Familial acanthosis nigricans with p.K650T FGFR3 mutation.

Acanthosis nigricans (AN) is a pigmentary skin disorder, which may present in association with clinical disorders such as obesity and malignancy. Occasionally, this unique skin manifestation is seen in alliance with several skeletal disorders, such Crouzon syndrome, achondroplasia and hypochondroplasia (HCH). These orthopedic disorders are known to have genetic changes in FGFR3. Recently, AN was reported in HCH with p.K650T mutation in FGFR3, and to date, there are only three reports, comprising 18 cases, describing AN harboring this specific gene mutation. Herein, we detail three new cases of

[When to think of a constructional bone disease?].

When to think of a constitutional bone disease?. Constitutional bone diseases are a heterogenous group of disorders, variably causing: growth anomalies, joint limitation and/or hypermobility, pain, bone frailty and/or dysmorphic elements in the craniofacial sphere and the extremities. Clinical examination, radiographies with a phosphocalcic test allow to guide the diagnosis. Medical therapy for these pathologies is most of the time symptomatic, combining painkillers, kinesitherapy, and functional and/or surgical orthopedic treatment. Genetic consultation is essential to confirm the diagnosis,

Endoscopic-assisted Repair of Neglected Rupture or Rerupture After Primary Repair of Extensor Hallucis Longus Tendon.

Rerupture of the extensor hallucis longus tendon after primary repair and neglected rupture of the tendon poses surgical challenges to orthopedic surgeons. Open exploration and repair of the tendon ends usually requires large incision and extensive dissection. This may induce scarring and adhesion around the repaired tendon. Endoscopic-assisted repair has the advantage of minimally invasive surgery including less soft tissue trauma and scar formation and better cosmetic result. The use of Krackow locking suture and preservation of the extensor retinacula allow early mobilization of the great t

Goldenhar Syndrome and Pain-Related Temporomandibular Disorders. A Case Report.

Goldenhar syndrome (GS) is a development syndrome, characterized by incomplete development of the craniofacial region. The involvement is mainly unilateral; it varies from being mild to severe; and it can range from malocclusion and facial asymmetry to a more complex phenotype with complete absence of the mandibular ramus and temporomandibular joint. However, orthopedic symptoms of orofacial pain and dysfunction have not generally been considered as part of the symptom complex in GS cases. The case presented here is of a 15-year-old Caucasian patient, who was referred for evaluation because of

Patellar Dislocations and Reduction Procedure.

Acute patellar dislocations are a common injury occurring in adolescents involved in sports and dancing activities. This injury usually occurs when the knee is in full extension and sustains a valgus stress on the knee. The medial patellofemoral ligament is the medial restraint that assists in stabilizing the patella from lateral dislocations. The patella usually dislocates laterally and is usually not difficult to reduce after patient evaluation and prereduction radiographs. After postreduction radiographs confirm proper position of the patella postreduction and the absence of fractures, the

[Surgical treatment of skeletal metastases].

The prevalence of cancerous diseases in Germany is rising. The skeletal system represents the third most common localization for metastases. Nearly two thirds of metastases are found in the region of the spine. Due to longer survival times of tumor patients in the metastasis stage, an increase in the number of patients with bone metastases is to be expected. The treatment of patients with osseus metastases is nowadays an integral component of orthopedic trauma surgery practices. This article presents the principles of the diagnostics, provides aids for estimation of the prognosis and danger of

Management of neglected femoral neck fracture in above knee amputated limb: A case report.

The treatment of an above knee amputee who has sustained a fracture of the femoral neck is a chal- lenging situation for both the orthopedic surgeon and the rehabilitation team. These fractures may be managed acutely either by reduction and internal fixation or by endoprosthetic replacement based on the same criteria as in any other patient with otherwise intact limbs. We present a neglected case treated successfully with valgus osteotomy. We conclude that these fractures should be treated with the same urgency and expertise as similar fractures in non-amputees as long-term survival and good qu

[Foot arch shape in children aged 2-5 years according to the data of plantography and holographic interferometry].

25 children aged 2 to 5 years were examined orthopedically using the methods of plantometry and holographic interferometry of three-dimensional casts of footprints. The computer maps of the foot arch surface were obtained and the graphic reconstruction of the arch shape was performed in normal cases and in children with flatfoot. Most significant deviations of the foot arch shape, probably associated with the development delay, were detected in 4-5-year-old children under the dynamic load. Some additional advantages of holographic interferometry for the early diagnosis of flatfoot in children

Osteochondritis dissecans of the knee: pathoanatomy, epidemiology, and diagnosis.

Although several hypotheses have been described to explain the cause of osteochondritis dissecans, no single hypothesis has been accepted in the orthopedic community. Given its increased incidence among athletes, most in the sports medicine community agree that repetitive microtrauma plays at least some role in its development. Knowledge regarding the epidemiology and pathoanatomy of osteochondritis dissecans has helped the understanding of osteochondritis dissecans; however, much is still to be learned about this condition and its cause. This article reviews the history of osteochondritis as

[Sacrum fractures and lumbopelvic instabilities in pelvic ring injuries: classification and biomechanical aspects].

Multiplanar posterior pelvic ring instabilities are severe injuries and typically occur in the os ilium, the sacroiliac joint, the sacrum and/or in a combination of these sites. They pose challenges to the orthopedic trauma surgeon during reconstruction, particularly when these injuries are associated with multiplanar sacral fractures and involvement of the lumbosacral junction. Due to the multidirectional forces affecting the pelvic ring, one has to have basic knowledge about the mechanism of injury, its biomechanics, and the various treatment options. In the following we give an overview on

Isobaric spinal anesthesia: a suitable approach for a morbidly obese patient.

