Gradual and safe technique for sinus floor elevation using trephines and osteotomes with stops: a cadaveric anatomic study.

OBJECTIVE: The aim of this study was to develop a new technique for maxillary sinus floor elevation through the crestal approach, using trephines and osteotomes with stops, and to assess the risk of sinus membrane lesion. STUDY DESIGN: The study was performed on 30 heads removed from fresh nonpreserved cadavers with subsinus bone height >or=5 mm. The anatomic specimens were sectioned axially on a plane passing 1 cm below the infraorbital foramen, to be able to see and film the sinus floor covered by the membrane. A total of 112 implants were placed using this technique (48 without grafting ma

Maxillary sinus osteoplasty with vascularized pedicled bone flap.

OBJECTIVE: This article describes the use of a vascularized pedicled bone flap, superiorly based on schneiderian membrane, for osteoplasty of maxillary sinus. STUDY DESIGN: A rectangular bony window was created on the anterior maxillary wall and pedicled from its inner surface with the schneiderian membrane attached to it and left intact superiorly. After the sinus pathology was addressed, the pedicled bony window was repositioned and stabilized with sutures. This technique was performed in 9 maxillary sinuses in 9 patients. A nasoantral window was opened in all of the cases. RESULTS: The fo

A retrospective study of the effects of sinus membrane elevation on bone formation around implants placed in the maxillary sinus cavity.

OBJECTIVE: The aim of this study was to investigate the height of new bone formation in the maxillary sinus after implants were inserted into the maxillary sinus without any additional grafting material after sinus membrane elevation. STUDY DESIGN: Nine patients with a combined total of 10 implants that were inserted into the maxillary sinus more than 4 mm (range, 4-6 mm) after sinus membrane elevation were evaluated for endosinus bone gain and crestal peri-implant bone loss 4 months after implantation using computerized tomography and dental radiographs. RESULTS: The mean endosinus bone gai

Five-year follow-up of a root canal filling material in the maxillary sinus: a case report.

This report describes the 5-year clinical and radiographic follow-up of an accidental extrusion of a root canal filling material into the maxillary sinus and emphasizes the importance of monitoring of similar cases. The most favorable prognosis is achieved by surgically removing the extruded material from the sinus. Excess zinc oxide-eugenol-based materials in the sinus might cause aspergillosis sinusitis, and this entity often needs many years to develop. In this case, endomethasone was the filling material; however, the patient refused to undergo any surgical intervention. After a follow-up

Measurement of volume changes of sinus floor augmentation covered with buccal fat pad: a case series study.

OBJECTIVES: The purpose of this study was to evaluate the volumetric changes of the maxillary sinus graft in conjunction with buccal fat pad (BFP) covering the lateral sinus wall. STUDY DESIGN: In this article, the radiographic results are presented on 11 consecutively treated patients using an equal mixture of the autogenous bone harvested from the tuberosity and natural bone mineral (Bio-Oss) used to augment the maxillary sinus. Buccal fat pad was used over the lateral sinus wall in all cases as a membrane to cover the augmentation material. The mean initial bone height (IBH) was 3.82 mm as

Causes of chronic obstructive parotitis and management by sialoendoscopy.

BACKGROUND: Sialoendoscopy is a relatively new technique to detect causes of obstruction in the parotid gland directly and manage the chronic obstructive parotitis effectively combined with continuous lavage and drug perfusion simultaneously. OBJECTIVES: To describe the cause, exploration, and combined management of chronic obstructive parotitis using sialoendoscopy. STUDY DESIGN: Between October 2004 and June 2006, 23 patients with obstructive symptoms were diagnosed by sialography and explored by diagnostic sialoendoscopy. The obstructions were then removed by interventional sialoendoscopy

Lymphadenoma of parotid gland: Two additional cases and a literature review.

