Hepatocyte transplantation as a bridge to orthotopic liver transplantation in terminal liver failure.

The limited donor organ supply has led to several bridging techniques to sustain patients with acute and subacute liver failure. We report here the prospective, controlled trial of transplanted isolated fresh and cryopreserved human hepatocytes as a bridge to orthotopic liver transplantation. Five hepatocyte transplant recipients with grade IV encephalopathy and multisystem organ failure and four patients of equal illness severity due to liver failure were studied. Medical therapy resulted in a significant (P<0.05), but not normal, fall in blood ammonia, and a significant (P<0.02) resolving bi

Yield of Whole-Exome Sequencing in Children With Nonacquired Chronic Ataxia: A Prospective Study From a Single Tertiary Care Hospital in South India.

BACKGROUND: Chronic childhood ataxia presents a diagnostic challenge as multiple conditions (congenital, acquired, and genetic) in pediatric neurology present with chronic ataxia. Overlapping phenotypes, developmental variability, and limited availability of specialized testing complicate diagnosis. Around 50-70% cases remain undiagnosed with traditional approaches. While acute ataxias frequently result from acquired etiologies such as infection or toxicity, chronic and progressive cerebellar ataxia cases are predominantly genetic in etiology. In recent years, wide availability and reduced cos

Exploring skeletal disorders in cattle and sheep: a WGS-based framework for diagnosis and classification.

BACKGROUND: Genetic skeletal disorders are a heterogeneous group of syndromic or non-syndromic diseases characterized by abnormal bone, joint or cartilage development. These disorders generally occur sporadically in ruminants. Although a genetic etiology is often suspected, only a limited number of causal variants have been identified and no comprehensive genetic analyses of a cohort of bovine and ovine skeletal developmental defects have been published. The aims of our study were (1) to propose a nosology of genetic skeletal disorders in cattle and sheep and (2) to contribute to the nosology

Study protocol of the HD-MED study aiming to personalize drug treatment in Huntington's disease: a longitudinal, observational study to assess medication use and efficacy in relation to pharmacogenetics.

BACKGROUND: Huntington's disease (HD) is a hereditary, neurodegenerative disorder characterized by motor, cognitive, and psychiatric symptoms. Currently, HD can only be managed symptomatically, including a large variety of prescribed drugs. Many HD patients experience negative medication effects (e.g. side effects or non-response). Pharmacogenetic (PGx) studies show how genetic variation affects both medication efficacy and toxicity and holds the potential to improve these outcomes of drug treatment. PRIMARY OBJECTIVE: To classify the effect of the PGx profile of CYP2C19 and CYP2D6 in HD gene

Twenty-two-year evolution of a Medical Breast Service: Filling the important gaps between breast surgery and medical oncology.

BACKGROUND: Historically, surgeons have provided subspecialty breast care. The development of a robust medical breast program in a large academic center staffed by trained primary care providers initially showed a shift in care of benign breast disease away from surgeons. In this review, we aim to revisit the practice after 20 years. Medical patients are defined as patients with symptomatic issues (eg, pain or lump), those at increased risk (due to family history, genetic mutations, or benign atypical lesions), or survivors in need of long-term care. METHODS: Data for this review were collect

Novel Compound Heterozygous CFAP53 Variants in a Fetus With Situs Inversus Totalis: A Case Report.

Heterotaxy defects can include morphological deviations that affect left-right symmetry. In this case report, we observed novel compound heterozygous CFAP53variants that affect laterality. Whole exome sequencing (WES) was performed during pregnancy. Prenatal exome sequencing is particularly valuable for fetuses with laterality defects, given the increasingly high diagnostic success observed in this category of morphological abnormalities. Molecular testing plays an important role in genetic counselling, allowing parents to make decisions about pregnancy, calculating risk, and often predicting

Abnormal DHR is not always synonymous with a diagnosis of chronic granulomatous disease.

Myeloperoxidase (MPO) deficiency is a rare inherited neutrophil disorder associated with an increased risk of infections. We present a case of a male child with no underlying medical conditions who presented with a difficult-to-manage renal abscess, enterococcal urosepsis and an abnormal dihydro-rhodamine 123 test. On genetic evaluation, he was found to have a homozygous mutation in the MPO gene, leading to the diagnosis of MPO deficiency. This case highlights the clinical importance of MPO activity in the clearance of specific microorganisms. MPO deficiency should be considered in the differe

Tailing growth, neonatal jaundice and anaemia.

At medical school, there is a phrase to help us remember that common things are common: 'If you hear hooves think horses, not zebras'. However, zebras do exist, and from time to time in general paediatric and neonatal practice, we will encounter these rare diagnoses, more of which we can now accurately diagnose through the ever-expanding field of genomics. Our case demonstrates how a rare diagnosis can present with common features of growth restriction, jaundice and anaemia. Paediatricians therefore require a high index of suspicion and increasing knowledge of the logistics of genetic testing.

