Effectiveness of an Interprofessional and Multidisciplinary Musculoskeletal Training Program.

BACKGROUND: Musculoskeletal (MSK) problems are common, and a recent US Bone and Joint Initiative calls for new models of education and professional collaboration. Evidence of feasibility and acceptability of innovative methods are needed. OBJECTIVE: We assessed if an experimental immersion interdisciplinary MSK curriculum would be acceptable to residents from different specialties, be feasible within existing rotations, and be effective in strengthening clinical skills. METHODS: Through funding from the Veterans Affairs Office of Academic Affiliations and the Office of Specialty Care, we dev

Load Carriage-Related Paresthesias: Part 1: Rucksack Palsy and Digitalgia Paresthetica.

This is the first of a two-part article discussing loadcarriage- related paresthesias, including brachial plexus lesions (rucksack palsy), digitalgia paresthetica, and meralgia paresthetica. Paresthesias are sensations of numbness, burning, and/or tingling, usually experienced as a result of nerve injury, compression, traction, or irritation. Rucksack palsy is a traction or compression injury to the brachial plexus, caused by the shoulder straps of the rucksack. The patient presents with paresthesia, paralysis, cramping with pain, and muscle weakness of the upper limb. Muscle-strength losses a

Trunk restraint therapy: the continuous use of the harness could promote feedback dependence in poststroke patients: a randomized trial.

The objective of this study was to evaluate the long-term effects of the task-specific training with trunk restraint compared with the free one in poststroke reaching movements. The design was randomized trial. The setting was University of Campinas (Unicamp). Twenty hemiparetic chronic stroke patients were selected and randomized into 2 training groups: trunk restraint group (TRG) (reaching training with trunk restraint) and trunk free group (TFG) (unrestraint reaching). Twenty sessions with 45 minutes of training were accomplished. The patients were evaluated in pretreatment (PRE), posttreat

[Calcifications after operations of the subacromial space].

OBJECTIVE: Although there are several studies on the development of periarticular calcifications after different operation procedures of the joints, such calcifications have only seldomly been described in connection with operations in the subacromial space. Therefore, the present study was aimed at investigating the incidence of periarticular calcifications after operations in the subacromial space and to assess their clinical relevance. PATIENTS AND METHODS: In a retrospective study we examined 152 patients (51 female, 101 male) who had been operated on for primary shoulder impingement by o

[Tendon ruptures of the shoulder].

Common sports, involving raising the arms above the head, i.e., throwing, racquet games and swimming, often result in rotator cuff tendinitis. During the throwing motion, the humeral head and its overlying biceps tendon and rotator cuff must pass rapidly under the coraco-acromial arch. Damage to these structures can occur by several mechanism. First, an increase in the size of the structures passing underneath the arch may lead to impingement. This can occur either by way of hypertrophy of the musculotendinous cuff or by way of inflammation of the cuff. Second, a decreased space available unde

Radial-to-Axillary Nerve Transfer Resolves Symptoms of Axillary Nerve Injury Due to Proximal Humerus Fracture-Dislocation in an Elderly Patient Treated With Hemiarthroplasty.

Proximal humerus fractures in elderly patients are a common injury that can often be treated nonoperatively. However, surgery is indicated with some fracture patterns. Arthroplasty is an attractive option with poor bone quality, when there is a low likelihood of success with open reduction and internal fixation, and due to a timely return to function and weight bearing of the extremity in this patient population. A prerequisite for shoulder function for both native and replacement joints is a functional deltoid. Unfortunately, elderly patients with complex fracture patterns can sustain axillar

A Case Study on Successful Ayurvedic Management of a Rare Case of Reiter's Syndrome.

Reiter's syndrome is a clinical syndrome of arthritis, urethritis, conjunctivitis, and mucocutaneous lesions. Skin and mucosal involvement is observed in about 10% of the cases. A 34-year-old male was brought in severe condition. He had red colored foul smelling maculopapular skin lesions all over body, swollen and painful knee and shoulder joints. History revealed that he was suffering from Reiter's syndrome since 2003. He used to get admitted between the months of February and May every year due to aggravation of symptoms. Every time he was treated with systemic antibiotics, corticosteroids,…

Adhesive capsulitis of the hip: a review.

Adhesive capsulitis of the hip (ACH) is a rare clinical entity. Similar to adhesive capsulitis of the shoulder, ACH is characterized by a painful decrease in active and passive range of motion as synovial inflammation in the acute stages of the disease progresses to capsular fibrosis in the chronic stages. Once other diagnoses have been ruled out, management of ACH is tailored to reduce inflammation in the acute stages with NSAIDs, intra-articular steroid injections, and targeted physical therapy while biomechanical dysfunction in the spine, hip, sacroiliac joint, or lower limb joints is addre

Synovial chondromatosis of the temporomandibular joint.

Synovial Chondromatosis (SC) is a disease whose etiology is unknown, can be defined as a benign synovial process characterized by the formation of metaplastic cartilaginous nodes inside connective tissue of articular surfaces, is considered an active metaplastic phenomenon better than a neoplastic process; it presents a greater preference to affect women who constitute almost 70% of reported cases, the age range is wide and oscillates between 18-75 years (average 44.6 years). Between the main clinical findings are: pain, crackle, volume augmentation and a limited buccal opening. SC is an unusu

Clinical and radiologic outcomes of surgical and conservative treatment of type III acromioclavicular joint injury.

