Preliminary report: the distally pedicled dorsoulnar forearm flap for hand reconstruction.

In this paper, a new flap for the coverage of soft tissue defects in the hand is described. To obtain a distally pedicled dorsoulnar flap, the dissection of the standard dorsoulnar flap is continued distally under the descending branch of the dorsal branch of the ulnar artery onto the dorsum of the wrist after the dorsal branch of the ulnar artery is ligated and divided. Our modification of the standard dorsoulnar flap converts this flap to a distally based flap, which provides a potentially longer pedicle and increases the arc of rotation of the flap. Two successful cases are reported.

Strategies in multiple toe transplantation for bilateral type II metacarpal hand reconstruction.

Multiple toe transplantation has been established as an effective means of restoring prehensile function in the metacarpal hand. The attainable outcome, however, must be weighed against potential donor site morbidity. Restoration of prehensile function in bilateral type II metacarpal hands can be achieved by reconstructing three opposable digits in the dominant hand and two opposable digits in the nondominant hand. This article outlines the strategy for optimizing functional outcome and discusses the rationale behind toe selection to reduce donor morbidity.

Complete primary closure of bladder exstrophy. Epispadias and bladder exstrophy repair.

Bladder exstrophy remains one of the most challenging problems in pediatric urology. Recent efforts have focused on primary reconstruction rather than urinary diversion to treat exstrophy. Complete primary closure appears to offer improved continence and decreases the number of surgical procedures required to treat exstrophy.

One size fits all: a surgical technique for the correction of all types of prominent ears.

Patient satisfaction after surgery to correct the prominent ear relates to the degree of retroposition of the ear. Angular breaks in the cartilage seen with other cartilage-transecting procedures (and even after conchal resections), which are noticed by patients, led to the development of this technique. It is an easy operative technique that may be modified to suit all prominent ears, including the constricted cup ear, and it does not leave any sharp, visible ridges in the region of the anti-helix or concha. The procedure combines and modifies a number of techniques that have been described p

Prevention and treatment of wide scar and alopecia in the scalp: wedge excision and double relaxation suture.

The visible linear scar of the scalp is a cosmetically serious complication of a scalp incision in scalp surgery, forehead lift, and craniofacial surgery, especially on the temporal scalp. Its causes are cicatrical alopecia and scar widening. To solve this problem, we performed the wedge excision of the scalp and the double relaxation suture of the galea in 2 patients undergoing facial surgery through the coronal approach and in 15 patients with scalp alopecia ranging from 0.5 to 3.0 cm in width. The wedge excision using the beveling incision at an angle of 30 degrees to the hair follicles pre

Soft-palate reconstruction with a "SCARF" superior-constrictor advancement-rotation flap.

Reconstruction of hemi-soft-palate defects after tumor resection is usually done by means of a regional flap, free-tissue transfer or a prosthesis. These options vary in complexity and have a number of shortcomings. A local myomucosal flap was designed that employs a superior-constrictor advancement-rotation flap (SCARF) to achieve circumferential closure of the velopharynx and to re-establish its valvular sphincteric function. Ten patients underwent a SCARF reconstruction of the velopharynx after 35% to 65% of the soft palate was resected. All patients re-established normal velopharyngeal fun

Free transverse colon transplantation for functional reconstruction of intra-oral lining: a clinical and histologic study.

A variety of free-tissue transplantations are available for oropharyngeal (lining) reconstruction. The most commonly used flap, the radial forearm flap, is relatively thin, adaptable, and reliable but does not provide lubrication. In this study, the potential of the transverse colon to provide a lubricating tissue for replacement of oral lining was investigated. Eleven patients requiring replacement of oral lining received transverse colon flap transplantation between November of 1993 and December of 1995. There was one complete flap loss and one partial loss. Flap size used for reconstruction

Congenital partial absence of the facial muscles.

Facial asymmetry in a newborn infant suggests various diagnoses, including birth trauma and congenital hemifacial microsomia. But congenital absence or hypoplasia of facial muscles has not been known except for the depressor anguli oris muscle (DAOM). The authors' experience with surgical treatment of congenital hypoplasia of the risorius, zygomaticus major and minor, and levator labii superioris muscles is presented. Congenital facial muscle absence is uncommon. Although the hypoplasia of the DAOM is known, this disease is not fully clarified. The authors report a case of congenital partial p

Two reconstructive techniques for flatfoot deformity comparing contact characteristics of the hindfoot joints.

