Malignant hyperthermia: a case report in Thai Anesthesia Incidents Study (THAI Study).

A 3-year-old Thai boy underwent open reduction and internal fixation with K-wire of condylar fracture of humerus under general anesthesia. The patients developed generalized muscle regidity, masseter muscle spasm, elevated creatinine kinase, high temperature (39.3 C), inappropriate tachycardia, and arterial base excess was more than-8 mEq/L. The clinical grading scale of diagnosis of malignant hyperthermia was 58 (grade D6; almost certain malignant hyperthermia). Succinylcholine has been identified as the trigger agent, as other possible trigger agents were not involved. The treatment included

Late presentation of a displaced subcapital fracture of the hip in transient osteoporosis of pregnancy.

Transient osteoporosis of the hip is rare and has two demographic peaks, one during the third trimester of pregnancy and the other around the fifth to sixth decade of life. During pregnancy, osteoporosis presents with insidious onset of hip pain and antalgic limp with no antecedent infection or trauma. Radiographs show pronounced osteopenia of the femoral head and neck with preservation of the joint space. 1 Bone scan and magnetic resonance imaging are sensitive but not specific for diagnosis, and laboratory studies are typically normal. 2 We present the case of a patient with a displaced, gro

Wrist arthrography in acute carpal injuries.

A prospective study of 10 patients with acute complex carpal dislocations was undertaken. Preoperative wrist arthrograms were done in these patients. We then observed the ligamentous tear through both volar and dorsal approaches at the time of open reduction. Perilunate dislocation was always an integral part of the pathology in all patients, except in one patient with radiocarpal dislocation. Communication between the radiocarpal and midcarpal joint was found in all patients, except in one patient with radiocarpal fracture-dislocation. Volar extrusion of the contrast material occurred in all

Dislocation of the hip with traumatic separation of the capital femoral epiphysis: report of a case with successful outcome.

Experience with a successfully treated transepiphyseal fracture-dislocation of the femoral head in a 4-year-old boy, a review of 6 similar injuries from the literature, and consideration of the anatomy of the immature femoral head and neck, suggest that the treatment of choice for this injury is immediate open reduction and internal fixation with a smooth pin followed by cast protection. Contrary to recommendations in the literature, closed manipulation does not seem justified at any time, since it could eliminate residual blood supply to the femoral head by disruption of the soft tissue hinge

Minimally invasive treatment of tibial plateau depression fractures using balloon tibioplasty: Clinical outcome and absorption of bioabsorbable calcium phosphate cement.

The exact reconstruction of the tibial plateau and articular surface is the main operative aim in the treatment of tibial plateau depression fractures. For selected cases, a novel technique with the use of balloon tibioplasty in combination of bioabsorbable calcium phosphate cement is available. In this study, the first objective was to answer the question whether the clinical outcome parameters after balloon tibioplasty are comparable to open reduction procedures described in the literature. Secondly, we asked whether the cement absorption is safe in relation to adverse effects like osteolysi

Strategies for Proximal Femoral Nailing of Unstable Intertrochanteric Fractures: Lateral Decubitus Position or Traction Table.

BACKGROUND: The aim of this prospective randomized study was to compare the traction table and lateral decubitus position techniques in the management of unstable intertrochanteric fractures. METHODS: Eighty-two patients with unstable intertrochanteric fractures between 2011 and 2013 were included in this study. All patients were treated surgically with the Proximal Femoral Nail Antirotation implant (DePuy Synthes). Patients were randomized to undergo the procedure in the lateral decubitus position (42 patients) or with the use of a traction table (40 patients). Patients whose procedure was n

Assessment of "YouTube" Content for Distal Radius Fracture Immobilization.

OBJECTIVE: Distal radius fractures (DRFs) are the most common orthopedic fractures, with >70% of cases treated by closed immobilization using a short arm cast or a sugar tong splint. However, inadequate immobilization is a risk factor for loss of reduction requiring repeat reduction or surgical treatment. Therefore, education of clinical skills for appropriate immobilization of DRFs is important. With the increasing use of web-based information by medical learners, our aim was to assess the quality and quantity of videos regarding closed immobilization of DRFs on YouTube. DESIGN: Retrospectiv

Novel concepts: emerging data and the role of extended prophylaxis following hip fracture surgery.

The typical duration of hospitalization following major knee and hip surgeries may not provide adequate time for effective prophylaxis of venous thromboembolism (VTE) when inpatient therapy only is administered. Results from a clinical trial that extended the duration of VTE prophylaxis after hip fracture surgery are reviewed. The risk of VTE after major knee and hip arthroplasty has been demonstrated to extend into the outpatient period. Although extending the duration of VTE prophylaxis has been demonstrated to decrease the incidence of VTE following total hip replacement, no randomized, dou

Computed tomography-guided fixation of unstable posterior pelvic ring disruptions.

Open reduction and internal fixation (ORIF), the current treatment of choice of posterior pelvic ring disruptions with instability, has significant disadvantages. These include relatively "blind" placement of the fixation screws, infection, exsanguinating hemorrhage, and high wound complication rates. We feel fluoroscopy does not offer significant clarity in defining the posterior structure. Advantages of computed tomography (CT)-guided sacral fixation are direct visualization of the course of the screws and absence of significant wound complications. This technique provides superior visualiza

Proximal Femoral Fractures Following Ipsilateral Transfemoral Amputation in Working-Age Men: A Report of Two Cases.

Lower limb amputation is associated with progressive loss of bone mineral density (BMD) in the residual limb, particularly at the amputated-side proximal femur. However, proximal femoral fractures in working-age individuals with transfemoral amputation who are independently ambulatory with a prosthesis remain insufficiently described. We report the cases of two working-age men who sustained ipsilateral proximal femoral fractures after minor falls at home while not wearing their prostheses. Case 1 sustained a left femoral neck fracture classified as Garden grade I six years after amputation, wh

Locking compression plate fixation of Vancouver type-B1 periprosthetic femoral fractures.

BACKGROUND: Fractures occurring at or near the distal tip of a hip prosthesis with a stable femoral stem (Vancouver type-B fractures) are associated with many complications because of the inherently unstable fracture pattern. Locking compression plates use screws that lock into the plate allowing multiple points of unicortical fixation. Such unicortical fixation may lower the risk of damage to the cement mantle or a stable femoral stem during the treatment of a periprosthetic femoral fracture. The purpose of this study was to analyze clinically and radiographically a group of patients with a V

Outcomes and complications after dual plate vs. single plate fixation of displaced mid-shaft clavicle fractures: A systematic review and meta-analysis.

INTRODUCTION: Open reduction and internal fixation (ORIF) of displaced midshaft clavicle fractures is associated with higher union rates and earlier functional recovery. However, ORIF with plate fixation is associated with complications including implant irritation and implant failure. Dual plate fixation provides fixation in orthogonal planes, and uses a lower profile fixation technique in comparison to pre-contoured and surgeon-contoured small-fragment locking plates, which may be more prominent. The objective of this study was to conduct a systematic review to summarize outcomes and complic

Treatment of the tibial pilon fractures using the antero-medial fibula approach: Ten case series.

Pilon fractures are always results of the high-energy trauma. They are often accompanied with serious soft tissue injury, and tension blisters happened in most cases. For comminuted fractures and poor soft tissue, how to select the incision is challenging. This study aimed to explore the outcomes of the treatment of ten 43-B/C pilon fractures using an anteromedial fibula approach.Ten closed pilon fractures combined with fibula fractures were treated in our hospital from January 2015 to July 2016. Six cases were AO/OTA type 43-B and 4 cases were 43-C, including 9 males and 1 female with a mean