The management of acute acromioclavicular joint dislocations is controversial. The purpose of this study was to compare the incidence of posttraumatic anatomic alterations after surgical or conservative treatment of type III injuries and to analyze their effect on the outcome. Forty-three patients were evaluated retrospectively, clinically and radiographically, at a 12-month minimum follow-up. Thirty-two were treated surgically, using the Phemister technique, and 11 had conservative treatment. A comparison of the overall clinical results in both groups showed no statistically significant diffe

Resection interposition arthroplasty of the shoulder affected by inflammatory arthritis.

To determine the therapeutic value of resection-interposition arthroplasty (RIAP) of shoulders in rheumatoid or other inflammatory arthritis, 53 patients were reviewed clinically and radiographically after a mean follow-up period of 8.2 +/- 4.3 years (3.5 to 17.5 years). The Constant Score averaged 42.33 +/- 16.2 (12 to 76). Patients with a follow-up period of more than 10 years performed significantly worse than those with shorter follow-up periods. In most cases, radiographs showed a progressive medial displacement and loss of size of the humeral head as well as a decrease of the joint space

[Prospects and utilization of xeroradiography in the osteoarticular diagnosis and study of soft parts].

Xeroradiography and its physical phenomena are briefly described and attention is drawn to its offer of marked contrast and hence greater perception of details than conventional radiography. A comparison is made between ordinary and xero pictures of the spine, mediastinum and lungs with regard to the definition and identification of individual parts. The literature dealing with results obtained in areas where xeroradiography is best employed (joints and accompanying soft parts) is reviewed and personal experience in this field, based on a nosographic criterion and specific diagnostic choice, i

Reliability of classifications derived from Cyriax's resisted testing in subjects with painful shoulders and knees.

STUDY DESIGN: Intrarater and interrater reliability. OBJECTIVES: Examine intrarater and interrater reliability of the resisted-testing component of Cyriax's selective tension testing for patients with painful shoulders and knees. BACKGROUND: Clinicians make diagnostic and intervention decisions about lesions in contractile tissues based on resisted testing. Diagnostic and intervention decisions require reliable data gathering, especially when more than 1 physical therapist manages a patient. No studies have examined agreement of the results of the resisted tests used in selective tension tes

[Results of an inverted shoulder prosthesis after resection for tumor of the proximal humerus].

PURPOSE OF THE STUDY: Treatment of primary bone tumors of the proximal humerus is a major challenge, both in terms of oncological cure and limitation of functional handicap. Extracorporal radiation and reimplantation have provided satisfactory results for the treatment of bone tumors and the inverted shoulder prosthesis has be found to be effective in treating rotator cuff deficiency. We therefore assessed patients treated with a combination of these two treatments in order to search for a better functional outcome. MATERIAL AND METHODS: Conservative resection of the tumor with reconstruction

Muscle strength characteristics and the predictive value of handgrip strength in young obese males: A multi‑joint isokinetic and LASSO regression analysis.

Obesity is a primary global public health concern, often associated with impaired muscle function and an increased risk of exercise-related injuries. This cross-sectional study aimed to elucidate the associations among muscular strength, body load, and functional capacity in young adults with obesity. A total of 75 male university students were recruited, including a normal-weight group (n = 47, BMI 18.5-24.99) and an obese group (n = 28, BMI ≥ 30). Assessments included handgrip strength, one-repetition maximum (1RM) for squat, deadlift, and bench press, as well as isokinetic strength evaluati

Radiosynovectomy in hemophilic synovitis: correlation of therapeutic response and blood-pool changes.

AIM: The aim of this study was to evaluate the efficacy of 90Y and 186Re radiosynovectomy in patients with hemophilic synovitis. METHODS: Radiosynovectomy was performed in 32 joints of 20 patients with hemophilic synovitis by using 90Y citrate colloid in the knee and 186Re sulfide colloid in the elbow, shoulder, and ankle. The indication for radiosynovectomy was the continuous presence of intra-articular blood or effusion and three or more hemorrhages into the same joint within the last 6 months. Response to therapy was first assessed at the 4th month with blood-pool imaging. Patients were fo

One-Stage or Two-Stage Revision for Infected Shoulder Arthroplasty: A Head-to-Head Meta-Analysis of Comparative Studies.

BACKGROUND: Periprosthetic joint infection (PJI) following shoulder arthroplasty is a devastating complication associated with substantial morbidity and revision burden. While 2-stage revision has traditionally been regarded as the gold standard for chronic shoulder PJI, single-stage revision has emerged as a potential alternative. The aim of this systematic review was to directly compare infection-related outcomes and complications between single-stage and 2-stage revision arthroplasties for shoulder PJI. METHODS: This systematic review was prospectively registered with PROSPERO (CRD42025115

Local Infiltration Analgesia Versus Interscalene Block for Pain Management Following Shoulder Arthroplasty: A Prospective Randomized Clinical Trial.

BACKGROUND: While providing effective analgesia following shoulder arthroplasty, an interscalene block has known complications. Local infiltration analgesia (LIA) using ropivacaine has been successfully employed in other joint arthroplasties, but its efficacy in shoulder arthroplasty has not been studied extensively. The purpose of this study was to compare pain and opioid consumption between LIA and an interscalene block following shoulder arthroplasty. METHODS: Patients undergoing primary shoulder arthroplasty were prospectively randomized into 2 groups: the block group received an intersca