The effect of two different methods of reconstruction of flatfoot deformity and the role of the posterior tibial tendon on the contact characteristics of the hindfoot joints were quantified using pressure-sensitive film. Each of 10 cadaver feet was loaded quasi-statically by an axial compressive force to simulate varying loads. First, a specimen was tested intact, then it was tested after sectioning the spring ligament and loading the specimen cyclically to create one type of flatfoot deformity. It was then tested again after reconstructing the deformity. Reconstructions used were the Dillwyn-

Construction of continent catheterizable urinary conduit from an isolated segment of colon.

BACKGROUND: Continent urinary diversion may be necessary in a range of urological abnormalities. In circumstances where the standard techniques are not possible, alternative innovative techniques may be used. METHODS: In a female patient with bladder exstrophy, a continent diversion was recommended. The appendix was not available, the ureters were not suitable and a continent stoma was fashioned from an isolated segment of colon. RESULTS: The stoma proved to be continent, although it was somewhat stenotic. However, clean intermittent catheterization maintained its patency. CONCLUSIONS: A co

Late toe-to-hand transfer for the reconstruction of congenital defects of the long fingers.

Toe-to-hand transfer is a well established procedure and is one of the best ways of reconstructing the missing fingers or parts of fingers. In congenital deformities of fingers and hands such as absence of digits, second toe transfer may create the grip function and is superior to all other procedures. We present three cases of four second toe transfer to reconstruct long finger defects after congenital malformations of the long fingers. The aim of this paper is to discuss some aspects of the procedure and to evaluate achieved results. All our four patients were operated on when they were over

Intrascar excision for persistent perioral hypertrophic scar.

Scar revision is almost a daily work of plastic surgeons, but sometimes it is a challenge to them. Persistent hypertrophic scar in the perioral area is very difficult to treat because of its anatomic location. Many methods of scar removal, including simple scar revision, Z- or W-plasty, V-Y or Y-V flap, local cutaneous flaps, etc., may not be suitable to remove the perioral scar. The specially designed intrascar excision technique is a good method to solve this problem. The long-term follow-up, up to 5 years, in this clinical series revealed that this method does not distort the perioral anato

Treatment of the facial angiofibromas of tuberous sclerosis.

Tuberous sclerosis is an inherited syndrome with mental retardation, epilepsy, and acne rosacea being the classic triad of manifestations. The facial skin problems can present a cosmetic deformity or, as in the three patients presented here, can produce difficulties with hygiene and nasal breathing. Various approaches to treatment have been described over the last century, but no long-term results have been presented. Because of the full-thickness skin involvement, complete removal is not practical, and treatment should be directed toward acceptable long-term results with minimal surgical morb

[Mid-plantar pedicle skin flap. Apropos of 5 clinical cases].

The blood supply to the non-weight bearing midsole area is not derived exclusively from the medial plantar artery. This allows elevation of a subfascial mid-plantar cutaneous flap for the reconstruction of the heel. The technique of elevation of the flap is described. This plantar flap can be designed to include sensation without the need for deep dissection. Five clinical cases are discussed. This flap is useful for medium-sized chronic ulcerations of the heel.

[Passerini-Glazel technique of vulvovaginoplasty: a solution for upper outlet of the vagina].

Since 1988, we have performed the Passerini-Glazel technique, in 5 patients, between 7 months and 3 years old. All showed female pseudohermaphroditism with severe signs of masculinization, and consequently, high outlet of the vagina in the urethra, secondary to Congenital Adrenal Hyperplasia (CAH). The technique use is to get a functional and esthetically acceptable vagina at the expense of a urethral flap and two flaps of skin, coming from the hypertrophic clitoris. The results were excellent in all the patients. In one occasion it was necessary to insert a cylinder of vesical mucous to get a

[Multiple free transplants in microsurgery for cervicofacial reconstruction].

Microsurgical techniques open the way for much creativity and originality for cervicofacial reconstruction and tumoral repair. Between 1976 and 1994, we performed 481 free transplants for cervico-facial reconstruction. A recently developed strategy has been the use of multiple free transplants, either simultaneously or successively. Between 1990 and 1994, we operated and reconstructed 9 patients using multiple free cervico-cephalic transplants after multitissular and agressive exeresis for tumoral or traumatic lesions. In 3 cases, the transplants were made simultaneously and in the 6 others su

A new design for the radial forearm free-flap phallic construction.

One-stage phallic construction using the radial forearm free flap was originally reported in 1984. This potentially sensate, thin, minimally hair-bearing flap, whose design incorporated a vascularized urethra, provided significant advantages over previously described multistaged insensate phallic reconstructions. The original design, as described by Chang and Hwang, is limited in size and has the propensity to develop meatal stenosis. This article describes a new design of the radial forearm flap for phallic construction performed on four patients. The techniques described incorporates a centr