For classification purposes, proper identification of infrequent and unique salivary gland tumors requires the gradual accumulation of a sufficient number of cases. Lymphadenoma (i.e., an adenomatous, generally parotid-based lesion with an exaggerated lymphocytic infiltrate, but a lack of sebaceous differentiation) has approximately 9 reported cases. This report adds 2 additional cases occurring as a discrete, at least partially encapsulated nodule in the parotid gland. Embedded within the extensive lymphocytic component were isolated nests of solid or glandular epithelium, with 1 case display

The effects of high frequency, low intensity ultrasound therapy on parotid flow rates.

OBJECTIVES: Saliva plays a critical role in maintaining oral health and comfort. Development of a single device capable of simulating salivary flow with no taste or adverse side effects is desirable. STUDY DESIGN: This clinical investigation on 23 healthy volunteers with no dry mouth complaints evaluated the effectiveness of ultrasonic therapy in mechanical stimulation of the parotid gland to increase saliva flow. Stimulated and unstimulated parotid saliva was collected. Differences in mean salivary flow rates and affects of age and ethnicity were measured. RESULTS: The mean difference betwe

Sonographic diagnosis of Sjögren syndrome: evaluation of parotid gland vascularity as a diagnostic tool.

OBJECTIVE: To evaluate the usefulness of the vascularity in parotid glands in sonographic diagnosis for Sjögren syndrome. STUDY DESIGN: Sonographic images of 72 cases of previously suspected Sjögren syndrome (including 43 actual cases) were analyzed retrospectively for the abnormal vascularity in the parotid gland parenchyma. The relationships between the vascularity and the results of sialographic, serologic, and histopathologic examinations were analyzed. We also compared the diagnostic accuracy of B-mode only with that of B-mode plus Doppler-mode. RESULTS: Sjögren-positive cases showed si

Myoepithelial carcinoma of the submandibular gland: report of a case with multiple cutaneous metastases.

Myoepithelial carcinoma is a rare and aggressive neoplasm of the salivary glands. One-third of the patients may develop regional distant metastases, and lungs and kidneys have been regarded as the most usual sites for implantation. There is, however, little information on the metastatic behavior of this malignancy. We report the first case of patient with multiple cutaneous metastases from a myoepithelial carcinoma of the submandibular gland, which depicted a very aggressive clinical course.

Cystic hygroma: characterization by computerized tomography.

Lymphangiomas are benign nonencapsulated lesions composed of sequestered noncommunicating lymphoid tissue lined by lymphatic endothelium and are thought to be caused by congenital obstruction of lymphatic drainage. They are subclassified by vessel size, such as the capillary, which is rare and located in subcutaneous tissue, cavernous (located about the mouth and tongue), and cystic (cystic hygromas). The cystic hygromas show a predilection for the neck (75%) and maxilla (20%), and the remaining 5% arise in rare locations such as the mediastinum, retroperitoneum, bone, kidney, colon, liver, sp

Pilot study: digital subtraction radiography as a tool to assess alveolar bone changes in periodontitis patients under treatment with subantimicrobial doses of doxycycline.

BACKGROUND: Subtle changes in marginal alveolar bone level can be demonstrated using digital subtraction of sequential radiographs. OBJECTIVE: We aimed to evaluate the practical application of geometrically corrected digital subtraction in a clinical study of alveolar bone response to a drug previously shown to inhibit alveolar bone loss. STUDY DESIGN: Selected periapical radiographs were acquired with projective standardization of patients with clinical marginal alveolar bone loss. Subsequently, patients received a 6-month regimen of subantimicrobial doxycycline or placebo. Standardized rad

Comparison of the efficiencies of permanent maxillary tooth removal performed with single buccal infiltration versus routine buccal and palatal injection.

OBJECTIVE: The aim of the present study was to demonstrate whether articaine/HCl administered alone as a single buccal infiltration in maxillary tooth removal provided favorable palatal anesthesia as compared to buccal and palatal injection, for a surgical procedure. STUDY DESIGN: In total, 71 patients for removal of bilateral permanent maxillary teeth were enrolled in the present study. For the experimental side, 1.7 mL of 4% articaine/HCl was injected into the buccal vestibule of the tooth. After 5 minutes, tooth extraction was performed. On the control side a similar protocol was applied w
