Eight patients (aged 8 to 22 years) with the Pierre Robin syndrome underwent sleep studies. Seven demonstrated significant although minor degrees of increased sleep disturbances and apneas, and less time spent in the rapid-eye-movement (REM) stage of sleep. One patient who had previously undergone mandibular corrective surgery had major sleep abnormalities (central apnea index of 81.7 although an obstructive sleep apnea index of only 1.9). The patients had small mandibles, as demonstrated by lateral cephalometric roentgenography, and mildly increased right ventricular diastolic dimensions, as

Automatic estimation of sleep structure is an important aspect in moving sleep monitoring from clinical laboratories to people's homes. However, the transition to more portable systems should not happen at the expense of important physiological signals, such as respiration. Here, we propose the use of cardiorespiratory signals obtained by a suprasternal pressure (SSP) sensor to estimate sleep stages. The sensor is already used for diagnosis of sleep-disordered breathing (SDB) conditions, but besides respiratory effort it can detect cardiac vibrations transmitted through the trachea. We collect

Obstructive sleep apnea is characterized by intermittent hypoxic events during sleep, and is associated with substantial neurocognitive morbidity, particularly in children. Intermittent hypoxia (IH) leads to increases in apoptosis in the cortex and hippocampus of the adult rat, peaking at 48 h of exposure. To examine whether the susceptibility to IH exhibits developmental differences, rats were exposed to 48 h of IH at ages 2, 5, 10, 15, 20, 25, 30, 60, and 120-day postnatally, and apoptosis was determined by terminal deoxy-nucleotidyl transferase-mediated in situ end labeling and immunohistoc

A review of 1,722 tonsil and adenoid procedures performed between 1978 and 1986 is presented. Each year there has been a progressive decrease in the number of procedures performed; the incidence of adenoidectomy has declined particularly. Although infection remains the predominant indication for surgery, there has been a dramatic rise in obstructive sleep apnea as a significant indication, from 0% in 1978 to 19% in 1986. At present, this trend promises to continue as physicians become increasingly aware of the prevalence and seriousness of adenotonsillar hypertrophy as a cause of sleep apnea,

The clinical diagnoses in 695 cases using PSG were analysed. The results are as follows: Obstructive sleep apnea syndrome (OSAS) in 482 cases (69.4%), chronic obstructive pulmonary diseases with nocturnal hypoxemia in 25 cases (3.6%), epilepsy in 31 cases (4.5%), other sleep disorders in 5 cases (0.7%), and essentially normal in 152 cases (21.9%). Some parameters of PSG in 65 cases with OSAS and those in 2 cases before and after treatment were analysed. It is emphasized that PSG is using for diagnosis of different types of OSAS with appraisable seriousness and of other related sleep disorders.

Some attempts have been made to screen sleep apnea syndrome (SAS). In this study, we performed daytime polysomnography (DPSG) as a means of screening SAS. 33 patients (28 males and 5 females, a mean age of 50.7 years) who were seen with the complaints of sleep-wake disorders, snoring and breathing disturbance in their nocturnal sleep were subjects for the screening. EEG, EMG, and EOG according to standard polysomnographic technique were recorded from 3:30 (p.m.) for 60 minutes, as additional indicators, oro-nasal airflow and abdominal movement were monitored simultaneously to confirm cessation

Hypoglossal nerve stimulation for obstructive sleep apnea - Updated position paper of the German Society of Oto-Rhino-Laryngology, Head and Neck Surgery. Since the first statement of the German Society of Oto-Rhino-Laryngology, hypoglossal nerve stimulation (HNS) is meanwhile an established treatment option for obstructive sleep apnea (OSA). There are three HNS systems available in Germany which differ in their technical details of the underlying comparable basic principle. For the unilateral HNS with respiratory sensing, several comparative studies, high-volume register analysis and long-term

