Hybrid procedures for an infant with hypoplastic left heart syndrome with intact atrial septum.

A boy, prenatally diagnosed as hypoplastic left heart syndrome (HLHS) with intact atrial septum (IAS) was successfully treated by hybrid procedures. He underwent emergent catheter atrial septostomy and stent insertion in the atrial septum on Day 1 and then underwent bilateral pulmonary artery banding, ductal stent insertion, modified Norwood operation, bidirectional Glenn's operation and finally Fontan type operation at 2 years of age. Considering the presence of decompression pathway from the left atrium in HLHS with IAS, we should organize a treatment team for collaborative work and plan an

Association of congenital descending aorto-left atrial fistula with the aortopulmonary window and atrial septal defect.

We report a case of fistula between the descending aorta and the left atrium with associated aortopulmonary window and secundum atrial septal defect. No previous reports of such association have been found in the literature. A 5-month-old infant presented with heart failure from the age of 2 weeks. Echocardiography confirmed the presence of an aorto-left atrial fistula, aortopulmonary window, and atrial septal defect. The diagnosis was additionally supported by computed tomography. The entity aorto-left atrial fistula is rare. It is mostly acquired and usually between the ascending aorta and t

Massive atrial myxoma: surgical treatment for an incidentaloma causing dyspnoea.

A patient presented having a one-week history of recurrent falls and confusion and weight loss over an unspecified period of time. A chest radiograph revealed bilateral pleural effusions and the patient was treated for community acquired pneumonia. His weight loss and suspicion of malignant disease prompted computer tomography to be conducted. This revealed widespread mediastinal and oesophageal adenopathy. An echocardiogram showed a large hyperechoic mobile mass. Coronary angiography showed complete occlusion of the left anterior descending and right coronary arteries. He underwent two-vessel

A rare disappearing right atrial mass.

Cardiac tuberculosis is rare and usually involves the pericardium. Myocardial tuberculoma is a very rare occurrence and only a few cases have been reported. We describe a rare case of cardiac tuberculoma involving the whole of the lateral right atrial wall, extending from the superior vena cava/right atrial junction up to a tricuspid valve. The initial diagnosis of right atrial myxoma was made based on the echocardiography report and surgical excision was planned. Intraoperatively, the excision of the mass was deferred due to the extensive nature of the disease and a high suspicion of malignan

Long-term frequency gradients during persistent atrial fibrillation in sheep are associated with stable sources in the left atrium.

BACKGROUND: Dominant frequencies (DFs) of activation are higher in the atria of patients with persistent than paroxysmal atrial fibrillation (AF), and left atrial (LA)-to-right atrial (RA) DF gradients have been identified in both. However, whether such gradients are maintained as long-term persistent AF is established remains unexplored. We aimed at determining in vivo the time course in atrial DF values from paroxysmal to persistent AF in sheep and testing the hypothesis that an LA-to-RA DF difference is associated with LA drivers in persistent AF. METHODS AND RESULTS: AF was induced using

Sequential biatrial linear defragmentation approach for persistent atrial fibrillation.

BACKGROUND: The strategy for catheter ablation of persistent atrial fibrillation (AF) and the procedural end point remain controversial. OBJECTIVE: To evaluate the feasibility of a sequential defragmentation approach. METHODS: One hundred thirty-five patients (aged 62.4 ± 9 years; 76 long-standing persistent AF) underwent first ablation procedure for persistent AF. With an end point of AF termination, the ablation procedure was performed sequentially in the following order: pulmonary vein antrum isolation, linear defragmentation of complex fractionated electrograms at left atrial (LA) roof,

Anatomic localization of rapid repetitive sources in persistent atrial fibrillation: fusion of biatrial CT images with wave similarity/cycle length maps.

OBJECTIVES: The aim of this study was to investigate the anatomic distribution of critical sources in patients with atrial fibrillation (AF) by fusion of biatrial computed tomography (CT) images with cycle length (CL) and wave similarity (WS) maps. BACKGROUND: Experimental and clinical studies show that atrial fibrillation (AF) may originate from rapid and repetitive (RR) sources of activation. Localization of RR sources may be crucial for an effective ablation treatment. Atrial electrograms showing rapid and repetitive activations can be identified by combining WS and CL analysis. METHODS:

Fistula between right coronary artery vein graft and right atrium as an immediate complication of percutaneous coronary intervention.

Fistula between saphenous vein graft (SVG) and a cardiac chamber or structure is a rare complication after coronary artery bypass grafting (CABG). We report the first case of a fistula between SVG and the right atrium (RA) as an immediate complication after a percutaneous coronary intervention (PCI) in an 86-year-old female. She presented with inferior ST-elevation myocardial infarction (STEMI) and was treated with thrombolytic therapy in a peripheral hospital, which was unsuccessful. PCI to SVG to the right coronary (RCA) was complicated by a fistula to RA. Cardiac magnetic resonance (CMR) co

Right atrial B-cell lymphoma in a patient with ocular melanoma.

