The Paramedian Forehead Flap for Nasal Reconstruction: From Antiquity to Present.

Nasal reconstruction is considered the historic foundation of facial plastic surgery, and the forehead flap remains the workhorse of repair. To recreate both the aesthetic contour and function of the nose, all anatomic layers must be addressed-covering, lining, and structural support. This article reviews the noteworthy history underlying the development of the paramedian forehead flap as the primary tool in reconstruction of large nasal defects while highlighting its implications on modern nasal repair. Current developments in the use of 2-staged paramedian forehead flap reconstruction are ex

Chin Augmentation Using Silastic Implants.

An aesthetically pleasing face is centered on strength, symmetry, and balance. A defined, well-projected chin and jawline are crucial to this harmony and recontouring the lower third of the face plays a significant role in plastic surgery. Chin augmentation can be performed with injectables, implantation, or osseous genioplasty. The selection of the best procedure is based on the type and extent of deformity. This article reviews chin augmentation with emphasis on alloplast implantation. The relevant anatomy, patient evaluation, implant indications and limitations, alloplast selection, and sur

Deep Sternal Wound Infection: Diagnosis, Treatment and Prevention.

Deep sternal wound infection (DSWI) is a rare but potentially devastating complication of median sternotomy performed in cardiac surgery. The incidence of DSWI is reported to be between 0.2% and 3%. Identifying high-risk patients and strategies to optimize risk factors plays an important role in reducing the incidence of DSWI. Management of DSWI can be complex and may require a multidisciplinary team approach involving infectious disease specialists, microbiologists, as well as cardiothoracic and plastic surgeons. Early detection, appropriate antibiotic treatment, aggressive surgical debrideme

A Child with Lung Hypoplasia, Congenital Heart Disease, Hemifacial Microsomia, and Inguinal Hernia: Ipsilateral Congenital Malformations.

A 3-year-old Chinese boy was diagnosed with ipsilateral congenital malformations: right lung hypoplasia, dextroversion of heart, atrial septal defect, hepatic vein drainage directly into the right atrium, facial asymmetry, right microtia and congenital deafness, and indirect inguinal hernia. He underwent indirect inguinal hernia repair at the age of 2. Although without any facial plastic surgery performed, he underwent a repair of atrial septal defect and recovered uneventfully. At 6-month follow-up, the patient was free from any symptom of dyspnea; his heart function returned to the first gra

[One-stage retrosternal colonic esophagoplasty in treatment of patients with scar burn stricture of esophagus].

Esophagoplasty was done in 23 patients an average of 10.4±2.3 months after chemical burn of esophagus. It was necessary to perform 29 corrective operations in 18 cases for training to one-stage plastic surgery such as gastrostomy, resection of stomach, retrocolic gastrojejunostomy on short loop. One-stage esophagoplasty was done in 21 of 23 cases. Functionally the most unusable anastomosis was cervical esophageal-intestinal anastomosis side-to-side. The number of complications after esophagocoloplasty by using of right flank was less than after using of left flank (39.92% vs. 50% respectively)

[Reconstructive mastoidoplasty following sanitation surgery on the ear].

The objective of the present study was to enhance the effectiveness of surgical rehabilitation of the patients presenting with "operated ear disease". Sixty four patients underwent sanitation surgery on the ear by means of plastic correction with the use of a muscular-periosteal flap on a feeding pedicle with the simultaneous restoration of the posterior wall of the external auditory canal by the implantation of septal autocartilage. The surgical technique is described. The analysis of long-term results of the treatment of such patients has demonstrated its high efficacy.

Skin graft.

Skin graft is one of the most indispensable techniques in plastic surgery and dermatology. Skin grafts are used in a variety of clinical situations, such as traumatic wounds, defects after oncologic resection, burn reconstruction, scar contracture release, congenital skin deficiencies, hair restoration, vitiligo, and nipple-areola reconstruction. Skin grafts are generally avoided in the management of more complex wounds. Conditions with deep spaces and exposed bones normally require the use of skin flaps or muscle flaps. In the present review, we describe how to perform skin grafting successfu

[General principles of burn reconstruction].

The aims of burn reconstruction should be to preserve, restore, and maintain function and appearance so the patient can recover a social life. Reconstruction surgery is delayed until the scar tissue has fully matured; it occurs generally by 9 to 12 months or more (merits of vitro-pressure test). Exception in presence of severe contracture (growth disturbance, unstable scars, corneal exposure with ectropion, severe microstomia) Occupational and physical therapy, scar management, compressive techniques, psychotherapy are all integral components of burn rehabilitation, but the place of plastic su

[Treatment of chronic recurrent synovial fistula following radiosynoviorthesis: two case reports].

Chronic recurrent synovial fistulae of the knee are uncommon. They can occur after traumatic injuries, as complication after surgical treatment or in patients with rheumatoid arthritis as spontaneous complication after trauma. No consensus exists about the adequate treatment and literature on this subject is scarce. Two cases are presented in which synovial fistula had occurred after radiosynoviorthesis. Previous conservative and conventional surgery failed to solve the problem as the fistulae remained chronic and recurrent. However both, radical debridement and plastic reconstruction by eithe

[Results of left atrium surgery performed during corrective surgery of mitral valve disease complicated by atriomegaly].