OBJECTIVE AND BACKGROUND: Excessive fragmentary myoclonus (EFM) is characterized by subtle arrhythmic and excessive jerks that are usually asymmetric and asynchronous. EFM occurs in different areas of the body, mainly the face and distal parts of the arms and legs, and is detected by surface electromyography during sleep. The present study aimed to determine the prevalence of EFM in Parkinson's disease (PD) patients at a tertiary level outpatient clinic as well as to describe the clinical and polysomnographic profiles of these patients.
METHODS: A total of 62 consecutive PD patients were incl

BACKGROUND: There has been marked expansion in the literature and practice of pediatric sleep medicine; however, no recent evidence-based practice parameters have been reported. These practice parameters are the first of 2 papers that assess indications for polysomnography in children. This paper addresses indications for polysomnography in children with suspected sleep related breathing disorders. These recommendations were reviewed and approved by the Board of Directors of the American Academy of Sleep Medicine.
METHODS: A systematic review of the literature was performed, and the American

A 3-year-old girl with a history of excessive weight gain from birth presented with obesity, somnolence, and cyanosis, characteristic of the Pickwickian syndrome. Obesity was familial and exogenous without endocrine or neurologic anomaly. Respiratory center sensitivity to carbon dioxide was normal. Excessive somnolence was due to the obesity, which during sleep caused airway obstruction, apnea, and awakening, finally resulting in sleep deprivation. The sleep apneas and the daytime somnolence disappeared with weight reduction, showing that obesity alone had been responsible for the disorder.

In recent years, increased attention has been directed to sleep apnea syndrome due to its high prevalence and preventable severe health consequences. Besides enhancing the risk of cardiovascular, cerebrovascular, and metabolic disorders, it determines increased daytime somnolence, cognitive impairment, and delayed reaction time. These symptoms, determined by sleep fragmentation and chronic hypoxemia, can result in a decrease in professional performance and, moreover, could have tragic implications, especially in patients with high-risk professions. We present the case of a 58-year-old male-tru

UNLABELLED: Sleep is a dynamic physiological process, its primary function being the restoration of the central nervous system. Because sleep disorders affect a significant part of the population the dynamic long term monitoring of human respiration plays a very important role in diagnosis and treatment.
MATERIAL AND METHODS: We describe a remote monitoring system designed for in-hospital or at-home detection with a high degree of accuracy of sleep-related disorders in patients experiencing episodes of obstructive sleep apnea. Patient's respiratory rate is continuously measured by using wirel

Infantile Pompe disease is a rare, autosomal recessive disorder due to deficiency of the enzyme acid α-glucosidase that degrades lysosomal glycogen. Clinical features of diffuse hypotonia, cardiomyopathy, and weakness are present within the first days to months of life in patients with classic infantile Pompe disease. Progression of the disease often leads to respiratory failure. Although sleep apnea is reported in late-onset Pompe disease, sleep pathology is not well characterized in infantile disease. In this retrospective study, we analyzed nocturnal polysomnography results from 17 patients

Decades of research on neurodegenerative diseases such as Parkinson's disease (PD) have shifted our focus from clinical symptomatic stages to one step earlier, the prodromal stage. There is convincing evidence showing that sleep disorders such as excessive daytime somnolence, insomnia, obstructive sleep apnea, and particularly REM sleep behavior disorder (RBD) are potential prodromal features starting years and decades before conversion to PD. This review aims to provide an overview on various sleep disorders as prodromal features of PD. We start with a discussion of how neuroanatomy of sleep

OBJECTIVES: Sleep apnea/hypopnea syndrome is a well-recognized independent risk factor for stroke in middle-aged population, but controversy remains in older subjects. We examined the possible association between different respiratory parameters and risk of stroke in a prospective population-based cohort of 394 stroke-free elderly subjects.
MATERIAL AND METHODS: Fully overnight polysomnography was performed at baseline. Over the 6 year follow-up period, 20 ischemic strokes occurred. Differences in stroke-free survival between subjects according to central apnea index (CAI) were assessed.
