Coronary-to-pulmonary artery collateral in tetralogy of Fallot.

Angiography in a 14-year-old boy with cyanosis since birth confirmed the diagnosis of tetralogy of Fallot with a subaortic ventricular septal defect, large overriding aorta, severe pulmonary stenosis, and a large collateral arising from the left circumflex artery. The collateral was isolated and ligated at its origin, and the patient underwent an uneventful repair with ventricular septal defect patch closure, infundibular resection, pulmonary valvotomy, and right ventricular outflow tract reconstruction with an autologous pericardial patch.

Pulmonary stenosis development and reduction of pulmonary arterial hypertension in atrioventricular septal defect: a case report.

A 24-year-old patient was admitted for dyspnoea and syncope. He had a previous history of complete atrio-ventricular septal defect and trisomy 21. At the age of 6 months, in 1984, cardiac catheterization revealed a quasi-systemic pulmonary arterial hypertension with a bidirectional shunt corresponding to an Eisenmenger syndrome. Corrective cardiac surgery was not performed at this time because surgical risk was considered too high. Until the age of 20 years old, he showed few symptoms while under medical treatment. But since 2006, his functional status became worse with an increased dyspnoea,

Aortic translocation, Senning procedure and right ventricular outflow tract augmentation for congenitally corrected transposition, ventricular septal defect and pulmonary stenosis.

The management of congenitally corrected transposition of the great arteries and associated lesions is frequently challenging. Significant pulmonary stenosis is a contraindication to the conventional double-switch. Instead repair may be accomplished by the Rastelli-Senning procedure, using an extracardiac conduit to achieve continuity between the morphological left ventricle and the pulmonary arteries. This however can be accompanied by conduit and intra-ventricular baffle-related problems that can necessitate surgical re-intervention and lead to late mortality. We describe the use of aortic t

Invasive measurements of atrioventricular conduction parameters prior to and following ventricular septal defect closure with the amplatzer device.

BACKGROUND: Because of anatomical proximity, closure of paramembranous ventricular septal defect (pm-VSD) with an Amplatzer occluder has the potential to affect the atrioventricular (AV) conduction system. PATIENTS AND METHODS: Electrophysiology studies (EPS) were performed prior to and following closure of the pm-VSD in 19 children aged 8.9 +/- 4.5 years. Data from electrocardiograms, Holter monitoring and exercise tests before intervention and at follow up (1.9 +/- 0.8 years) were reviewed. RESULTS: There were no EPS catheter-induced complications. Incidence of total conduction anomalies w

Device fracture and severe tricuspid regurgitation after percutaneous closure of perimembranous ventricular septal defect: a case report.

We report a case in which fracture of an Amplatzer perimembranous ventricular septal defect (VSD) device occurred within 1 month after placement. This was associated with severe tricuspid regurgitation. Surgical removal of the device and repair of the tricuspid valve was performed with reasonable outcome. We propose a hypothesis on how this complication might have occurred and how it could be prevented.

A case of hemolysis after percutaneous ventricular septal defect closure with a device.

Hemolytic anemia related to intracardiac prosthetic materials has been described. We describe a case of a 74-year-old female with a history of hypertrophic obstructive cardiomyopathy that had a postoperative ventricular septal defect closed with a muscular ventricular septal defect occluder. The device caused intractable hemolytic anemia that required surgical removal and repair. We describe a rare cause of hemolysis complicating transcatheter closure of a post-traumatic ventricular septal defect.

Surgical vs. percutaneous atrial septal defect closure: Remote effects on left ventricular diastolic function.

While surgical closure used to be the only treatment for atrial septal defects (ASD), recently, placement of an occluding device has become the treatment of choice for this condition. We sought to study the remote effects of surgical vs. device ASD closure on the indices of left ventricular diastolic function. Forty patients--16 after device and 19 after surgical closure--were evaluated at least 1 year after the procedure. Mitral inflow and tissue Doppler indices were studied. The results were compared to those of 65 control subjects. The early mitral inflow velocities E were higher in control

Severe tricuspid stenosis during percutaneous occlusion of perimembranous ventricular septal defect with the new Amplatzer device.

BACKGROUND: The new Amplatzer device has been used with success for occlusion of the perimembranous ventricular septal defects and the index of complications is very low. RESULTS: The reported case corresponds to a very rare severe acute tricuspid stenosis, caused by adherence of the prosthesis to the anterior tricuspid leaflet. CONCLUSION: Device release was achieved by manipulation on the tricuspid apparatus.

Aortic valve prolapse associated with outlet-type ventricular septal defect.

BACKGROUND: Aortic valve prolapse is frequently associated with juxta-arterial ventricular septal defect. The significance of its association with other outlet types of ventricular septal defect, however, remains unclear. METHODS: From 1987 to 2002, 677 patients (male:female ratio, 424:253) who received surgical repair for ventricular septal defect extending to the outlet septum were reviewed. Based on surgical findings, ventricular septal defects were classified as juxta-arterial, perimembranous outlet, or muscular outlet type. RESULTS: Aortic valve prolapse occurred in 373 of 677 patients

Spontaneous closure of ventricular septal defects followed up from <3 months of age.

