We report a case of intrarectal tear and hemorrhage after a fall while water skiing. The injury resulted from rectal douche, or jet enema, which is dangerous because of the high pressure of the water jets forced into the rectum. The patient, a middle-aged woman, had colonoscopy, which revealed a deep rectal laceration. During subsequent colostomy, a full-thickness laceration and an extensive pelvic and retroperitoneal hematoma with some free blood were found. Two patients are reported with pleomorphic lipoma, a benign subcutaneous fatty tumor showing atypia in which histologic diagnosis may be difficult. Atrial arrhythmia is not uncommon in congenital atrial septal defect. Its incidence increases with age and the hemodynamic compromise of right ventricle. In the literature, atrial fibrillation is the most common presentation in middle-aged adults with congenital atrial septal defect, followed by atrial flutter and paroxysmal supraventricular tachycardia. A middle-aged woman who had been living in close surroundings of a fluorine-emitting factory for many years, was examined because she claimed to be suffering from skeletal fluorosis. Radiological, histological and laboratory findings did not give evidence of fluorosis. Fluoride concentrations were normal in samples of blood, bone, pooled urine, nails and hair. Even this proof to the contrary did not shake her conviction of many years that she was suffering from fluorosis. Thus, psychological aspects have to be considered in the assessment of health implications of pollutants.

Visceral larva migraine syndrome in most often caused by the migration of Toxocara canis larvae in the tissues after ingestion of the embryonated eggs. The disease, characterised by granulomas, is most commonly found among children in whom it causes fever, hepatomegaly, respiratory disturbances and eosinophilia. Diagnosis is by biopsy and serum tests and prophylaxis is essential since no specific therapy exists. The case of toxocariasis described was encountered in a middle aged women and is unusual as it had lasted for several years whereas spontaneous remission is the rule.

A middle-aged woman developed a postgastric bypass megaloblastic anemia which responded to treatment. She eventually had the bypass reversed 6 1/2 yr after it had been performed. Seven cases of Claude Bernard-Horner's syndrome due to malignant pleural mesothelioma are described. All the patients were middle aged farmers and lived in the rural part of Central Anatolia. Two of them lived in Karain and had been exposed to inhale erionite type zeolite fibres. Hence the other five subjects lived in asbestos deposit areas. Pleural effusion, pleural thickening and nodular pleural lesions were the radiological findings. Rib erosion was found in one case. All the patients had tissue diagnosis by pleural punch biopsy, thoracoscopy or thoracotomy.

Idiopathic blepharospasm is reexamined and the differential diagnosis discussed on the basis of the signs and symptoms of 27 unpublished cases. As in tardive dyskinesia, middle-aged and elderly women are mostly affected. Contrary to general opinion, spontaneous improvement occurred in about one-fifth of the patients. The habituation of the orbicularis oculi reflexes (investigated electromyographically) was diminished in 52% of the patients. The authors personally studied 138 patients with liver granulomas: seventy-five (54%) had sarcoidosis, twenty-six (19%) had primary biliary cirrhosis, twenty-three (17%) had miscellaneous recognizable disorders and fourteen (10%) were undiagnosed. The modes of presentation, the clinical, biochemical and radiological features are compared, and the course of each disease is followed. Benign symmetric lipomatosis (BSL) leads to characteristic multiple lipomas of the neck and submandibular region. BSL is easily diagnosed by its typical feature, "Madelung's neck". BACKGROUND: Malignant hyperthermia (MH) is an inherited muscle disorder induced by volatile anesthetics and depolarizing muscle relaxants. While the incidence of MH is high in young, there are few reports on the clinical features of pediatric MH. In this study, we selected pediatric cases from an MH database and analyzed the clinical findings by age group. We hypothesized that there would be age-related differences in the clinical characteristics.
