T Cell-Mediated Targeting of Interneurons in Mice Shapes Hippocampal Remodeling and Epilepsy.

OBJECTIVE: Autoimmune encephalitis (AE) is associated with autoantibodies targeting distinct neuronal populations. In AE, antibodies against glutamate decarboxylase 65 (GAD65), expressed in GABAergic interneurons, are frequently detected. In GAD65-AE, hippocampal biopsies often show infiltrates of CD8+ cytotoxic T cells (CTLs), suggesting a prominent T cell-associated pathology. This has led to the hypothesis that CTLs contribute to neuronal injury in GABAergic circuits. We investigated whether selective CTL-mediated dysfunction of hippocampal interneurons may contribute to temporal lobe epile

Astrocyte redox imbalance underlies prelimbic neuronal hypoactivity and maladaptive affective behaviors in epilepsy.

A fundamental but unanswered question in neuropsychiatry is whether the psychiatric symptoms of epilepsy are caused by the same or a separate pathophysiology as seizures. To address this question, we investigated a monogenic form of epilepsy (pyridoxine-dependent epilepsy) caused by aldehyde dehydrogenase 7 family member A1 (ALDH7A1) mutations. ALDH7A1 global knockout mice exhibited both seizure-associated and maladaptive affective behavioral phenotypes. However, seizure phenotypes were caused by ALDH7A1 deletion in hepatocytes whereas maladaptive affective behaviors were caused by ALDH7A1 del

Association Between Multiple Inflammation-Derived Indices and the Risk of Post-Stroke Epilepsy.

BACKGROUND: Post-stroke epilepsy (PSE) is a common and serious complication of stroke, yet simple and efficient risk assessment tools remain lacking in clinical practice. This study aimed to investigate the association between multiple core inflammation-derived indices and PSE, and to explore their potential reference value in risk stratification. METHODS: In this retrospective case-control study, we extracted data from hospitalized stroke patients at Hefei Second People's Hospital between January 2018 and December 2025 from the electronic medical record system. A 1:1 propensity score matchin

Dissociating decision-making deficits in temporal and frontal lobe epilepsy using Bayesian methods.

PURPOSE: People with epilepsy exhibit various cognitive impairments, including deficits in executive functioning that could impact their decision-making. This study examined differences in objective risk-based decision-making among individuals with focal epilepsy, specifically comparing right temporal lobe epilepsy (RTLE), left temporal lobe epilepsy (LTLE), and frontal lobe epilepsy (FLE) against healthy controls. METHODS: Decision-making was assessed in a case-control study design with 43 adults with pharmacoresistant focal epilepsy (15 RTLE, 18 LTLE, 10 FLE) and 15 controls using the Cambr

Case Report: Progressive myoclonus epilepsy as an early manifestation of neuronopathic Gaucher disease.

Gaucher disease (GD) is a lysosomal storage disorder caused by biallelic GBA1 variants. Epilepsy is uncommon in GD and rarely manifests as progressive myoclonus epilepsy (PME), making early recognition difficult. We describe a 20-year-old man with childhood-onset myoclonus that progressed to drug-resistant generalized seizures and cognitive decline. Video-electroencephalography (VEEG) showed generalized polyspike-wave discharges associated with myoclonic jerks, whereas brain magnetic resonance imaging was initially normal. Cerebrospinal fluid studies, metabolic screening, and autoimmune enceph

Defective EV-mediated transport of SHH alters neural fate specification in EPM1 epilepsy.

The extracellular milieu, including extracellular vesicles (EVs), plays a pivotal role in brain development. In this study, we sought to elucidate the pathogenesis of progressive myoclonus epilepsy type 1 (EPM1), a disease caused by mutations in the CSTB gene, using cerebral organoids (COs) derived from patient cells. The results demonstrate that EPM1 COs display increased electrophysiological activity and disrupted excitatory/inhibitory (E/I) balance. Single-cell RNA sequencing analysis of ventral EPM1-COs revealed an abnormal specification of progenitor fate, with a shift toward dorsal neuro

Pediatric occipital lobe epilepsy: A modern review of etiological classification, management, and outcomes.

Pediatric occipital lobe epilepsy (OLE) comprises syndromes with seizures originating from the posterior cerebral cortex. Previously "benign", recent International League Against Epilepsy (ILAE) updates reclassified these as self-limited or structural focal epilepsies, reflecting their complex causes and morbidity potential. The 2017 ILAE guidelines shifted from syndromic recognition to an etiology-driven approach. High-resolution magnetic resonance imaging is crucial to differentiate self-limited genetic syndromes from structural OLE (e.g., focal cortical dysplasia), as the latter often requi

Case Report: Epilepsy-related pure word deafness in the elderly: an underreported presentation?

We describe a rare case of epilepsy-related PWD in the elderly. A 61-year-old right-handed man presented with status epilepticus. Although electroencephalogram revealed no epileptiform discharges, brain magnetic resonance imaging (MRI) showed hyperintensity in the left temporoparietal lobe and thalamic pulvinar on diffusion-weighted imaging (DWI). Cerebrospinal fluid revealed no pleocytosis and negative PCR results for herpes simplex virus and varicella-zoster virus. Following the recovery of consciousness, he exhibited a profound dissociation between auditory and speech/language modalities: h

Molecular Mechanism of Compound Glycyrrhizin in Kainic Acid-Induced Epilepsy in Rat Model.