Recent Insights into Pediatric Primary Sclerosing Cholangitis.

This article reviews recent literature on the pathogenesis, presentation, diagnosis, comorbidities, natural history, and management of pediatric primary sclerosing cholangitis (PSC). The authors shed light on the role of genetic and environmental factors in PSC, although recognize the limitations in the understanding of PSC pathogenesis. They reflect on presenting disease phenotypes, including the association with inflammatory bowel disease and frequent histologic presence of autoimmune hepatitis features. The current lack of effective medications is discussed, and disease complications and pr

Update on Gambling Disorder.

Gambling disorder (GD) is estimated to be experienced by about 0.5% of the adult population in the United States. The etiology of GD is complex and includes genetic and environmental factors. Specific populations appear particularly vulnerable to GD. GD often goes unrecognized and untreated. GD often co-occurs with other conditions, particularly psychiatric disorders. Behavioral interventions are supported in the treatment of GD. No medications have a formal indication for the GD, although clinical trials suggest some may be helpful. Noninvasive neuromodulation is being explored as a possible

Management strategies in facioscapulohumeral muscular dystrophy.

Facioscapulohumeral muscular dystrophy (FSHD) also known as Landouzy-Dejerine disease, is an autosomal-dominant disorder of the skeletal muscles with the name according to the various muscle groups it affects: the face, shoulders and upper arms. It is the third most common genetic degenerative disorder of the skeletal muscles without specific patterns in all the affected individuals. At present there is no cure for the disease but numerous management strategies are available to improve the quality of life and prevent further degeneration of various muscle groups. This review aims to provide an

Tubulointerstitial Nephritis.

Tubulointerstitial nephritis (TIN) is a cause of acute kidney injury in children characterized histologically by an inflammatory cell infiltrate in the kidney interstitium. The most common causes of TIN in children include medications, infections, inflammatory disorders, and genetic conditions. TIN typically presents with nonoliguric acute kidney injury and may be associated with systemic symptoms, including fever, rash, and eosinophilia. The long-term prognosis is generally favorable, with full kidney recovery; however, some patients may develop progressive chronic kidney disease. Immunosuppr

Identical Twins with Crouzon Syndrome: Eight-Year Follow-up, Genetic Considerations, and Operative Management.

A case report of monozygotic (MZ) twins with Crouzon syndrome was previously published to highlight variables in clinical presentation. The postnatal and epigenetic causes for this variation are not well understood. An 8-year follow-up discusses their pertinent clinic course with consideration of genetic and nongenetic variables. The phenotypic and symptomatic obstacles encountered since their initial assessment are reviewed, and the use of three-dimensional Medical Modeling (Golden, CO) as a preoperative planning strategy is addressed. Analyzing the longitudinal clinical course of MZ twins wi

Atopic dermatitis in children: clinical features, pathophysiology, and treatment.

Atopic dermatitis (AD) is a chronic, relapsing, highly pruritic skin condition resulting from disruption of the epithelial barrier and associated immune dysregulation in the skin of genetically predisposed hosts. AD generally develops in early childhood, has a characteristic age-dependent distribution and is commonly associated with elevated IgE, peripheral eosinophilia, and other allergic diseases. Medications such as antihistamines have demonstrated poor efficacy in controlling AD-associated itch. Education of patients regarding the primary underlying defects and provision of a comprehensive

Asthma associated with incontinentia pigmenti and Fanconi anemia: variable airflow limitation without cellular bronchitis.

Airway inflammation is considered a central component of asthma and, therefore, international guidelines recommend antiinflammatory medications. We describe the clinical history of a 34-year-old woman with airway hyperresponsiveness and asthma who had a reduced ability to mount an inflammatory response due to two unrelated and rare genetic conditions: Fanconi anemia and incontinentia pigmenti. Absence of eosinophils in blood and sputum led to a successful reduction in the dose of corticosteroids without loss of asthma control demonstrating the clinical utility of monitoring treatment using bio

Stem Cells Transplantation and Huntington's Disease.

Huntington's disease (HD) is a progressive and devastating neurodegenerative disorder that results in movement abnormalities, cognitive impairments, dementia, and affective disturbances. As no proven medical therapy for this genetic disease is currently available, symptoms mitigation is the primary treatment for HD. Stem cells can play an important role in cell therapy therapeutic strategies to replace dysfunctional or dying cells in HD. Here, we present a brief overview of the current state of stem cells therapy and of the results obtained in animal models of HD, and discuss neuro-protective