Morbid obesity is a relatively common and vastly increasing condition that can have a profound impact on morbidity and mortality during the administration and maintenance of general and regional anesthesia. Physiological derangements, difficult airway management, and biological augmentation in pharmacokinetics are some of the clinical challenges involved with this particular patient population. This case report discusses the advantages of regional versus general anesthesia in the morbidly obese patient population, in conjunction with an analysis of the various types of spinal anesthetics. [Bilateral elbow dislocation related to Essex-Lopresti injury].

Elbow dislocation is second in frequency, after the shoulder, whereas bilateral dislocation is uncommon, even less than dislocations with concurrent associated fractures. One of the least frequent associations is the Essex-Lopresti injury which consists of a fracture of the radial head affecting the distal radioulnar joint with injury to the interosseous membrane. This is a case of bilateral elbow dislocation, one of the elbows associated with the Essex-Lopresti injury. During treatment, the premature closed reduction prevails, previously making sure the elbow is stable, the premise which will

Suture anchor repair of patellar tendon rupture after total knee arthroplasty.

Extensor mechanism disruption after total knee arthroplasty (TKA) is a complex problem that often requires surgical repair for functional deficits. We present a brief technical note on suture anchor fixation of a patellar tendon rupture after TKA. A surgical technique and literature review follows. Although suture anchor fixation is well described for tendinous repairs in other areas of orthopedic surgery, no study has discussed the use of suture anchors in patellar tendon repair after TKA. The technique must be evaluated in more patients with longer follow-up before adoption.

Does arthroscopic partial meniscectomy always cause arthritis?

The goal of arthroscopic partial meniscectomy is to preserve a stable rim of meniscal cartilage, removing only unstable, diseased tissue. As one of the most common orthopedic procedures performed in the United States, arthroscopic partial meniscectomy developed as an alternative to open total meniscectomy that leads to degenerative changes in the knee over time. The short-term results of this meniscal-sparing procedure are favorable. Evidence of long-term results is limited to outcome measures that are not standardized. However, the long-term trend of clinical outcomes is satisfactory, whereas

Well-Differentiated Central Cartilage Tumors of Bone: An Overview.

Well-differentiated hyaline cartilage tumors are among the most common tumors encountered in the skeleton; their radiographic and pathologic classification and clinical management can be challenging. Pathologists find cartilage tumors difficult because their precise classification is as dependent on the clinical and radiographic findings as the histologic features; the distinction between benign and malignant cartilage neoplasms demands good communication and teamwork between pathologists, orthopedic surgeons, and radiologists. This review focuses on the necessary clinical, radiographic, and p

Dysplasia epiphysealis hemimelica of the elbow.

Dysplasia epiphysealis hemimelica (DEH) or Trevor's disease is a rare disorder affecting the epiphyses and short bones of the limbs, and characterized by a benign overgrowth of the medial-half of the epiphysis resembling osteochondroma. Upper limb involvement is very rare and most commonly affects the carpal bones. Only five established cases of DEH around the elbow were found in the orthopedic literature. We herein report an additional case of Trevor's disease of the elbow in a child with a symptomatic ulnar nerve compression and its management. To the best of our knowledge, this is the first

Astley Cooper: his life and surgical contributions.

Sir Astley Paston Cooper (1768-1841) was a successful and influential British surgeon during the early decades of the 19th century. He was a dedicated anatomist, an accomplished researcher, an inspiring teacher, and a skillful surgeon. Cooper published about the cause and treatment of Dupuytren's disease 10 years before Dupuytren. His contributions were widespread and substantially advanced the understanding and treatment of breast disease, vascular aneurysms, and abdominal wall hernias, along with orthopedic, otologic, and hand surgery. His contributions to surgical science have endured and a

Management of elderly patients with hip fractures and cardiac rhythm devices.

The annual incidence of hip fractures in the United States is expected to double by the year 2050. An additional challenge is that comorbidities are common in elderly patients. As indications for implantation of cardiac rhythm devices continue to broaden, the number of elderly patients with a pacemaker or an implantable cardioverter-defibrillator seen in the orthopedic surgeon's practice is likely to increase. We review the unique properties and functions of the most commonly implanted cardiac rhythm devices, provide an algorithm to assist the surgeon in gathering important patient information

Research update and recent developments in the management of scoliosis in neurofibromatosis type 1.

Neurofibromatosis type 1 (NF-1) is an autosomal dominant hereditary disease characterized by an abnormal proliferation of cells from the neural crest. Neurofibromatosis type 1 is often associated with orthopedic disorders, especially scoliosis, which is the most common skeletal manifestation of NF-1. The effects of treatment of scoliosis in NF-1 are less satisfactory than other scoliotic types due to the particular pathogenesis and clinical characteristics. Early diagnosis and treatment may be the best way to improve outcomes. This article summarizes the recent genetic and clinical development

Current thinking about acute compartment syndrome of the lower extremity.

Acute compartment syndrome of the lower extremity is a clinical condition that, although uncommon, is seen fairly regularly in modern orthopedic practice. The pathophysiology of the disorder has been extensively described and is well known to physicians who care for patients with musculoskeletal injuries. The diagnosis, however, is often difficult to make. In this article, we review the clinical risk factors of acute compartment syndrome of the lower extremity, identify the current concepts of diagnosis and discuss appropriate treatment plans. We also describe the Canadian medicolegal environm