BACKGROUND: This study evaluates the incidence and timing of spontaneous closure (SC) of ventricular septal defect (VSD) using Doppler color flow mapping. METHODS: A total of 225 infants (mean age 30 days) were diagnosed with uncomplicated VSD: 31 (14%) subpulmonary VSD, 159 (70%) perimembranous, and 35 (16%) muscular. The patients were divided into two groups according to the presence or absence of congestive heart failure (CHF). SC was confirmed with color Doppler. RESULTS: Surgical closure was performed in 59 patients (26%). SC occurred in 107 patients (48%); three (10%) of 31 with subpul

Transcatheter closure of perimembranous ventricular septal defects.

The isolated perimembranous ventricular septal defect is one of the most common congenital cardiac malformations. Although surgery has been performed and has a low associated risk, it still involves morbidity due to factors such as residual leaks, atrioventricular block, postpericardiotomy syndrome and arrhythmias. It has been speculated that percutaneous closure of these defects could minimize such complications. Recently, a device designed specifically for perimembranous ventricular septal defect closure, the Amplatzer membranous ventricular septal defect occluder, has been developed. Initia

Multiple pulmonary artery aneurysms due to infective endocarditis.

Bilateral pulmonary artery aneurysms developed in the course of staphylococcal endocarditis in a 6-year-old girl with ventricular septal defect. Consecutive computed tomography scans revealed the progressive enlargement of one of the aneurysms. She underwent an urgent left upper lobectomy because of the impending rupture and a possible life-threatening hemorrhage. The second ipsilateral aneurysm was plicated in order to exclude the aneurysm sac. In the next operation the ventricular septal defect was closed and vegetations located on the tricuspid valve were removed. On follow-up spontaneous t

Transcatheter closure of perimembranous ventricular septal defects with the Amplatzer asymmetric ventricular septal defect occluder: preliminary experience in children.

OBJECTIVE: To close perimembranous ventricular septal defects (PMVSDs) in children with the new Amplatzer asymmetric ventricular septal defect occluder (AAVSDO). PATIENTS AND DESIGN: 10 children, aged 1.5-12 years, with PMVSDs underwent transcatheter closure with the AAVSDO. The device consists of two low profile disks made of Nitinol wire mesh with a 1.5 mm connecting waist. The left disk is 5 mm towards the apex and only 0.5 mm towards the aortic valve. The right disk is 4 mm larger than the waist. The prosthesis diameter was chosen to be 1-2 mm larger than the largest diameter of the defec

Successful trapdoor technique for two separate coronary ostia in a single aortic sinus in arterial switch operation for complete transposition of the great arteries.

Two separate origins of coronary arteries from a single aortic sinus remains a rare but significant congenital anomaly for surgical repair. We experienced a case of a newborn with complete transposition of the great arteries with unusual coronary arteries with two separate ostia positioned close to each other in the same right aortic sinus, and the left coronary artery coursing behind the pulmonary artery. No description of this case in the English literature has been published to our knowledge. Precise anatomical diagnosis was made at operation, and these anomalous coronaries were successfull

Bronchomalacia associated with pulmonary atresia, ventricular septal defect and major aortopulmonary collateral arteries, and chromosome 22q11.2 deletion.

Respiratory distress is one of the major complications in young infants with pulmonary atresia, ventricular septal defect and major aortopulmonary collateral arteries (PA-VSD-MAPCA); however, its aetiology remains obscure. We have previously reported an association of bronchomalacia with PA-VSD-MAPCA in patients with a hemizygous deletion of chromosome 22q11.2 (del.22q11). To clarify the clinical relevance of bronchomalacia in patients with PA-VSD-MAPCA and del.22q11, we reviewed the clinical and laboratory records of four patients with PA-VSD-MAPCA who had del.22q11 and bronchomalacia. Intermediate results of the anatomic repair for congenitally corrected transposition.

BACKGROUND: Anatomic repair of congenitally corrected transposition of the great arteries has several advantages over the traditional approach but lacks long-term evaluation. METHODS: The data on 12 patients who had the procedure between January 1989 and June 2000 were retrospectively reviewed. Associated lesions included ventricular septal defect in 12 patients, pulmonary stenosis in 10 patients, and moderate to severe tricuspid valve regurgitation in 4 patients. Mean age at operation was 9+/-3.6 months. All patients had venous switch Mustard procedure. Tunneling of the morphologic left vent

Incidence of spontaneous closure of outlet ventricular septal defect.

The present study examined the Doppler echocardiograms of 742 patients with an outlet ventricular septal defect (VSD) who were seen at the National Cardiovascular Center from 1987 to 1999. Of these, 450 patients were followed for longer than 1 year; 161 of these 450 were younger than 12 months when first examined. During follow-up, Doppler echocardiographic examination was repeated at least once a year. The medical charts of these patients were reviewed. Doppler echocardiographic findings were analysed in patients whose defect closed spontaneously:spontaneous closure at the age of 1-25 years w

Transcatheter closure of membranous ventricular septal defects with a new nitinol prosthesis in a natural swine model.

Transcatheter closure of a membranous ventricular septal defect (MVSD) is much more difficult than closure of other intracardiac defects because of the proximity to the aortic and tricuspid valves and their relatively large size in small children. In this report, transcatheter closure of naturally occurring membranous VSDs was attempted in 12 Yucatan minipigs. The prosthesis is constructed from fine Nitinol wires in the shape of two buttons and a connecting waist filled with polyester fiber. Two kinds of prosthesis were used in this study: concentric and eccentric left-sided retention disks. A
