Lysine lactylation represents a novel post-translational modification (PTM) involved in cellular functions including glycolysis and macropha … actylation's involvement in prevalent eye diseases, including myopia, retinopathy, ocular melanoma, uveitis, and macular degeneration. We further collate emerging data suggesting lactylation signalling pathways may represent potential therapeutic targets for ocular pathologies. This review aims to provide a comprehensive overview for holistic intervention …

Anti-TNF-α therapies, though known for their anti-inflammatory effects, have been associated with paradoxical inflammatory reactions, of which anterior uveitis is the most common. We report a case of recurrent bilateral cicatricial conjunctivitis linked to systemic adalimumab (ADA) treatment in a 66-year-old man with rheumatoid arthritis (RA). The patient experienced worsening bilateral red eyes, blurry vision, and discomfort following each ADA injection. Examination revealed signs of cicatricial inflammation, which responded to corticosteroids but recurred after subsequent ADA doses. Despite …

A previously healthy 22-year-old female presented with complaints of fatigue, joint pain, headache, fever, and night sweats for several months. She also reported a reddish painful left eye and a rash on her legs 2 months ago. On examination, her blood pressure was elevated at 160/100 mmHg and her lower limb pulses were hardly palpable. On investigation, she had anterior and intermediate uveitis, nasal septal ulcer, proteinuria, positive cytoplasmic anti-neutrophil cytoplasmic antibodies (C-ANCA), and a computed tomography angiography showed multiple abdominal aortic aneurysms and occlusion of …

This report describes three cases of iris retraction syndrome (IRS) associated with long-standing rhegmatogenous retinal detachment (RRD), all presenting with hypotony, abnormally deep anterior chambers, and ciliary body detachment, often accompanied by dense cataracts and uveitis-like findings that initially obscured the underlying pathology. In each case, anti-inflammatory therapy led to partial resolution of anterior segment abnormalities, subsequently revealing RRD that required pars plana vitrectomy with or without combined cataract surgery. Postoperatively, most cases maintained stable I…

Ocular tuberculosis in children poses significant visual risks and exhibits distinct characteristics compared to adults, necessitating careful diagnosis and management. This case illustration presents a 10-year-old girl with intermittent redness in her left eye. Despite initial treatment, her condition worsened, leading to blurred vision. She was diagnosed with ocular tuberculosis complicated by phlyctenular conjunctivitis, interstitial keratitis, diffuse anterior scleritis, anterior uveitis, and neuroretinitis. Antituberculosis therapy was initiated alongside corticosteroids and neuroprotecti…

A 14-year-old girl presented to the ophthalmology clinic with progressive diminution of vision, redness, pain and photophobia in both eyes for the last 1 month. She had abdominal pain, diarrhoea and weight loss during that period. Ocular examination revealed features of anterior uveitis, vitritis and retinal vasculitis. In view of gastrointestinal symptoms, abdominal imaging was done, which showed multiple enhancing bowel wall thickening with skip lesions in the terminal ileum and ascending colon. Colonoscopy showed ulcers in the ascending colon, caecum and terminal ileum. Histopathology revea…

Blau syndrome (BS) is a rare autosomal dominant familial granulomatous inflammatory disease presenting in early childhood with dermatitis, arthritis and uveitis. Early-onset sarcoidosis represents the sporadic form, and both are characterised by mutations in the CARD15/NOD2 gene on chromosome 16. We describe a 38-year-old man with known BS who presented for orthopaedic review following right-sided patellar dislocation. MRI of the injured knee demonstrated diffuse synovitis and prominent fatty tissue resembling lipoma arborescens with evidence of recent patellar dislocation. Synovectomy was per…

Ocular tuberculosis can manifest in a wide variety of clinical presentations. The prevalence is higher in endemic areas as a cause of granulomatous uveitis. While posterior segment manifestations are well known, anterior segment granulomas alone are relatively rare. We report two cases of unilateral iris granulomata in two young patients who presented with decreased vision and redness and were found to have well-circumscribed iris granulomas. Both underwent systemic evaluation and had a negative Mantoux test. Biopsy pathology of the lesions revealed granulomatous inflammation but were negative…

Adult T-cell Leukemia/Lymphoma (ATLL) is a sight- and life-threatening complication of human T-cell lymphotropic virus type 1 (HTLV-1) infection. Ophthalmic manifestations include uveitis, optic nerve oedema, retinal vasculitis, and lymphomatous infiltration. Orbital lesions are rare. We report the case of an orbital tumor revealing systemic ATLL in a 45-year-old Dominican patient who died despite treatment. Apart from late-grade cutaneous T-cell lymphoma, ATLL is the only T-lymphoma to develop in the orbit. Diagnosis is based on serologic evidence of HTLV-1 infection, cytology, and blood samp…

CLINICAL INTRODUCTION: A 56-year-old female with adult-onset asthma was admitted to the cardiology service with intermittent left-sided chest pain and progressive dyspnoea. Twelve months prior to this admission, she had received a course of prednisolone for bilateral anterior uveitis. Physical examination was unremarkable with blood sampling revealing a marked eosinophilia (eosinophil count 17.3×109/L) and a perinuclear antineutrophil cytoplasmic antibody staining pattern on indirect immunofluorescence microscopy (myeloperoxidase antibodies 83 IU/mL). ECG demonstrated anterolateral T-wave inve…

Patients with juvenile idiopathic arthritis (JIA) may need further care in the adult clinic as this disease frequently has continuous inflam … ide (CCP) (p = 0.05). No differences were observed in JIA form, presence of rheumatoid factor (RF), uveitis, and gender. Anti-CCP and older age at disease onset may identify pediatric JIA patients that will need further care in the adult clinic.

BACKGROUND: Vogt-Koyanagi-Harada (VKH) disease is characterized by bilateral granulomatous uveitis with neurologic, auditory, and dermatologic manifestations. However, acute myelitis complicating VKH disease has rarely been reported.
CASE REPORT: A 50-year-old Chinese Han woman presented with difficulty walking, numbness on the left side of the body, and difficulty with urination. The patient was diagnosed with incomplete VKH disease and received corticosteroid treatment prior to the neurological presentation. Acute myelitis was diagnosed based on both clinical and spinal-cord MRI findings.

Small animal patients with endocrinopathies are at risk of developing many ophthalmic conditions resulting from endocrine hormone imbalances. Diabetic animals frequently develop cataracts but can also have numerous other ocular problems, including uveitis, keratopathy, retinopathy, and the effects of lipid derangements and systemic hypertension. Cushing's patients can develop complications from hyperlipidemia and hypertension and sometimes present with corneal disease. Acute blindness from sudden acquired retinal degeneration has been associated with disease of the pituitary-adrenal axis. Grow…
