Carbidopa ameliorates high-fat diet-induced insulin resistance by suppressing gut-derived tryptamine/phenethylamine and systemic 5-HT levels.

Obesity and type 2 diabetes mellitus (T2DM) are major global health burdens, with insulin resistance (IR) as a key pathological driver. This study explores carbidopa, a selective aromatic l-amino acid decarboxylase inhibitor, as a modulator of gut-derived metabolites to alleviate high-fat diet (HFD)-induced IR. We observed that HFD markedly increased fecal tryptamine and phenethylamine levels, which positively correlated with elevated serum 5-hydroxytryptamine (5-HT). Carbidopa treatment suppressed gut production of these metabolites, reducing circulating 5-HT and improving insulin sensitivity

Association Between Renal Cell Cancer and Chronic Kidney Disease: An Update on a Never-Healing Wound.

The relationship between chronic kidney disease (CKD) and renal cell carcinoma (RCC) is both bidirectional and multifactorial. Several risk factors, including hypertension, diabetes mellitus, obesity, and smoking, have been associated with an increased risk for the development of CKD and RCC. CKD may predispose individuals to RCC through various mechanisms, including renal cystic diseases or induced oxidative stress effects. Conversely, RCC can induce CKD through the direct effects of the tumor, after surgeries for the management of the tumor (either partial or radical nephrectomy), and throug

Rare SRRM2 mutation in neurodevelopmental disorders involving hyperphagia triggering severe obesity and other complication.

SRRM2 encodes a nuclear protein, with functions in pre-mRNA splicing and the formation of nuclear speckles via liquid-liquid phase separation. Despite its critical role in cellular function, the association between SRRM2 and neurodevelopmental disorders is not well-understood. In this study, we reported a case of a patient exhibiting developmental delay, intellectual disability, delayed language development, facial dysmorphism, macrocephaly, short hands and feet, hyperphagia, and hypotonia, which are similar to the characteristics of previously reported cases of SRRM2-associated neurodevelopme

Male Breast Cancer: Epidemiology, Diagnosis, Molecular Mechanisms, Therapeutics, and Future Prospective.

Male breast cancer (MBC) is rare, representing 0.5%-1% of all breast cancers, but its incidence is increasing due to improved diagnostics and awareness. MBC typically presents in older men, is human epidermal growth factor receptor 2 (HER2)-negative and estrogen receptor (ER)-positive, and lacks routine screening, leading to delayed diagnosis and advanced disease. Major risk factors include hormonal imbalance, radiation exposure, obesity, alcohol use, and Breast Cancer Gene 1 and 2 (BRCA1/2) mutations. Clinically, it may resemble gynecomastia but usually appears as a unilateral, painless mass

Cardiometabolic Dysregulation and Heart Failure.

Heart failure (HF) is a complex clinical syndrome resulting from impaired myocardial function or structure, affecting approximately 56 million patients worldwide. Cardiometabolic risk factors, including hypertension, insulin resistance, obesity, and dyslipidemia play a pivotal role in both the pathogenesis and progression of HF. These risk factors frequently coexist as part of cardiometabolic syndrome and contribute to widespread organ and vascular dysfunction, leading to conditions such as coronary artery disease, chronic kidney disease, type 2 diabetes mellitus, non-alcoholic fatty liver dis

The associated factors of nutritional issues and body composition in Rett syndrome.

Individuals with Rett syndrome (RTT), a rare neurodevelopmental disorder, often face significant nutritional challenges. We explored factors affecting nutritional status and body composition in RTT individuals using dietary assessments and bioelectrical impedance analysis (BIA). This study involved 40 participants and found that underweight individuals with severe growth deficits often had reduced body fat, visceral fat, and arm circumference. Protein and zinc intake percentages were significantly lower in those with higher severity scores. Adolescents with the highest growth deficit and under

Extended Review and Updates of Nonalcoholic Fatty Pancreas Disease.

Non-alcoholic fatty pancreatic disease (NAFPD), also known as pancreatic steatosis, is a benign condition characterized by deposition of lipids in the pancreas and is associated with insulin resistance, malnutrition, obesity, metabolic syndrome, aging, and absence of heavy alcohol intake or infection. Similar to nonalcoholic fatty liver disease, NAFPD is a phenotypic entity that includes fat buildup in the pancreas, pancreatic inflammation, and subsequent fibrosis. The extent to which pancreatic fat infiltration is clinically important remains unclear. Despite these clinical associations, most

Arterial Stiffness and Heart Failure With Preserved Ejection Fraction.

Heart failure with preserved ejection fraction (HFpEF) is prevalent and associated with a poor prognosis, imposing a significant burden on society. Arterial stiffness is increasingly recognized as a crucial factor in the pathophysiology of HFpEF, affecting diagnosis, management, and prognosis. As a hallmark of vascular aging, arterial stiffness contributes to increased afterload on the left ventricle (LV), leading to diastolic dysfunction, a key feature of HFpEF. Elevated arterial stiffness is linked with common cardiovascular risk factors in HFpEF, such as hypertension, diabetes and obesity,

Targeting the sympathetic nervous system with the selective imidazoline receptor agonist moxonidine for the management of hypertension: an international position statement.

Hypertension is often linked with metabolic risk factors that share common pathophysiological pathways. Despite wide-spread availability of multiple drug classes, optimal blood pressure (BP) control remains challenging. Increased central sympathetic outflow is frequently neglected as a critical regulator of both circulatory and metabolic pathways and often remains unopposed therapeutically. Selective imidazoline receptor agonists (SIRAs) effectively reduce BP with a favorable side effect profile compared with older centrally acting antihypertensive drugs. Hard outcome data in hypertension, suc

Biliary complications associated with weight loss, cholelithiasis and choledocholithiasis.

Biliary complications like cholelithiasis and choledocholithiasis are more common in bariatric surgery patients due to obesity and rapid weight loss. Patients with a body mass index > 40 face an eightfold risk of developing cholelithiasis. Post-bariatric surgery, especially after laparoscopic Roux-en-Y gastric bypass (LRYGB), 30% of patients develop biliary disease due to rapid weight loss. The aim of this review is to analyze the main biliary complications that occur after bariatric surgery and its management. A review of the literature was conducted mainly from 2010 up to 2023 with regard to

Are the lipid-lowering effects of incretin-based therapies relevant for cardiovascular benefit?

PURPOSE OF REVIEW: This review examines the impact of glucagon-like peptide 1 receptor agonists (GLP-1RAs) on lipid profiles in individuals with type 2 diabetes mellitus and/or obesity, crucial for optimizing cardiovascular risk management. RECENT FINDINGS: GLP-1RAs affect lipid levels by reducing intestinal apolipoprotein B48 production and mesenteric lymph flow, while increasing catabolism of apolipoprotein B100. It remains unknown whether these effects are direct or indirect, but the improvements in lipid levels are strongly correlated to the drug-induced weight loss. Clinical trials demon

Immunosuppression in solid organ-transplant recipients and impact on nutrition support.

A key component to nutrition support is to consider immunosuppressive agents, the interaction with nutrients, and how the side effects of the medications influence nutrition support. The immunosuppression of the solid organ-transplant recipient involves the individualized titration of multiple therapeutic agents to prevent allorecognition and, thus, rejection of the transplanted organ. Induction immunosuppression includes the agents used at the time of transplant to prevent early rejection. Maintenance immunosuppression typically consists of oral medications taken for life. Regular therapeutic
