Presumably entomophthoramycosis in an HIV-infected patient: the first in Thailand.

The authors reported the case of a symptomatic HIV-infected woman with a slowly progressive infiltrative lesion which invaded in and around the nasal cavity over a 6-month period. Physical examination showed erythematous to violaceous plaques at the nasal and malar areas. Swelling of the inferior turbinate was noted in the right nare. Skin biopsy of the involved area revealed multiple nonseptate, broad, thin-walled hyphae within giant cells and granulomata. Entomophthoramycosis was diagnosed based on clinical features and histopathology. She was treated with intravenous amphotericin B for two

[Pneumothorax in distomatosis].

INTRODUCTION: Distomatosis is a parasite infection following ingestion of food contaminated by Fasciola hépatica, the tropism of which is essentially hepatic. The mechanism of this exceptional complication is discussed in two cases of pneumothorax. OBSERVATIONS: Two 35 and 60 year-old patients presenting spontaneous pneumothorax during distomatosis, the first revealing the disease, the second suggesting the evolution of a severe form of the disease. The surgical biopsy in the first patient revealed a granulomatous pleuro-pneumopathy rich in giant cells. DISCUSSION: Pulmonary manifestations o

Fine needle aspiration biopsy of calcinosis cutis. A case report.

BACKGROUND: Metastatic calcifications are currently an uncommon complication in patients with end-stage renal disease due to improvements in management of these patients. When present, however, calcifications may mimic neoplastic growth and can undergo fine needle aspiration biopsy (FNAB). CASE: A case of calcinosis cutis was diagnosed by FNAB in a 50-year-old male with a history of end-stage renal disease who presented with a subcutaneous nodule in the right side of his neck. CONCLUSION: The presence of histiocytes, foreign body-type giant cells and refractile material (calcium crystals) in

Microglial nodule encephalitis: limited CNS infection despite disseminated systemic cryptococcosis.

A 28-year-old patient with AIDS was found at autopsy to have disseminated cryptococcal infection involving the lungs, spleen, lymph nodes, kidneys, gastrointestinal tract, thyroid, bone marrow, and liver. Despite widespread organ dissemination the patient did not have clinical or pathological evidence of meningitis. Microscopic examination of the brain showed cryptococci limited to the brainstem and basal ganglia; microglial nodules with multinucleate giant cells, a histological hallmark of HIV-1 encephalitis, were shown to contain Cryptococcus neoformans. This feature may suggest a form of sy

Histologic evaluation of chronic human wounds treated with hydrocolloid and nonhydrocolloid dressings.

BACKGROUND: Studies of acute surgical wounds in pigs have shown granulomatous tissue reactions induced by hydrocolloid dressings. OBJECTIVE: We examined biopsy specimens from chronic wounds in humans treated with hydrocolloid or nonhydrocolloid dressings to determine whether such reactions occurred. METHODS: A blinded retrospective analysis of 22 chronic ulcer biopsy specimens from 22 patients was performed by two dermatopathologists independently. RESULTS: No differences were seen between hydrocolloid-treated and non-hydrocolloid-treated wounds. Granulation tissue was seen in all specimens

[Histological study of the resorbed dental crowns of impacted wisdom teeth].

Undecalcified sections of 31 impacted third molars are examined histologically for signs of resorption. Resorption starts at small spots in enamel, progresses to dentin and may even penetrate the pulp cavity. The incidence of such resorption processes increases after the age fifty. A prerequisite for resorption is the loss of reduced ameloblastic epithelium. Histological examination reveals multinuclear giant cells and apposition of spongious bone to destroyed dental substances. The results is dental ankylosis. Because partially resorbed teeth or ankylosed teeth may lead to serious surgical di

Note on Onchocerca armillata in the Sudanese camel (C. dromedarius). A histological and anatomo-pathological approach.