Short- and long-term results of left atrium's plasty in 144 patients performed during correction of mitral valvular disease complicated with atriomegalia were analyzed. Age of patients ranged 7 to 66 years (33+/-9.3 ys). Size of left atrium ranged 6.0 to 9.5 cm (7.24+/-0.95 cm on average). Plastic surgery of left atrium permits to remedy symptoms of compression of left ventricle and left primary bronchus. Timely surgical treatment permits to improve central hemodynamics, to recover sinus rhythm in 25% patients, to improve quality of life in long-term period of follow-up.

Gynecomastia: its features, and when and how to treat it.

Gynecomastia is common, being present in 30% to 50% of healthy men. A general medical history and careful physical examination with particular attention to features suggestive of breast cancer often suffice for evaluation in patients without symptoms or those with incidentally discovered breast enlargement. Men with recent-onset gynecomastia or mastodynia need a more detailed evaluation, including selected laboratory tests to search for an underlying cause. Treatment depends on the cause and may include observation, withdrawal of an offending drug, therapy of an underlying disease, giving andr

Pyoderma gangrenosum after reduction mammaplasty in an otherwise healthy patient.

Pyoderma gangrenosum (PG) is a rare postoperative complication of plastic surgery of the breast. Initial signs and symptoms resemble those of infection, and antimicrobial therapy is usually initiated and fails before considering PG as a diagnosis. Therapy consists of immune modulators, and use of corticosteroids is frequent, as is local wound care. Sufficiently small wounds are allowed to heal secondarily, but larger wounds require coverage with either skin grafts or flaps. Long-term (1 year or more) postoperative surveillance is necessary because late failure of the graft or flap can occur.

Papillary fibroelastoma of a mitral valve chorda.

A 43-year-old woman presented with an ischemic stroke in the right middle cerebral artery territory. Cardiac echography disclosed a tumor of a primary chordae of the anterior leaflet of the mitral valve. After neurologic recovery, the patient was referred to surgery for excision of the tumor and plastic reconstruction using a chordal transfer technique. Histological examination of the tumor showed a typical papillary fibroelastoma. Papillary fibroelastoma is the third most frequent cardiac benign tumor. The high embolic potential of this tumor is in favor of an aggressive surgical attitude.

[Microsurgical reimplantation of scalp: case report].

With the introduction of modern techniques in plastic surgery, severe injuries concerning the extremities and scalp are now amenable to surgical treatment. Successful reimplantation of the scalp either total or partial have been rarely reported in the literature. We treated a patient with avulsion affecting 2/3 of the scalp, the right ear and a cutaneous flap on right side of the face. We report here the initial care and operative procedure. The avulsed scalp was revascularized through microanastomoses of the temporal artery and vein. The reimplantation was well succeed and cosmetic results we

Reconstruction of axillary burn contractures with the latissimus dorsi fasciocutaneous flap.

Standard management of axillary burn contractures has been scar release and the use of skin grafts, despite the common problem of incomplete graft take, prolonged splinting, extended physical therapy, and recurrent contractures. A recent development in plastic surgery has been the "super flap" or fasciocutaneous flap. A series of axillary burn contractures released with the latissimus dorsi fasciocutaneous flap has been reported by Tolhurst. Our series of ten patients confirms that the latissimus dorsi fasciocutaneous flap is the treatment of choice for the release of severe axillary burn cont

Exosomes in Cutaneous Wound Healing and Facial Skin Regeneration: Clinical and Translational Perspectives.

Exosomes-small extracellular vesicles containing protein, lipid, and nucleic-acid cargo-modulate key pathways involved in inflammation, angiogenesis, and extracellular matrix remodeling. This narrative review synthesizes current pre-clinical and clinical evidence supporting exosome-based interventions in cutaneous wound healing and facial skin regeneration, with attention to delivery strategies, translational limitations, and regulatory considerations relevant to facial plastic and reconstructive surgery. Across multiple in vivo models, exosome-based therapies have been associated with acceler

Management of Benign Biliary Stricture in Chronic Pancreatitis.

Endoscopic therapy is the first line of management for chronic pancreatitis (CP)-related benign biliary strictures. Multiple plastic stents (MPS) exchanged at regular intervals and temporary placement of fully covered self-expanding metal stents (FCSEMS) are preferred modalities of endotherapy. FCSEMS placement is non-inferior to MPS and requires fewer sessions of endoscopic retrograde cholangiopancreatography than MPS placement. The presence of head calcifications, severe CP, and length of stricture are predictors of failure or recurrence after endotherapy. Failure of endotherapy should be co

Herpes Zoster after Botulinum Toxin Combined with Hyaluronic Acid Injection.

Botulinum toxin and hyaluronic acid injections are commonly used in plastic surgery. However, these treatment methods can also cause adverse events. This article reports a case of herpes zoster that occurred several days after botulinum toxin and hyaluronic acid injections. Antiviral treatment of herpes zoster virus proved successful in managing this complication. Furthermore, several articles concerning injection therapy-induced herpes zoster are reviewed. It is suggested that clinicians should consider herpes zoster in the differential diagnosis of unilateral erythema and papules, following

Whose surgery is this? Endometriosis of the round ligament.

We report two cases of endometriosis of the round ligament in a 37 and 26 - years old women, with a lump in their right groin and catamenial pain referred to plastic surgery department. MRI showed in both cases nodular lesion in the right inguinal region. The patological examination of the surgical specimen revealed endometriosis of the round ligament. The presence of a groin mass with pain increasing during the menstrual period must raise the suspicion of inguinal endometriosis and a gynecological evaluation is essential to provide a correct management of this rare condition.