Primary cardiac lymphoma (PCL) is defined as a non-Hodgkin lymphoma involving only the heart or pericardium. PCL is extremely rare and often misdiagnosed. We report the case of a healthy 53-year-old male who originally presented with blurred vision secondary to a right intraocular mass. Enucleation of the eye confirmed a ciliary body melanoma and, upon further investigation, the patient was discovered to have a mass in the right atrium suspicious for a myxoma. However, resection of the atrial mass revealed a low-grade B-cell PCL. The occurrence of PCL in an immunocompetent patient being invest

Right atrial perforation at the end of an atrial fibrillation ablation procedure.

A 74-year-old man with chronic atrial fibrillation underwent ablation under conscious sedation. After sheath removal from the left atrium, the patient flexed his thighs, resulting in a 'foetal position' developing tamponade due to an right atrial (RA) appendage perforation from sheath migration. This illustrates the importance of close monitoring during sedation weaning, recommending removal of all sheaths prior to sedation withdrawal.

Accidental connection between the atrial and ventricular stylets during a stimulation threshold test for pacemaker implantation.

This report describes a case of atrioventricular crosstalk during a stimulation threshold test. This phenomenon was likely due to the accidental connection between the atrial and ventricular stylets. Electrocardiologists should recognize atrioventricular crosstalk during a stimulation threshold test to avoid repositioning the atrial lead unnecessarily or inducing ventricular arrhythmia.

Diagnostic and surgical issues in emergency presentation of a pelvic leiomyoma in the right heart.

A computed tomography scan in a 43-year-old woman with a nonsignificant previous medical history demonstrated an inferior caval mass prolapsing through the right atrium and the tricuspid valve. The mass was misdiagnosed as a thrombus-in-transit, and heparin was started. The clinical picture suddenly evolved into cardiogenic shock, and the patient underwent an emergency resection of the intracardiac portion of the mass. Macroscopic and microscopic features were consistent with leiomyoma. In the presence of an inferior caval mass, historical elements and computed tomography imaging that do not c

Isolated right superior vena cava drainage into the left atrium diagnosed noninvasively in the peripartum period.

Isolated right superior vena cava drainage into the left atrium is an extremely rare cardiac anomaly, especially in the absence of other cardiac abnormalities. Only 28 of 5,127 reported consecutive congenital cardiac cases involved superior vena cava drainage into the left atrium, and all were associated with other cardiac anomalies. Of 19 reported cases of right superior vena cava drainage into the left atrium, most patients have been children who were experiencing mild hypoxemia and cyanosis. Herein, we describe the case of a 34-year-old woman who presented with asymptomatic hypoxemia in the

Penetration of left and right atrial wall and aortic root by an Amplatzer atrial septal occluder in a nine year old boy with Marfan syndrome: Case report.

BACKGROUND: To describe complications associated with Amplatzer septal occluders in a patient with Marfan syndrome CASE PRESENTATION: A nine-year-old boy with Marfan syndrome and a 22 mm atrial septal defect (ASD) was treated successfully by interventional closure of his ASD by placing a 24 mm Amplatzer septal occluder. Follow up examinations showed a good result but an increasing enlargement of aortic root, so the patient was scheduled for operation. Intraoperative findings showed a perforation of the left atrial roof and the non-coronary sinus by penetration of the occluder device as well a

Currarino syndrome shown by prenatal onset ventriculomegaly and spinal dysraphism.

The most frequent cause of ventriculomegaly is spina bifida when associated with Arnold-Chiari type II malformation. We report on a prenatal diagnosis of severe ventriculomegaly in association with spinal dysraphism that was indicative of a Currarino syndrome (CS) due to a c.584delA, p.H195fsX28 truncated mutation within the HLXB9 gene. Physiopathology of the ventriculomegaly is discussed in reference to the fetopathological examination and CS embryopathology. In the present case, prognosis was poor and pregnancy termination was authorized. However, such a decision may be controversial in fetu

An inverted left atrial appendage mimicking an intraatrial thrombus after a ross operation.

Inverted left atrial appendage (ILAA) is a rare manifestation. The case is reported of a patient with a history of factor V Leiden who underwent a Ross operation. An intra-atrial mass was discovered one month postoperatively and suspected to be a thrombus. Despite six months' anticoagulant treatment, the intra-atrial mass persisted. Ultimately, the patient was reoperated on due to hemostatic risks factors, and an ILAA was found. The diagnosis of ILAA remains a major challenge. Despite widespread use of postoperative echocardiography, the few reported cases confirm ILAA to be a rare phenomenon.
