Indwelling bowel management system as a cause of life-threatening rectal bleeding.

A 79-year-old male was transferred to the intensive care unit for postoperative respiratory support. An indwelling bowel management system was inserted for containment of noninfective diarrhoea. Following only 11 days of continual use the patient developed life-threatening rectal bleeding. Preoperative normal rectal mucosa and anatomy were documented. There was no evidence of postoperative coagulopathy. Mesenteric angiography identified bleeding from a branch of the superior rectal artery. Rectal mucosa pressure necrosis secondary to the indwelling Flexi-Seal® Fecal Management System was diagn

Genital ambiguity with a Y chromosome: does gender assignment matter?

Recommendations for sex of rearing in newborns with genital ambiguity, testicular differentiation and a Y chromosome continue to be challenging. Complaints from former patients have forced those providing the medical, surgical and psychological care for these individuals to reassess evaluation and treatment strategies. In this paper, the histories of six patients born with genital ambiguity and at least partial testicular differentiation with a karyotype containing a Y chromosome are presented. Three of these patients were assigned as males and three as females. The factors involved in these i

[New views on social phobias].

Social phobia is a very frequent mental disorder affecting in some stage of life up to 13% of the population, and it may be associated with serious health complications (frequent comorbidity with affective and other anxiety disorders, with addictions, more frequent suicidal behaviour) and socioeconomic consequences (increased health care consumption, trend of lower education, poorer adaptation to work, higher demands on social support). The concept of this disorder was in modern classification systems (DSM and ICD) adopted only recently at the beginning of the eighties. The author describes ba

Patient- and provider-level determinants of self-assessed health and well-being among adults with chronic conditions in Slovenian primary care.

BACKGROUND: Chronic diseases significantly impact patients’ quality of life and healthcare systems. In Slovenia, understanding the factors influencing self-assessed health and well-being can support more effective primary care policies. The purpose of this study was to identify patient- and provider-level factors associated with general health and well-being among Slovenian adults with chronic conditions. METHODS: This cross-sectional study was conducted as part of the OECD-led PaRIS initiative. Data were collected from 2133 patients aged ≥ 45 years with at least one chronic condition and 70 p

Health Care Providers' Perspectives of Clinical Decision Support Tools for Pediatric Sepsis in Bangladesh: Qualitative Study.

BACKGROUND: Sepsis, a life-threatening condition resulting from a dysregulated immune response to infection, disproportionately affects children in low- and middle-income countries (LMICs). Children with sepsis in LMICs face high mortality rates, with early detection and clinical monitoring posing significant challenges to effective management. There is great potential for digital technologies, such as wearable biosensor devices and mobile health (mHealth) clinical decision support (CDS) tools, together referred to as clinical decision support systems (CDSSs), to enable closer monitoring and m

MiCare study, an evaluation of structured, multidisciplinary and personalised post-ICU care on physical and psychological functioning, and quality of life of former ICU patients: a study protocol of a stepped-wedge cluster randomised controlled trial.

INTRODUCTION: Over 70% of the intensive care unit (ICU) survivors suffer from long-lasting physical, mental and cognitive problems after hospital discharge. Post-ICU care is recommended by international guidelines, but evidence for cost-effectiveness lacks. The aim of this study is to evaluate the clinical effectiveness and cost-effectiveness of structured, multidisciplinary and personalised post-ICU care versus usual care on physical and psychological functioning and health-related quality of life (HRQoL) of ICU survivors, 1- and 2-year post-ICU discharge. METHODS AND ANALYSIS: The MONITOR-I

Economic evaluation of non-invasive test pathways for high-risk metabolic dysfunction-associated steatotic liver disease (MASLD) in the United Kingdom (UK).

INTRODUCTION AND OBJECTIVES: Non-invasive tests (NITs) identifying high-risk MASLD in primary care is suggested but, these strategies cost-effectiveness remain uncertain in the United Kingdom (UK). MATERIALS AND METHODS: A cost-utility/budget impact model was developed for cost-effectiveness evaluation of two screening strategies (1) FIB-4 followed by Enhanced Liver Fibrosis (ELF) (FIB-4/ELF); (2) FIB-4 followed by Transient Elastography (FIB-4/TE) compared to standard of care (SoC). A cohort of primary care MASLD patients with an advanced fibrosis prevalence of 4.20 % was simulated. A decisi

Perspectives and Experiences of Adult Patients With Obesity in Dietetic Primary Health Care: A Qualitative Study in the Netherlands.

BACKGROUND: Obesity is defined as a chronic, relapsing disease, characterised by its persistence and the frequent recurrence of weight gain following weight loss. It is linked to chronic diseases and reduced quality of life. Sustainable weight loss requires long-term dietary and behavioural changes. Dietetic treatment outcomes vary per patient, leading to differing success rates and effectiveness. Although understanding patient experiences is crucial for designing effective, patient-centered strategies, little is known about their lived experiences. This study aimed to obtain a better understa

Occupations and balance during the transition to motherhood with a lifetime chronic illness: A scoping review examining cystic fibrosis, asthma, and Type-1 diabetes.

INTRODUCTION: Throughout the transition to motherhood, changes are experienced across a woman's physical, mental, social, and occupational self. Maternal chronic illness adds the complexity of increased healthcare needs and navigating a high-risk, medicalised pregnancy, birth, and post-natal period. Literature concerning motherhood transitions in chronic illness generally focusses on the mother's medical health and pregnancy outcomes; little is known about the impacts on women's occupations, balance, and quality of life. Understanding these issues may help support women in a more tailored and

Minding the gap: access to palliative care and the homeless.

BACKGROUND: With an ever increasing number of individuals living with chronic and terminal illnesses, palliative care as an emerging field is poised for unprecedented expansion. Today's rising recognition of its key role in patients' illnesses has led to increased interest in access to palliative care. It is known that homelessness as a social determinant of health has been associated with decreased access to health resources in spite of poorer health outcomes and some would argue, higher need. This article aims to discuss the current state of affairs with regards to accessing palliative care
