Erythema nodosum (EN) is a hypersensitivity reaction associated with many diseases. We describe a family in which 4 sisters had acute or recurrent EN. HLA typing showed a common haplotype in the affected members of the family. A review of familial EN and HLA distribution in EN is presented.

Two cases of anaphylactoid reaction from hypersensitivity to celery are reported. The two patients were also allergic to ragweed and mugwort pollen. The RAST inhibition test suggested slight cross-allergenicity between celery and mugwort pollen. The common allergenic factor could have been the alpha-methylene gamma-butyrolactone group.

Coccidioidomycosis skin testing appears to be uncommon, based on US health insurance claims data. Patient demographic features were consistent with the approval of the test for adults, but few patients had previous coccidioidomycosis diagnosis codes supporting its use for detecting delayed-type hypersensitivity in those with a history of pulmonary coccidioidomycosis.

Antiseptics are widely used in medical practice. Their cutaneous secondary effects such as allergic contact dermatitis are well known. However, anaphylactic reactions are less. The scope of this article is to describe antiseptics currently used which cause immediate hypersensitivity reactions. Finally, the diagnostic tools and therapeutic approach will be discussed.

A case of generalized granuloma annulare is described in a patient with Hodgkin's disease 3 weeks after autologous bone marrow transplantation (BMT). This is, to our knowledge, the first report of generalized granuloma annulare post-BMT. Immunological pathogenesis is suggested and the association with delayed-type hypersensitivity reaction is discussed.

A potentially serious complication of epidural bupivacaine used for a repeat, elective Caesarean section is presented. The main features of this reaction were erythema, itching, generalised and pulmonary oedema. In addition, pulmonary oedema was subsequently noted in the baby. An immunological hypersensitivity to bupivacaine or lignocaine is the likely cause.

Bufenox, a diuretic with a sodium-uretic and light potassium--uretic effect, has an immediate but short-term effect, stimulates IgM antibody production, depressed delayed-type hypersensitivity. The immunomodulating effect of bufenox may be due to changes in Na+ ions concentrations since sodium load abrogates immunoactive properties of the drug.

Stevens-Johnson syndrome is an immune mucocutaneous disorder which can follow viral infections, mycoplasma infections and the ingestion of some drugs. There is also evidence supporting this disease as a hypersensitivity disorder. We observed a four-year-old girl with Stevens-Johnson syndrome attributed to ingestion of salmon berries (Rubus spectabilis).

Acute intraperitoneal rupture is a rather rare complication of hepatic hydatid cysts; anaphylaxis and sudden death have frequently been reported in such cases. In the present report we are presenting a case of acute traumatic intraperitoneal rupture of a huge hydatid cyst to which the patient showed no hypersensitivity reactions.

Circulatory collapse and respiratory arrest occurred in a young woman shortly after taking 200 mg of zomepirac for relief of headache. She had no history of drug hypersensitivity and had tolerated zomepirac without adverse effects 18 months prior to this reaction. The patient was successfully treated and suffered no sequelae.

In cases of conjunctivitis associated with the use of soft lenses, delayed hypersensitivity to disinfecting components of the used soft lens liquids has to be considered. In 7/15 patients, positive patch test reactions to merthiolate, chlorhexidine or the soaking solutions were observed. The significance of these findings is discussed.

We compared bridging and nonbridging coronary artery segments with respect to the vasoconstrictor effect of acetylcholine. Bridging segments were hypersensitive to the constrictor effect of acetylcholine, and results suggest that the effect of nitric oxide on the acetylcholine-stimulated condition is decreased, or that the smooth muscle sensitivity to acetylcholine is increased.

In medical literature, there are many cases reported of anaphylaxis due to fruits, before or after allergic reactions to latex. We present two atopic patients, with allergy to latex (by clinical history, positive skin test and RAST). Both patients showed a positive skin test and RAST to fruits (banana, kiwi, avocado and chestnut), but clinically caused no symptoms.
