BACKGROUND: The repeated airway obstructions in the common disorder Obstructive Sleep Apnea (OSA) cause health risks. Continuous Positive Airway Pressure (CPAP), the standard treatment, faces adherence challenges. Mandibular Advancement Devices (MADs) have been used successfully for mild to moderate OSA, as a good alternative for these patients.
OBJECTIVE: to evaluate the effectiveness of MADs in reducing the Apnea-Hypopnea Index (AHI) and improving symptoms in patients with very severe OSA unable to tolerate CPAP.
METHODS: This retrospective study included 22 patients with very severe OSA (

Sleep apnea is a prevalent disorder affecting 10 % of middle-aged individuals, yet it remains underdiagnosed due to the limitations of polysomnography (PSG), the current diagnostic gold standard. Single-lead electrocardiography (ECG) has been proposed as a potential alternative diagnostic tool, but interpretation challenges remain. Recent advances in machine learning and deep learning technologies offer promising approaches for enhancing the detection of sleep apnea through automated analysis of ECG signals. This meta-analysis aims to evaluate the diagnostic accuracy of machine learning (ML) a

BACKGROUND: Obstructive sleep apnea represents an arrhythmia recurrence risk following catheter ablation of atrial fibrillation (AF). Continuous positive airway pressure (CPAP) therapy may reduce recurrence, but individual adherence to CPAP varies, potentially leading to inconsistent results across studies.
METHODS AND RESULTS: We analyzed the impact of CPAP adherence on AF recurrence after catheter ablation in patients with severe obstructive sleep apnea (apnea-hypopnea index ≥30). Of 1864 consecutive patients undergoing an index catheter ablation of AF, those with severe obstructive sleep a

OBJECTIVES: MassHealth Limited (MHL), a subset of Massachusetts' Medicaid program, covers sleep studies but not obstructive sleep apnea (OSA) treatment, including positive airway pressure (PAP) therapy. This study characterizes the demographics and OSA severity of patients receiving MHL and compares them to those with full MassHealth (MH) benefits to assess inequities in treatment access.
PARTICIPANTS AND METHODS: We conducted a retrospective cohort study of 134 MHL and 402 MH patients who underwent a sleep study for OSA evaluation at the Brigham and Women's Faulkner Hospital (2019-2024). We

Cardiovascular diseases (CVDs) are prevalent among obstructive sleep apnea (OSA) patients, presenting significant challenges in predictive modeling due to the complex interplay of these comorbidities. Current methodologies predominantly lack the dynamic and longitudinal perspective necessary to accurately predict CVD progression in the presence of OSA. This study addresses these limitations by proposing a novel multi-level phenotypic model that analyzes the progression and interaction of these comorbidities over time. Our study utilizes a longitudinal cohort from the Wisconsin sleep cohort, co

PURPOSE: Periodontitis (PD) and obstructive sleep apnea (OSA) are widespread conditions with profound health consequences. Increasing evidence suggests shared pathophysiological mechanisms between PD and OSA, prompting this study to explore their genetic connections using advanced transcriptomic approaches.
METHODS: Gene expression data was obtained from GEO, integrating bulk and single-cell RNA sequencing (scRNA-seq). Differentially expressed genes (DEGs) were identified, and common DEGs were analyzed via protein-protein interaction (PPI) networks and functional enrichment. Machine learning

Background and Objectives: Obstructive sleep apnea (OSA) is a prevalent disorder associated with increased cardiovascular (CV) risk. Materials and Methods: We conducted a PRISMA-compliant systematic review and meta-analysis of prospective cohort studies assessing OSA and incident CV outcomes. Results: From 2463 records, 18 studies (>25,000 participants; median follow-up 9 years) were included. OSA was associated with increased CV risk (pooled HR 1.82, 95% CI 1.45-2.28). Dose-response analysis showed a progressive risk increase: mild OSA (HR 1.21, 95% CI 0.98-1.50), moderate (HR 1.56, 95% CI 1.

