Ankle joint contact forces and muscle contributions during walking differentiate chronic ankle instability from copers and uninjured controls.

BACKGROUND: Chronic ankle instability (CAI) is associated with abnormal joint loading and an increased risk of early joint degeneration, yet the underlying biomechanical mechanisms remain unclear. It is unknown whether altered ankle loading reflects chronic effects of repeated ankle sprains rather than a single traumatic event, and how muscle forces contribute to these differences. METHODS: Ankle joint contact forces (JCF) and muscle contributions during walking were compared among 21 uninjured controls, 21 copers (individuals with a prior ankle sprain but no persistent symptoms), and 21 indi

Effects of local muscle vibration on plantarflexor strength and activation in individuals with chronic ankle instability: a randomized crossover trial.

BACKGROUND: Chronic ankle instability (CAI) is associated with neuromuscular impairments that increase reinjury risk. Although vibration can enhance neuromuscular function, the effects of local muscle vibration (LMV) in individuals with CAI are unknown. This study examined the immediate effects of LMV on plantarflexor strength and activation and whether stance position influences these responses. METHODS: Twenty individuals with CAI (age, 21±2 years; 3±1 lifetime ankle sprains) participated. All participants met the inclusion criteria for CAI established by the International Ankle Consortium.

Functional Assessment of Peroneal Muscles Using Ultrasound Imaging in Chronic Ankle Instability.

BACKGROUND: Lateral ankle sprains (LAS) result in chronic ankle instability (CAI), causing ongoing instability. Although peroneal muscle weakness is documented in CAI, surface electromyography shows similar activation patterns between CAI and healthy individuals, suggesting structural rather than neural deficits. Ultrasound imaging (USI) uniquely enables noninvasive assessment of muscle morphology and quality through cross-sectional area and echogenicity measurements. However, previous USI studies examined peroneals only in nonweight-bearing positions, potentially missing functional deficits.

Assessing the role of posterior to anterior calcaneal screw fixation in tibiotalocalcaneal nailing after ankle trauma: A retrospective comparative study.

Tibiotalocalcaneal (TTC) nailing is commonly used to treat severe ankle trauma, especially in patients with poor bone quality or compromised soft tissue. Although biomechanical studies suggest that posterior-to-anterior (P-A) screws enhance stability, the clinical impact of these screws remains uncertain. This study aims to compare complication rates between patients who underwent TTC nailing with and without P-A screws after ankle trauma. A retrospective analysis was conducted on 33 patients who underwent TTC nailing for ankle trauma between 2016 and 2022. Patients were divided into two group

Differences in gait analysis and clinical outcome after dynamic fixation or screw fixation in acute syndesmosis tear: a prospective randomized pilot study.

INTRODUCTION: Acute syndesmosis tears can be treated by static screw or dynamic fixation. Various studies have compared these techniques regarding postoperative outcome. However, to our knowledge, no study has used 3D-instrumented gait analysis (IGA). We hypothesized that a dynamic fixation would perform non-inferior to screw fixation in terms of biomechanical and clinical outcomes. MATERIALS AND METHODS: Patients were prospectively randomized to both groups. All patients received the same follow-up rehabilitation and consultations (6 and 12 weeks; 6 and 12 months) postoperatively. Standardiz

Adding Neurofeedback Training to Neuromuscular Training for Rehabilitation of Chronic Ankle Instability: A 3-Arm Randomized Controlled Trial.

BACKGROUND: Neurofeedback training (NFT) can aid in the treatment of the abnormal patterns of the brain brought on by physical injury, enhancing cognitive and behavioral abilities. The present study aimed to compare the effectiveness of combining neuromuscular training (NMT) and NFT (NMT+NFT) with NMT alone in rehabilitating athletes with chronic ankle instability (CAI). HYPOTHESIS: NMT+NFT will be more effective than NMT alone. STUDY DESIGN: A 3-arm, single-blind randomized controlled trial. LEVEL OF EVIDENCE: Level 2. METHODS: A total number of 62 athletes, aged 18 to 25 years, with CAI,

Proximal Adaptations in Chronic Ankle Instability: Systematic Review and Meta-analysis.

INTRODUCTION: Chronic ankle instability (CAI) is known to induce impairments throughout the lower quarter kinetic chain, however there is currently no synthesized information on proximal adaptations of the trunk, hip, thigh, and knee for neuromuscular and biomechanical outcomes during strength, balance, jumping, and gait among CAI patients. The purpose of this systematic review and meta-analysis was to synthesize trunk, hip, thigh and knee neuromuscular and biomechanical outcome measures during functional assessments when comparing CAI to healthy groups. METHODS: Cumulative Index of Nursing a

Comparative Effectiveness of Plantar-Massage Techniques on Postural Control in Those With Chronic Ankle Instability.