This study explores the effects of compound glycyrrhizin (GL) on seizures in the rat model and its potential molecular mechanisms. Eighty male, healthy, and clean-graded Sprague Dawley (SD) rats, aged 1 month, were used in this study. Of these, 60 rats were selected to construct the epilepsy model and were randomly divided into the GL group (status epilepticus (SE) + GL, n = 60) and the model group (Model, n = 10), and received tail vein injections of GL or saline, respectively. Another ten normal rats served as the control group (Control). According to different doses of GL, they were divided

Restoration of blood brain barrier integrity post neurosurgical resection in drug resistant epilepsy.

Surgery for temporal lobe epilepsy (TLE) is a well-recognised therapy for drug resistant seizures which occur in more than 50 % of patients with TLE. Blood-brain barrier (BBB) dysfunction is commonly observed in resected brain tissue from patients with treatment resistant epilepsy however, no studies have documented the recovery of BBB function following surgery. We firstly prospectively performed dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) on seven patients scheduled for temporal lobe resections before and after resection. DCE-MRI revealed BBB dysfunction in frontal and tem

On-scalp magnetoencephalography based on optically pumped magnetometers to investigate temporal lobe epilepsy.

Cryogenic magnetoencephalography (MEG) has a lower yield in temporal lobe epilepsy (TLE) than in extratemporal lobe epilepsy. The advent of optically pumped magnetometers (OPMs) might change this situation thanks to on-scalp MEG. This study aims to investigate the detection/localization accuracy of interictal epileptiform discharges (IEDs) using on-scalp MEG in TLE and the added value of face-OPMs. Eleven patients underwent a 1-h MEG recording with OPMs placed both on the scalp (flexible cap, scalp-OPMs) and on the face (glasseslike structure, face-OPMs). Nine patients underwent cryogenic MEG.

Combined impact of gray and superficial white matter abnormalities: Implications for epilepsy surgery.

OBJECTIVE: Drug-resistant focal epilepsy is associated with abnormalities in the brain in both gray matter (GM) and superficial white matter (SWM). However, it is unknown if both types of abnormalities are important in supporting seizures. Here, we test if surgical removal of GM and/or SWM abnormalities relates to post-surgical seizure outcome in people with temporal lobe epilepsy (TLE). METHODS: We analyzed structural imaging data from 143 patients with TLE (pre-op diffusion magnetic resonance imaging and pre-op T1-weighted MRI) and 97 healthy controls. We calculated GM volume abnormalities

Self-compassion, mental health, and emotion regulation in adults with temporal lobe epilepsy.

BACKGROUND: People with temporal lobe epilepsy (TLE) are at increased risk of mental health difficulties, most commonly anxiety and depression. Self-compassion, comprised of self-kindness, common humanity, and mindfulness, has been associated with better mental health in wider epilepsy populations. Previous research also suggests that self-compassion may be protective by fostering better emotion regulation. However, this has not been studied in TLE. AIMS: To (1) compare adults with TLE, other types of epilepsy (non-TLE), and without epilepsy (non-epilepsy) on measures of self-compassion, anxi

Sleep-related epilepsy through the lens of stereo-EEG: Clinical and research update.

The complex interactions between sleep and epilepsy have drawn increasing attention, and stereo-electroencephalography (SEEG) has become a pivotal tool for investigating their underlying pathophysiological mechanisms. This review highlights key contributions from SEEG studies over the past two decades, with a focus on Sleep-Related Hypermotor Epilepsy (SHE). Considered a disorder of frontal lobe origin, SHE is now recognized as a network-based epilepsy with a broader involvement of cortical regions. Sleep instability in Non-Rapid Eye Movement (NREM) sleep, indexed by the cyclic alternating pat

Impact of anterior callosal disconnection on picture naming in frontal lobe epilepsy surgery.

Epilepsy surgery in focal, drug-resistant frontal lobe epilepsy can be curative and resection is aimed at seizure freedom. The cognitive impact of surgery, however, is less clear-cut. On one hand, resection of the epileptogenic zone can disconnect essential brain networks and therefore cause dysfunction. On the other hand, surgery may prompt recovery of normal brain function by restoring normal electrical activity as propagating epileptic discharges affect cognition outside the epileptogenic zone. To understand the impact of surgery on cognitive outcome, we investigated picture naming in 51 pa

Evaluating the efficacy of non-invasive brain stimulation techniques in managing pediatric epilepsy.

BACKGROUND: Pediatric epilepsy significantly affects cognitive and developmental outcomes, with drug-resistant epilepsy (DRE) posing a major challenge. While pharmacological and surgical interventions remain standard treatments, they often fail in refractory cases. Non-Invasive Brain Stimulation (NIBS), including Transcranial Magnetic Stimulation (TMS) and Transcranial Direct Current Stimulation (tDCS), has emerged as a promising therapeutic alternative. NEW METHOD: This study systematically reviews and compares the efficacy, safety, and feasibility of TMS and tDCS in pediatric epilepsy. The

Hypoxia-Regulated CD44 and xCT Expression Contributes to Late Postoperative Epilepsy in Glioblastoma.

BACKGROUND/OBJECTIVES: Late epilepsy occurring in the late stage after glioblastoma (GBM) resection is suggested to be caused by increased extracellular glutamate (Glu). To elucidate the mechanism underlying postoperative late epilepsy, the present study aimed to investigate the expressions and relations of molecules related to Glu metabolism in tumor tissues from GBM patients and cultured glioma stem-like cells (GSCs). METHODS: Expressions of CD44, xCT and excitatory amino acid transporter (EAAT) 2 and extracellular Glu concentration in GBM patients with and without epilepsy were examined an