BACKGROUND AND OBJECTIVES: Timely treatment of pediatric obstructive sleep apnea (OSA) can prevent or reverse neurocognitive and cardiovascular morbidities. However, whether distinct phenotypes exist and account for divergent treatment effectiveness remains unknown. In this study, our goal is threefold: i) to define new data-driven pediatric OSA phenotypes, ii) to evaluate possible treatment effectiveness differences among them, and iii) to assess phenotypic information in predicting OSA resolution.
METHODS: We involved 22 sociodemographic, anthropometric, and clinical data from 464 children

OBJECTIVE: Depression is a frequent comorbidity in obstructive sleep apnea (OSA) patients. There is a scarcity of data on the impact of depression on the outcomes of OSA.
MATERIAL AND METHODS: Using the National Inpatient Sample (2018), we identified hospitalizations in the US with OSA. Following propensity-score matching, the two cohorts of OSA with depression (OSA+D+) vs. without depression (OSA+D-) were compared for demographic and comorbidities profiles differences. Multivariable regression analyses were performed to assess the odds of events with depression versus those without.

INTRODUCTION: This review explores the interplay between comorbid insomnia and sleep apnea (COMISA) and erectile dysfunction (ED), 2 conditions that significantly impact men's health. COMISA, a recently recognized condition characterized by the coexistence of insomnia and obstructive sleep apnea, has been shown to disrupt sleep architecture and cause intermittent hypoxia. These disturbances are increasingly linked to the exacerbation of ED, a prevalent issue among men. Understanding the connection between COMISA and ED is crucial for developing integrated treatment approaches that address both

STUDY OBJECTIVES: Physicians-in-training (residents, fellows) and advanced practice providers (APPs) receive limited education on sleep disorders, including obstructive sleep apnea (OSA). They often assess patients first. We aimed to understand their views on OSA and screening for OSA in the perioperative period.
METHODS: Electronic-mail surveys were sent to physicians-in-training and APPs, in 5 categories of practice (anesthesiology, internal medicine, family medicine, obstetrics/gynecology/gynecologic oncology, and surgery) at 9 major institutions. Cochran-Mantel-Haenszel tests evaluated as

BACKGROUND: Observational studies have suggested an association between attention-deficit/hyperactivity disorder (ADHD) and obstructive sleep apnea (OSA), but these findings are often inconsistent due to potential biases from medication use, and varying diagnostic criteria. Genetic analyses can help mitigate these confounding factors, providing additional evidence.
METHODS: This study evaluated the genetic correlations between ADHD and OSA using Genome-wide association study (GWAS) summary data, applying linkage disequilibrium score regression (LDSC) and SUPER GeNetic cOVariance Analyzer (SUP

BACKGROUND: Identifying modifiable patient-related risk factors may guide interventions aimed at reducing prolonged postoperative opioid use. Obstructive sleep apnea (OSA) has been hypothesized as one such condition. However, the association between an OSA diagnosis and prolonged postoperative opioid use remains unclear.
METHODS: Using administrative healthcare claims data from the MarketScan Commercial and Medicare Claims Databases, we examined the association between preoperative OSA, defined as two or more prior claims with the OSA International Classification of Diseases 10th revision (IC

STUDY OBJECTIVES: Sleep-disordered breathing is incompletely characterized by the apnea-hypopnea index. Although typically viewed as milder disease, snoring and flow limitation can yield major deficits in sleep health for both patients and their bed partners. Here we tested whether a combined noradrenergic and antimuscarinic intervention to activate pharyngeal muscles yields improved snoring loudness and flow limitation severity, plus self-reported outcomes, by snorers and their bed partners.
METHODS: In a randomized placebo-controlled double-blind crossover study, adults with loud habitual s

PURPOSE: To explore whether Periodic Limb Movements during Sleep (PLMS) are an intrinsic feature of sleep physiology or influenced by the progression of sleep apnea using longitudinal data. METHODS: This study analyzed data from 39 participants in the Wisconsin Sleep Cohort who transitioned from no or mild (Apnea-Hypopnea Index - AHI < 10) to moderate-to-severe sleep apnea (AHI > 15). Polysomnographic recordings measured PLMS and related leg movement indices, including respiratory-related limb movements (RRLM), at baseline and follow-up. RESULTS: AHI increased significantly between baseline an

Rett syndrome (RTT) is a neurodevelopmental disorder that is caused by loss-of-function mutations in the methyl-CpG binding protein 2 (MeCP2) gene. RTT patients experience a myriad of debilitating symptoms, which include respiratory phenotypes that are often associated with lethality. Our previous work established that expression of the M1 muscarinic acetylcholine receptor (mAchR) is decreased in RTT autopsy samples, and that potentiation of the M1 receptor improves apneas in a mouse model of RTT; however, the population of neurons driving this rescue is unclear. Loss of Mecp2 correlates with