CONTEXT: One of the major concerns after an acute lateral ankle sprain is the potential for development of chronic ankle instability (CAI). The existing research has determined that clinician-delivered plantar massage improves postural control in those with CAI. However, the effectiveness of self-administered treatments and the underlying cause of any improvements remain unclear. OBJECTIVES: To determine (1) the effectiveness of a self-administered plantar-massage treatment in those with CAI and (2) whether the postural-control improvements were due to the stimulation of the plantar cutaneous

Altered Kinematics and Time to Stabilization During Drop-Jump Landings in Individuals With or Without Functional Ankle Instability.

CONTEXT: It has been proposed that altered dynamic-control strategies during functional activity such as jump landings may partially explain recurrent instability in individuals with functional ankle instability (FAI). OBJECTIVE: To capture jump-landing time to stabilization (TTS) and ankle motion using a multisegment foot model among FAI, coper, and healthy control individuals. DESIGN: Cross-sectional study. SETTING: Laboratory. PATIENTS OR OTHER PARTICIPANTS: Participants were 23 individuals with a history of at least 1 ankle sprain and at least 2 episodes of giving way in the past year

Influence of a proprioceptive training on functional ankle stability in young speed skaters - a prospective randomised study.

The influence of a 12-week-proprioceptive training on functional ankle stability was investigated in young speed skaters. Twenty-eight speed skaters were randomly divided into an intervention (n = 14) and into a control group (n = 14). A 15-min circle training was performed 5 times per week over a 12-week period. Measurements were taken prior to the training, after 6 and 12 weeks of training. Kinaesthesia was evaluated with the Isomed2000 in all movements of the ankle joint. Dynamic balance was tested with the Biodex Stability System at the stable level 8 and at the unstable level 2, measuring

Synthetic osteochondral grafting of ankle osteochondral lesions.

BACKGROUND: The treatment of osteochondral lesions, especially those with a cystic component or those that have failed the standard arthroscopic procedures can be challenging. Synthetic grafts have several potential advantages over other second line treatments including the fact that only one operation is required and no other joint is violated. METHOD: We report the results of the first series of synthetic grafts used in the treatment of osteochondral lesions of the ankle with a minimum of one year follow up. RESULTS: There was significant improvement in the American Orthopaedic Foot & Ankl

Early weight bearing after posterior malleolar fractures: an experimental and prospective clinical study.

The distribution of axial load to the lower end of the tibia at different positions of the ankle joint for the anterior, middle, and posterior part of the joint was studied in both photoelastic models and fractured ankle joints in cadaveric specimens. Synthetic models were used to simulate both normal ankle joints and ankles with fractures of the posterior lip of the tibia. Tests were performed with the ankle at dorsiflexed, neutral-flexed, and plantarflexed positions of the ankle joint. The clinical portion of the study evaluated 15 patients with fracture of the posterior malleolus that compr

The effect of recurrent ankle inversion sprain and taping on proprioception at the ankle.

PURPOSE: A common explanation for the tendency for ankle inversion sprains to recur is that proprioception is impaired by the initial sprain. It is further hypothesized that the application of tape prevents further sprains by enhancing proprioceptive acuity. The purpose of this study was to determine whether proprioception was impaired in people with recurrent ankle inversion sprain, and whether taping the ankle provided enhanced proprioceptive ability. METHODS: We compared the ability to perceive passive plantarflexion and dorsiflexion movements imposed at the relaxed ankle in 25 subjects wi

Balance training for persons with functionally unstable ankles.

STUDY DESIGN: A nonrandomized 2-group pretest-posttest design. OBJECTIVES: To determine the effects of a 4-week balance training program during stance on a single leg. BACKGROUND: Individuals who have experienced multiple episodes of inversion ankle sprains often participate in balance training programs. Balance training is performed to treat existing proprioceptive deficits and to restore ankle joint stability, presumably by retraining altered afferent neuromuscular pathways. The effectiveness of such programs on individuals with functionally unstable ankles has yet to be established. METH

Distal tibiofibular synostosis after ankle fracture. A 14-year follow-up study.

Over an eight-year period up to 1983, a total of 322 consecutive patients had operations for ankle fractures; 176 were Weber type B and 128 type C. We were able to review 230 of these patients after a mean follow-up of six years (1 to 11) including 128 with Weber B and 102 with Weber C fractures. We used an ankle score which combined symptoms and clinical and radiological findings, with a maximum score of 100 points. The mean score for all 230 was 92 (68 to 100). Fifteen of these patients had developed a distal synostosis between the tibia and fibula, three after a Weber B and 12 after a Weber
