Successful device closure of a post-infarction ventricular septal defect.

Ventricular septal defect (VSD) is a lethal complication of myocardial infarction. The event occurs 2-8 days after an infarction and patients should undergo emergency surgical treatment. We report on successful device closure of post-infarction VSD. A previously healthy 66-year-old male was admitted with aggravated dyspnea. Echocardiography showed moderate left ventricular (LV) systolic dysfunction with akinesia of the left anterior descending (LAD) territory and muscular VSD size approximately 2 cm. Coronary angiography showed mid-LAD total occlusion without collaterals. Without percutaneous

Forgetting heart break: a fascinating case of transient left ventricular apical ballooning syndrome associated with dissociative amnesia.

Transient left ventricular apical ballooning syndrome (TLVABS), also known as takotsubo cardiomyopathy, is characterized by transient left ventricular dysfunction, electrocardiographic changes, and release of myocardial enzymes that mimic acute myocardial infarction in patients without angiographic evidence of coronary artery disease. Most patients are post-menopausal women and an emotional or physiologic stressor frequently precedes the presentation. Psychogenic or dissociative amnesia is a memory disorder characterized by sudden retrograde memory loss with inability to recall personal inform

Takotsubo cardiomyopathy associated with titration of duloxetine.

Takotsubo cardiomyopathy is characterized by transient multisegmental left ventricular dysfunction, dynamic electrocardiographic changes that mimic acute myocardial infarction, and the absence of obstructive coronary disease. Takotsubo cardiomyopathy has been solidly associated with antecedent emotional and physical stressors that trigger catecholamine surges, which lead to coronary vasospasm or direct myocardial injury. Some medications can also cause catecholamine surges, although this phenomenon is not as well described. Duloxetine is a combined serotonin and norepinephrine reuptake inhibit

Tachycardiomyopathy induced by ventricular premature complexes: complete recovery after radiofrequency catheter ablation.

Ventricular premature complexes (VPCs) are known to be one of the most benign cardiac arrhythmias when they occur in structurally normal hearts. We experienced a 32-year old man who presented with dyspnea, palpitations and very frequent VPCs (31% of the total heart beats). Echocardiography revealed a dilated left ventricle (LV 66 mm at end-diastole and 57 mm at end-systole) and a decreased ejection fraction (34%). Very frequent VPCs had been detected 10 years previously and he underwent a failed radiofrequency catheter ablation (RFCA) procedure at that time. The patient had been treated with h

Permanent junctional reciprocating tachycardia: an incessant tachycardia in children.

UNLABELLED: Permanent junctional reciprocating tachycardia is an infrequent form of reentrant supraventricular tachycardia but is the commonest incessant tachycardia in childhood. The persistent nature of the tachycardia can lead to heart failure secondary to left ventricular dysfunction and the arrhythmia is often refractory to drug control. STUDY OBJECTIVE: To review the main clinical features and therapeutic options currently available for this arrhythmia. DESIGN: Retrospective study. PATIENTS: A group of 5 children with a diagnosis of permanent junctional reciprocating tachycardia. RES

Cardiac valve surgery in patients with poor left ventricular function.

Of 316 patients who underwent operation for valvular heart disease at the Toronto Western Hospital, between January 1978 and December 1981, 41 (31 men, 10 women), ranging in age from 24 to 74 years, had severely impaired left ventricular function (ejection fraction less than 40% and left ventricular end-diastolic pressure more than 18 mm Hg). All 41 patients were in New York Heart Association (NYHA) class III or IV. Fourteen patients had disease of the aortic valve, 13 of the mitral valve and 14 of both aortic and mitral valves. Twenty-two patients also had serious coronary artery disease. The

Reverse Takotsubo Syndrome in Women During High Hormonal States Related to In Vitro Fertilization.

Takotsubo syndrome (TTS) has heterogenous clinical presentations for which the pathophysiologic mechanisms remain incompletely understood. Identifying patterns to the variants of TTS, including reverse TTS, may provide insight into TTS pathophysiology. In this report, we share 3 cases of reverse TTS in premenopausal women at different stages of in vitro fertilization, suggesting a link between heightened hormonal states and midbasal left ventricular dysfunction in the setting of TTS.

Diffuse hypertrophic obstructive cardiomyopathy complicated by apical ventricular aneurysm and excessive left ventricular trabeculation: a case report.

This article reports a rare case of diffuse hypertrophic obstructive cardiomyopathy complicated with left ventricular apical aneurysm and excessive trabeculation. Genetic testing of the patient revealed a heterozygous variant of the MYBPC3 gene (c.3343G> A: p.Val1115Ile). Despite optimal medical therapy and implantable cardioverter-defibrillator (ICD) implantation, the patient developed progressive cardiac dysfunction and recurrent ICD shocks. We discuss the clinical course, genetic findings, and imaging features of this case.

Cardiac arrest in a patient with trichorhinophalangeal syndrome and dilated cardiomyopathy.

A 44-year-old woman with known trichorhinophalangeal syndrome presented with an unheralded out of hospital cardiac arrest. Transthoracic echocardiography showed severe left ventricular systolic dysfunction with an ejection fraction <25% and cardiac MRI confirmed a diagnosis of congenital non-ischaemic dilated cardiomyopathy. The case highlights a very rare syndrome, it is previously unknown association with dilated cardiomyopathy and the possible benefit of cardiac screening for patients with known trichorhinophalangeal syndrome.

