From Head to Toe: Investigating Postconcussion Risks for Lower Extremity Injuries in Young Athletes.

BACKGROUND: Between 1.7 and 3 million sports-related concussions occur every year. While most concussion symptoms resolve within 3 weeks, the absence of biomarker-based tests makes evaluating return to sport difficult. Existing guidelines primarily focus on cognitive and executive functions and often overlook proprioceptive abilities. This omission may increase athletes' risk of subsequent lower extremity injuries during competition. PURPOSE/HYPOTHESIS: The purpose of this study was to assess the rate of lower extremity injuries in athletes in the year following their first concussion. It was

Primary Repair of Medial Collateral Ligament Tears With Suture Augmentation.

BACKGROUND: The medial collateral ligament (MCL) is considered to have superior healing and has classically been treated nonoperatively in low-grade MCL injuries. The MCL injury in the setting of concomitant anterior cruciate ligament (ACL) injury is usually treated with a delay of surgery to allow for nonoperative MCL treatment. Recent studies have shown that even with grade II MCL injury there may be residual laxity which may affect place greater strain on an ACL graft in the multi-ligamentous setting and can be corrected for with early primary repair of the MCL which also allows for early d

Denervation impairs healing of the rabbit medial collateral ligament.

Little is known about the contribution of innervation to ligament healing after traumatic disruption, although there is good evidence of an important role for the peripheral nervous system in the healing of fractures and skin injuries. Tissues such as ligament, with a low resting blood supply, are dependent on substantial increases in blood flow and vascular volume during the initial stages of repair. We hypothesized that this initial healing response would be strongly promoted by neurogenic inflammation. Since the saphenous nerve (a major sensory branch of the femoral nerve) supplies the medi

Arthroscopic Continuous Anterior Cruciate Ligament-Medial Collateral Ligament Reconstruction Using a Single Rectus Femoris Autograft With Posterior Oblique Ligament Reconstruction: A Technical Note.

Multiligament knee injuries are challenging to manage and often require harvesting multiple grafts. Rectus femoris tendon autograft provides predictable length and diameter, making it a promising option for anterior cruciate ligament (ACL) reconstruction and combined procedures. In this technical note, we describe an arthroscopic technique for combined reconstruction of the ACL and medial-sided structure-the medial collateral ligament (MCL) and posterior oblique ligament-in the setting of acute ACL, MCL and posterior oblique ligament rupture. A single rectus femoris autograft is used to recons

Posterior Cruciate Ligament Reconstruction: A Technique Utilizing Tibial Double Knotless Anchors Fixation and Femoral Suspensory Fixation.

The posterior cruciate ligament (PCL) is a critical stabilizing structure within the knee joint. Although its injury is relatively uncommon, severe cases can lead to significant dysfunction and long-term sequelae. PCL reconstruction is an essential treatment for PCL injuries, and among traditional techniques, trans-tibial interference screw fixation is a common method. However, due to the higher graft length requirement in PCL reconstruction, interference screw fixation is prone to fixation laxity resulting from insufficient graft length. To address the issue, we describe an improved PCL recon

Arthroscopy-Assisted Type 2 Posterior Horn Lateral Meniscus Transtibial Root Repair And Concurrent Inside-Out Lateral Meniscus Horizontal Cleavage Tear Repair With an Anterolateral Ligament Reconstruction Utilizing Iliotibial Band Autograft.

The anterior cruciate ligament (ACL) and anterolateral ligament (ALL) play key roles in maintaining rotational stability of the knee joint. Injuries that cause ACL tears have been shown to also frequently compromise the integrity of the ALL. Patients with laxity of the ALL are predisposed to increased rotation of the tibia and subsequent meniscal injury after an ACL reconstruction (ACLR). If such meniscal injury does occur, performing an ALL reconstruction (ALLR) at the time of meniscal repair will potentially limit the extremes of internal tibial rotation, protecting the meniscal repair to al

Bilateral Spontaneous Quadriceps Tendon Ruptures after Staged Bilateral Total Knee Arthroplasties: A Case Report.

Quadriceps tendon ruptures are rare complications of total knee arthroplasty (TKA) and may be associated with preexisting tendon degeneration, vascular injuries during surgery, damage to the quadriceps tendon with over resection of the patella, or incomplete healing after a particular surgical approach. Moreover, postoperative causes include tissue necrosis due to component malalignment, trauma, and infection. Patient factors such as chronic systemic diseases, medications, increased body weight, and sedentary lifestyle could also be responsible. The number of reported cases on bilateral sponta

Semiextending Nailing for Combined Shaft and Ankle Injuries of the Leg.

Over recent years, orthopaedic traumatologists have been gaining experience with tibial nailing in the extended position. Originally developed to address the complexities of alignment in proximal tibial fractures, this technique has also proven valuable in the treatment of tibial shaft fractures combined with ankle injuries. Depicted in this video presentation is the surgical techniques used to simultaneously address a spiral fracture of the distal tibia associated with a large coronal plane fracture of the tibial articular surface. Discussed are leg positioning, technical aspects of the media

Surgical management of anterior cruciate ligament injuries in patients with open physes.

Because of the increasing number of skeletally immature athletes who compete in highly demanding sports, more children than previously are sustaining anterior cruciate ligament injuries. Treatment and patient compliance with treatment recommendations are problematic. Pediatric issues include those specific to evaluation, projected growth, and surgery. Strict activity modification can protect the knee from further injury and delay surgery, sometimes until maturity. Surgical options include physeal-sparing, partial transphyseal, and complete transphyseal procedures. Surgical procedures are deman
