We present the first case of torsion of an appendix testis in a cryptorchid testicle. The difficulties in history, physical examination and imaging posed by such a presentation are presented as well as management considerations.

A 57-year-old man with insidious onset of progressive bilateral upper extremity weakness was referred to physical therapy by his primary care physician. We present the clinical histories and physical findings of two sisters with Rett syndrome. The physical examination, combined with a review of their medical charts, revealed that both patients met the necessary criteria for the diagnosis of Rett syndrome as defined by the Rett syndrome diagnostic criteria work group. The older sister, currently 25 years of age, is typically affected, whereas the younger sister, currently 22 years of age, is affected with a seizure disorder showing an unusually early onset.

Injuries involving tendons, ligaments, and bones are commonplace with many physical activities. Although the majority of these injuries are minor and cause only temporary discomfort, their potential for producing serious and occasionally permanent disabilities must not be ignored. The initial physical and roentgenographic examination of the injured part must be thorough. Only then can the severity of the injury be recognized and appropriate treatment undertaken. Promptness will limit the period of disability and minimize the chances of development of a chronic condition.

Testicular torsion is a urologic emergency that requires surgical intervention. Its diagnosis is rarely made in elderly men especially the subset of men on urethral catheter. A 75-year-old male presented with groin pain after an operation to treat lumbar spondylolisthesis (L5).