Cardiovascular complications of systemic sclerosis: What to look for.

Systemic sclerosis, an autoimmune disease characterized by fibrosis of the skin and various internal organs, is associated with cardiovascular abnormalities including pulmonary hypertension, atherosclerosis, right and left ventricular dysfunction, arrhythmias, conduction defects, pericardial disease, and valvular heart disease. Clinicians caring for patients with this disease should regularly screen for cardiac symptoms, and patients with abnormal findings should be managed in conjunction with a cardiologist to optimally modify cardiovascular risks.

A rare case of mitral valve dissection and aortic-left ventricular tunnel associated with possible autoimmune vasculitis.

We report a very rare case of mitral valve dissection and aortic-left ventricular tunnel caused by possible autoimmune vasculitis. We suspected Behcet's disease in this patient. There was no obvious clinical evidence of infective endocarditis. Echocardiography is the diagnostic tool of choice to recognize valvular dysfunction, related pathology, and possible complications. The patient may require immunosuppressive therapy due to the high likelihood of recurrence in the perioperation period.

Fenestrated ASD device "angioplasty": How to adjust a "pop-off" mechanism when needed.

We report on three cases of atrial septal defect (ASD) closure where a handmade fenestration had to be modified after device implantation. Two patients suffered from left ventricular diastolic dysfunction and one had severe pulmonary arterial hypertension. Amplatzer ASD occluder fenestration was created in all patients. Devices were implanted using an "over-the-wire" implantation technique. Due to a sub-optimal hemodynamic result, fenestration was upsized by using a new technique.

Takotsubo cardiomyopathy in a snake bite victim: a case report.

Takotsubo cardiomyopathy occurs in patients with severe emotional or physiologic stress. The prognosis is usually favorable, and the left ventricular wall motion dyskinesis normalizes within days to weeks. In this paper we report a case of snake bite complicated by takotsubo cardiomyopathy. We advise physicians to consider not only cardioinhibitory effect of snake venom but also takotsubo cardiomyopathy as a differential diagnosis if the patient develops cardiac dysfunction following snake bite.

Necrotizing fasciitis causing severe myocardial dysfunction with ST-segment elevation in a young man.

Necrotizing fasciitis is a life-threatening infection of the fascia and subcutaneous tissues. We report on a 30-year-old man with history of intramuscular injection resulted in gluteal abscess that progressed to necrotizing fasciitis. On admission, the patient developed circulatory collapse, severe left ventricular dysfunction, and ST-segment elevation in the inferior leads. Wound and blood cultures indicated staphylococcal infection. The clinical, laboratory, echocardiographic, and electrocardiographic features are discussed.

Heart failure: recent advances in prevention and treatment.

The most important advance in heart failure during the past decade has been the recognition that medications inhibiting neurohormonal activation relieve symptoms, reduce hospitalizations, and prolong survival in patients with heart failure from left ventricular systolic dysfunction. Recent trials with angiotensin-converting enzyme inhibitors, angiotensin receptor antagonists, aldosterone antagonists, and beta blockers have provided valuable information regarding the uses, dosing, and extent of therapeutic benefits of neurohormonal inhibition.

Reversible catecholamine-induced cardiomyopathy presenting as acute pulmonary edema in a patient with pheochromocytoma.

Cardiac injury caused by elevated levels of circulating catecholamines is a well-known phenomenon. We report a patient who, secondary to transient regional left ventricular dysfunction (despite normal coronary arteries), developed acute pulmonary edema. Echocardiography showed marked anteroapical hypokinesia and an apical thrombus. The diagnosis of pheochromocytoma was made by plasma catecholamine levels, abdominal ultrasound and finally at operation. The patient recovered completely long before surgical intervention was undertaken.

Elevated Pulmonary Artery Wedge Pressure in Group 1 Pulmonary Hypertension.

BACKGROUND: With pulmonary hypertension (PH), a pulmonary artery wedge pressure (PAWP)>15 mm Hg is used to diagnose left heart dysfunction, but some patients with adjudicated group 1 PH demonstrate PAWP>15 mm Hg. The primary objective of the study was to evaluate group 1 PH with high PAWP>15 mm Hg. METHODS: Patients with adjudicated group 1 PH from PVDOMICS between 2016 and 2019 were separated into high PAWP(>15 mm Hg) or normal PAWP and compared with adjudicated combined pre- and postcapillary (Cpc) PH related to heart failure with preserved ejection fraction (HFpEF). Participants underwent

Does LD100 E coli shock cause myocardial failure?

We have documented that myocardial dysfunction occurs in canine endotoxin shock and have designed this study to determine the effect of lethal live E coli-induced shock on the myocardium. Small adult heart "donor" dogs (wt range 6-9 kg) were infused with LD100 E coli (N = 12) or saline (N = 16) for 30 minutes. Two hours later, heart transfer surgery was initiated and once completed the isolated working left ventricle was allowed to equilibrate in the extracorporeal circuit of a "support" dog (wt range 22-32 kg). Myocardial performance was then evaluated by changing mean aortic pressure while c
