions called "ghost infarct cores." OBSERVATIONS: A 4-year-old, previously healthy boy presented with acute-onset, right-sided weakness and aphasia. Fourteen hours

Application of intraoperative infrared thermography in bypass surgery for adult moyamoya syndrome: A preliminary study.

BACKGROUND AND OBJECTIVES: Cerebral revascularization surgery is the mainstay of treatment for moyamoya syndrome (MMS) today, and intraoperative determination of the patency of the revascularized vessel is a critical factor in the success of the procedure. Currently, major imaging modalities include intraoperative indocyanine green (ICG) videoangiography (ICG-VA), digital subtraction angiography (DSA), and vascular ultrasound Doppler. Infrared thermography is a modern imaging modality with non-contact devices for the acquisition and analysis of thermal data. We aimed to investigate the feasibi
