Proximal row carpectomy versus limited wrist arthrodesis for advanced Kienböck's disease.

The outcomes in 20 patients with advanced Kienböck's disease treated by proximal row carpectomy (seven patients) or limited wrist arthrodesis (13 patients) were reviewed retrospectively. Postoperatively, the results were more satisfactory in terms of wrist pain, the range of wrist flexion-extension, and grip strength following limited wrist arthrodesis than after proximal row carpectomy, although the differences were not statistically significant. We recommend scaphotrapeziotrapezoid arthrodesis in selected patients with advanced Kienböck's disease who have a fragmented lunate.

Posterior cervical fixation for fracture and degenerative disc disease.

There are numerous newer techniques that have been developed for the internal fixation of the cervical spine in recent years. Wiring techniques are still appropriate for posterior stabilization of the cervical spine. The halo vest is still widely used for the conservative management of cervical fractures and for postoperative external immobilization. The authors stress that the surgical indications for more modern rigid implants should be adhered to strictly. These implants also should be selected by weighing their advantages versus potential risks. In the upper cervical spine, the surgeon may

[Unusual course of scapho-capitate syndrome].

The scapho-capitate-syndrome has been defined as combined fractures of the scaphoid and capitate with rotation of the proximal fragment of the capitate through 90 or 180 degrees. Most injuries are due to falls from heights or car accidents. Only 26 cases of the scapho-capitate fracture syndrome have been reported since 1937. In nearly one third of these cases, there was a delay of more than two weeks in diagnosis. We report a case of a young man in whom the complex injury was erroneously diagnosed as an isolated fracture of the scaphoid. The scaphoid fracture was treated by a screw and healed

The fate of plates and screws after facial fracture reconstruction.

Rigid plate and screw fixation is the mainstay of treatment for complex fractures of the facial skeleton. Complications of plate and screw fixation include prominence, infection, exposure, and migration. Five hundred and seven patients undergoing plate and screw fixation for facial fractures (1112 fractures) from 1983 to 1988 were followed for complications. Sixty-one patients (12 percent) required hardware removal. The location on the facial skeleton influenced symptoms and the rate of hardware removal. Infection and exposure may be decreased with antiseptic irrigations, avoiding mucosal dama

Patellar fracture and avulsion of the patellar ligament complicating arthroscopic anterior cruciate ligament reconstruction.

Arthroscopic reconstruction of the anterior cruciate ligament-deficient knee using the middle third of the patellar tendon is an effective treatment for acute or chronic anterior cruciate ligament insufficiency. The strength of this bone-ligament-bone graft is superior to virtually all other autogenous tissue utilized. We report the complication of avulsion of one half of the remaining patellar tendon from the tibia associated with patellar fracture in the early postoperative period.

Bone-Patellar Tendon-Bone Allograft Preparation Technique for Anterior Cruciate Ligament Reconstruction.

BACKGROUND: Allograft anterior cruciate ligament (ACL) reconstruction, while it may have a higher failure rate in younger and more active populations, continues to serve as a viable graft option for the appropriately indicated patient. Efficient bone-patellar tendon-bone (BTB) allograft preparation is beneficial to reduce operating time and ensure optimal reconstruction with bony fixation. INDICATIONS: ACL reconstruction with BTB allograft is indicated for skeletally mature and older patients, patients who are less active and have fewer physical demands, patients who have had previously harve

Comparison of two methods in the treatment of congenital pseudarthrosis of clavicle: multicenter experience.

BACKGROUND: Congenital pseudoarthrosis of the clavicle (CPC) is an uncommon entity. Owing to its scarce presentation, treatment of this disorder has not been well established. This study aimed (1) to compare surgical treatment methods that included excision of pseudoarthrosis and iliac crest bone graft and fixate with either the elastic stable intramedullary nail (ESIN) or K-wires or plate and screws, and (2) to assess the clinical outcomes of two different surgical methods. METHODS: A multi-central retrospective study was performed between 2013 and 2017 in four tertiary teaching hospitals. F

Functional Outcomes After Temporary Bridging With Locking Plates in Lisfranc Injuries.