Aortic onchocercosis due to O. armillata was diagnosed in 45 (41%) out of 109 Sudanese camels. Although the thoracic aorta was regularly affected, involvement of other vessels such as the abdominal aorta, brachiocephalic, pulmonary and costocervical arteries was not excluded. Gross examination of the affected blood vessels revealed tortuous tunnels of parasitic tracks readily visible in the intimal surface of the vessels. Nodular lesions were more common on the adventitia. Microscopically, the lesions showed irregulary elevated intimal surfaces. The underlying tunica media contained a varying

Failed PC-IOL implantation in one eye and successful AC-IOL implantation in the other eye in a patient with pre-existing bilateral uveitis.

An irregular proteinaceous film populated by sessile macrophages and large giant cells was found on the surface of a posterior chamber lens implant. This had to be removed from a position with its haptics in the capsular bag 8 months after surgery because of increased uveitis. The implant had been placed at the time of extracapsular cataract extraction in a patient with pre-existing active bilateral chronic iridocyclitis of a non-granulomatous type. An anterior chamber lens implant placed after intracapsular cataract extraction in the other eye of the same patient was tolerated, and the same t

Wegener's granulomatosis in a 7 year old child.

A 7 year old boy presented with a history of recurrent episodes of sinusitis and otitis. He developed paresis of the external muscle of his right eye with exophthalmos. A biopsy of his sinus mucosa demonstrated necrotizing granulomatous involvement of the membranes with the presence of multiple giant cells interspersed in zones of fibrosis. A diagnosis of Wegener's granulomatosis was entertained. However the absence of pulmonary and renal manifestations did not allow for a definitive diagnosis to be made. He later developed hematuria and a renal biopsy demonstrated a renal lesions consistent w

Malignant melanoma with melanosis. Ultrastructural and histological studies.

Ultrastructural and histological investigations were performed on a case of generalized melanosis associated with superficial spreading melanoma. The hyperpigmentation of the general body surface, mucous membranes and nail beds was associated with deposition of melanin in macrophages in the dermis, together with some hyperactivity of epidermal melanocytes. Melanin granules were observed lying free in the stroma, suggesting pigment incontinence and phagocytosis by macrophages. Giant melanosomes were noted in melanocytes, keratinocytes and melanophages in the hyperpigmented skin. No evidence was

HIV-1 cell-to-cell infection of macrophages escapes type I interferon and host restriction factors, and is resistant to antiretroviral drugs.

HIV-1-infected macrophages participate in viral transmission, dissemination, and establishment of tissue virus reservoirs. Despite counteracting viral proteins (Vif, Vpu, Vpr and Nef), cell-free virus macrophage infection is restricted by host cell factors, including those induced by interferons. Here, we show that these viral proteins and type I interferon do not influence HIV-1 cell-to-cell transfer to macrophages by cell-cell fusion with infected T cells, still leading to the formation of multinucleated giant cells (MGCs). Accordingly, depletion of SERINC5 and APOBEC3G do not alter virus sp

A novel luciferase-based assay for quantifying coronavirus-induced syncytia.

Coronaviruses can induce cell‒cell fusion that results in the formation of multinucleated syncytia through the interaction of viral spike proteins with host cell receptors. Quantifying syncytial formation is crucial for screening potential efficacious antiviral compounds. However, some traditional methods for syncytial quantification are often labor-intensive and limited by a low-throughput capacity. Therefore, we developed a novel high-throughput assay for the efficient quantification of syncytial formation induced by feline coronavirus (FCoV) and SARS-CoV-2. This assay, which is based on the

Immune checkpoints in autoimmune vasculitis.

Giant cell arteritis (GCA) is a prototypic autoimmune disease with a highly selective tissue tropism for medium and large arteries. Extravascular GCA manifests with intense systemic inflammation and polymyalgia rheumatica; vascular GCA results in vessel wall damage and stenosis, causing tissue ischemia. Typical granulomatous infiltrates in affected arteries are composed of CD4+ T cells and hyperactivated macrophages, signifying the involvement of the innate and adaptive immune system. Lesional CD4+ T cells undergo antigen-dependent clonal expansion, but antigen-nonspecific pathways ultimately

Development of a simple standardized scoring system for assessing large vessel vasculitis by 18F-FDG PET-CT and differentiation from atherosclerosis.