The standard operative treatment of Lisfranc fracture dislocations currently consists of open reduction and transarticular fixation. Recently, bridge plating has been used more often. Using joint spanning, the reduced fracture dislocation is temporary stabilized to minimize articular damage. The present study describes the outcomes of patients treated with bridge plating after tarsometatarsal fracture dislocations compared with transarticular screw fixation. A retrospective cohort study was performed. Patients with an isolated tarsometatarsal injury who had been treated operatively from June 2

[Modern implant design for the osteosynthesis of osteoporotic bone fractures].

Osteoporosis is characterized by a reduction of bone mass and changes in bone micro-architecture. The resulting reduction in bone strength leads to the well recognized increase in the risk of fracture, particularly at the radius, hip, and spine. The treatment of osteoporotic fractures is challenged by the reduced mechanical capacity of osteoporotic bone, reflected in reduced holding power and increased fragility. The aim of successful fracture treatment in individuals with osteoporosis is early fixation of the fracture with immediate and almost unrestricted weight-bearing capacity. The key fac

Can bone SPECT/CT and 18 F-NaF PET/CT diagnose the cause of persistent pain due to motion sparing devices: a case series.

PURPOSE: Can positron emission tomography/CT (18 F-NaF PET/CT) and Bone single-photon emission CT (SPECT/CT) be used to diagnose the cause of persistent pain in motion-sparing devices? Interspinous devices (ISD) and cervical disc arthroplasty (CDA) are used increasingly. While a desired outcome is achieved in a portion of these patients, others develop persistent postoperative symptoms. Etiology of these symptoms is diagnosed with imaging such as x-ray, computed tomography (CT), or magnetic resonance imaging (MRI). Despite these modalities, there is a subset of patients with persistent pain wh

Advances in implants and bone graft types for lumbar spinal fusion surgery.

The increasing prevalence of spinal disorders worldwide necessitates advanced treatments, particularly interbody fusion for severe cases that are unresponsive to non-surgical interventions. This procedure, especially 360° lumbar interbody fusion, employs an interbody cage, pedicle screw-and-rod instrumentation, and autologous bone graft (ABG) to enhance spinal stability and promote fusion. Despite significant advancements, a persistent 10% incidence of non-union continues to result in compromised patient outcomes and escalated healthcare costs. Innovations in lumbar stabilisation seek to mimic

Biomechanical Evaluation of Suture Tape Versus Metallic Wire With Cannulated Screws Tension Band Fixation For Transverse Patella Fractures: A Biomechanical Study.

BACKGROUND: Transverse patella fractures requiring surgical fixation are typically treated with a tension band construct using metal wire. This technique is associated with high rates of complications and reoperation. Alternative fixation technique utilizing non-absorbable suture materials have been introduced. This biomechanical study aims to compare the stability of two tension band constructs, both utilizing headed cannulated lag screws, with either suture tape or traditional metal wire under cyclic load conditions, simulating repetitive physiological stress. METHODS: Six matched pairs of

Effect of humerus rotation on the initial stability of locking plate fixation for proximal humerus fracture: A biomechanics study.

BACKGROUND: Limb rotations of the upper arms frequently happen during daily activities, and these actions can produce significant torsional forces which aggravate patients' conditions. However, there is limited research concerning the impact of humerus rotation on the initial stability of locking plate fixation after proximal humerus fracture reconstruction. The current study conducted a biomechanics analysis to investigate this important issue. METHODS: Dynamic mechanical tests with repetitive torsional loads were executed in six fracture-model specimens. The mechanical behaviors of resistan

Intramedullary nailing versus sliding hip screw for AO/OTA 31-A2 and 31-A3 trochanteric fractures: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Trochanteric femoral fractures (TFFs) are frequent injuries in older adults, with unstable patterns (AO/OTA 31-A2 and A3) carrying a higher risk of complications. Intramedullary nailing (IMN) and sliding hip screw (SHS) are the two main surgical options. However, guideline recommendations differ, and prior meta-analyses are limited. This study aimed to systematically compare the efficacy and safety of IMN versus SHS in treating adult patients with unstable TFFs based on randomized controlled trials (RCTs). METHODS: MEDLINE, Embase, and CENTRAL (January 2008-March 2025) were search