PURPOSE: This study is to develop a structured approach to distinguishing large-artery vasculitis from atherosclerosis using 18-fluorodeoxyglucose positron emission tomography combined with low-dose computed tomography (FDG PET/CT). METHODS: FDG PET/CT images of 60 patients were evaluated, 30 having biopsy-proven giant cell arteritis (GCA; the most common form of large-artery vasculitis), and 30 with severe atherosclerosis. Images were evaluated by 12 nuclear medicine physicians using 5 criteria: FDG uptake pattern (intensity, distribution, circularity), the degree of calcification, and co-lo

Effect of lodoxamide on tear leukotriene levels in giant papillary conjunctivitis associated with ocular prosthesis.

Leukotrienes have been shown to play a role in the pathogenesis of ocular inflammatory and allergic reactions like vernal keratoconjunctivitis and contact lens-associated giant papillary conjunctivitis. This study was designed to determine leukotriene B4 (LTB4) and leukotriene C4 (LTC4) levels in the tears of patients with ocular prosthesis-associated giant papillary conjunctivitis (OP-GPC) and to evaluate the effects of lodoxamide 0.1% on tear LTB4 and LTC4 levels of OP-GPC patients. Tear LTB4 and LTC4 levels were determined by an ELISA technique in the tears of ten OP-GPC patients before and

Parvovirus B19 as a cause of acquired chronic pure red cell aplasia.

Parvovirus B19 infection causes chronic anaemia in immunodeficient individuals by selective suppression of erythropoiesis. The bone marrow morphology is characteristic of pure red cell aplasia (PRCA). To determine the frequency of B19-induced PRCA we retrospectively analysed a series of 57 PRCA patients. B19 DNA was present in serum of eight patients (14%) and could be extracted from bone marrow aspirate slides from five of these patients. Recent exposure to the virus was confirmed by the presence of anti-B19 IgM in sera from four and by the finding of giant pronormoblasts in marrow aspirates

[Incidence of anaplastic tumor in structure of other histologic forms of the thyroid gland cancer].

The degrees of invasiveness, proliferative activity, morphofunctional activity of nuclei in the thyroidal gland tumors were studied, while analyzing material, obtained in 1343 patients, suffering thyroidal gland cancer (THGC) and operated on in 2000-2013 yrs. Morphological point quantity of malignancy (as a criterion of the tumor progression grade) and mitotic activity in cellular population were determined in various kinds of THGC. Undifferentiated (anaplastic carcinoma) type of THGC is the most malignant one. There were determined a spindle-like, giant-cell and squamous-cell forms of undiffe

Acute parvovirus B19 infection mimicking myelodysplastic syndrome of the bone marrow.

A 36-year-old, previously healthy woman was referred to our institution with pancytopenia and splenomegaly for suspected acute leukemia. Bone marrow aspiration showed marked dysplastic changes, excess of blasts, and only spurious red blood cell precursors. Action was taken to prepare allogeneic bone marrow transplantation from an HLA identical sibling for myelodysplastic syndrome. Repeat cytological examination of the bone marrow revealed striking hyperplasia of the red cell line with presence of abnormal giant proerythroblasts. Acute parvovirus B19 infection was suspected and confirmed by det

CD4+CD45RBHi interleukin-4 defective T cells elicit antral gastritis and duodenitis.

We have analyzed the gastrointestinal inflammation which develops following adoptive transfer of IL-4 gene knockout (IL-4(-/-)) CD4(+)CD45RB(Hi) (RB(Hi)) T cells to severe combined immunodeficient (SCID) or to T cell-deficient, T cell receptor beta and delta double knockout (TCR(-/-)) mice. Transfer of IL-4(-/-) RB(Hi) T cells induced a similar type of colitis to that seen in SCID or TCR(-/-) recipients of wild-type (wt) RB(Hi) T cells as reported previously. Interestingly, transfer of both wt and IL-4(-/-) RB(Hi) T cells to TCR(-/-) but not to SCID mice induced inflammation in the gastric muc
