Motor performance and its association with Alzheimer's-related biomarkers: a systematic review.

PURPOSE: To examine associations between neurodegenerative and inflammatory biomarkers and motor function in adults with Alzheimer's disease (AD), mild cognitive impairment (MCI), or at risk of AD. METHODS: A systematic review was conducted using MEDLINE, Web of Science, Scopus, LILACS, and CENTRAL. RESULTS: Seventeen studies (2018-2026) were included. CSF Aβ42 levels were associated with gait speed, balance, and mobility outcomes, while p-tau, p-tau181, p-tau217, and t-tau correlated with mobility impairments, dual-task performance, and reduced physical function. Higher neurofilament light …

Elucidation of blink reflex characteristics in Parkinson's disease subtypes through prepulse inhibition.

This study investigated prepulse inhibition (PPI), a brainstem reflex, in Parkinson's disease (PD) patients. We compared PPI impairment between patients with postural instability and gait difficulty (PD-PIGD) and tremor-dominant (PD-TD) subtypes and explored potential lateralization effects. Fifty PD patients and 35 healthy controls underwent pre-pulse stimulation of the finger followed by stimulation of the supraorbital nerve. Compared to controls, PD patients exhibited impaired PPI across various stimulation intervals, with a more pronounced effect in the PD-PIGD subgroup. Interestingly, no …

A Case of Lacunar Stroke in a Patient with Possible Normal Pressure Hydrocephalus (NPH).

BACKGROUND: Normal pressure hydrocephalus (NPH) is a condition seen in the elderly, characterized by gait disturbances, urinary incontinence, and cognitive impairment. However, sudden onset neurological deficits suggest a vascular event rather than NPH. METHODOLOGY: We report a case of a 65-year-old male who presented with sudden onset gait difficulty and speech impairment, initially suspected to be idiopathic NPH. The diagnostic process and management strategies are discussed. RESULTS: The patient's symptoms, including insidious gait difficulties, speech impairment, and memory loss, led to …

Evaluating Activity Levels in Individuals with TBI using IMU.

Traumatic Brain Injury (TBI) is one of the leading causes of sensorimotor deficits in adults and often results in gait deficits. In this context, inertial measurement units (IMU) have emerged as an effective tool for objectively quantifying activity levels. This work evaluates IMU-derived activity levels using activity counts from ankles, wrists, and sacrum during walking and stair climbing tasks in individuals with TBI and healthy controls, and relates these counts to functional measures (timed up and go, dynamic gait index, and gait speed). Results show that TBI participants had reduced acti…

A dataset of optical camera and IMU sensor derived kinematics of thirty transtibial prosthesis wearers.

Motion analysis has played a crucial role in providing gait analysis for prosthetic users. Understanding kinematics in motion analysis allows for the evaluation of the effects of prostheses and the development of a prosthetic component design that aids in walking within the community. However, there are currently limited open datasets available to study the locomotion of individuals using transtibial prostheses, and most research studies involve a limited number of participants. This dataset shows 30 transtibial prosthesis users walking comfortably on a 10-meter walkway inside a laboratory. We…

A Case of Subacute Degeneration of the Spinal Cord Due to Vitamin B12 Deficiency Triggered by Nitrous Oxide Use.

In this case report, we discuss a young female who presented to the emergency department with a recent onset of weakness, paraesthesia, and gait disturbance suggestive of peripheral neuropathy and gait ataxia. This was attributed to the occasional use of recreational nitrous oxide (N2O) over the past 10 months. Subacute combined degeneration of the spinal cord is a condition affecting the lateral and posterior columns of the spinal cord, mainly caused by demyelination. The use of recreational N2O depletes the levels of vitamin B12 thus leading to this demyelination of the nervous system. Physi…

Motoric cognitive risk syndrome: Integration of two early harbingers of dementia in older adults.

Dementia is characterized by a long preclinical phase that may last years to decades before the onset of mild cognitive impairment. Slow gait speed and subjective memory complaint commonly co-occur during this preclinical phase, and each is a strong independent predictor of cognitive decline and dementia. Motoric cognitive risk (MCR) syndrome is a pre-dementia syndrome that combines these two early harbingers of dementia. The risk of cognitive decline or dementia is stronger for MCR than for either slow gait speed or subjective memory complaint alone. Slow gait speed and subjective memory comp…

Use it or lose it? Effects of age, experience, and disuse on crawling.

What happens to early acquired but later abandoned motor skills? To investigate effects of disuse on early-developing motor skills, we examined crawling in two groups of habitual crawlers (34 6-12-month-old infants and five adults with Uner Tan Syndrome) and two groups of rusty crawlers (27 11-12-year-old children and 13 college-aged adults). Habitual crawlers showed striking similarities in gait patterns, limbs supporting the body, and crawling speed, despite dramatic differences in crawling practice, posture, and body size. Habitual crawlers trotted predominantly, whereas rusty crawlers show…

Therapeutic Experience on Stance Control Knee-Ankle-Foot Orthosis With Electromagnetically Controlled Knee Joint System in Poliomyelitis.

A 54-year-old man with poliomyelitis had been using a conventional, passive knee-ankle-foot orthosis (KAFO) with a drop ring lock knee joint for about 40 years. A stance control KAFO (SCKAFO) with an electromagnetically controlled (E-MAG) knee joint system was prescribed. To correct his gait pattern, he also underwent rehabilitation therapy, which included muscle re-education, neuromuscular electrical stimulation, strengthening exercises for the lower extremities, and balance training twice a week for about 4 months. Both before and after rehabilitation, we conducted a gait analysis and assess…

Aging associated changes in the motor control of ankle movements in the brain.

Although age-related gait changes have been well characterized, little is known regarding potential functional changes in central motor control of distal lower limb movements with age. We hypothesized that there are age-related changes in brain activity associated with the control of repetitive ankle movements, an element of gait feasible for study with functional magnetic resonance imaging. We analyzed standardized functional magnetic resonance imaging data from 102 right-foot dominant healthy participants aged 20-83 years for age-associated effects using FSL and a meta-analysis using coordin…

The assessment of walking skills: Italian version.

Apraxia is a neuropsychological disorder that impairs voluntary, purposeful movements, with "Gait apraxia" specifically affecting walking. Because of the lack of standardized diagnostic tools, in Italian, we translated and adapted the "Assessment of Walking Skills" (AWS) scale, originally developed for identifying gait apraxia in Alzheimer's patients. The AWS includes 42 items that evaluate trunk and leg movements, useful for comparing the performance between patients and healthy controls. This translation and adaptation represent a critical step toward standardizing the AWS scale for the Ital…

Loeys-Dietz syndrome with bilateral radial head dislocations: a case report.

Loeys-Dietz syndrome is characterised by vascular aneurysms, hypertelorism, and a bifid uvula. We report on an 11-year-old boy with Loeys-Dietz syndrome who presented with bilateral radial head dislocations and severe osteopaenia with changes of avascular necrosis in both hips causing an out-toeing, wide gait. Considering the poor prognosis for elbow movement and possible radial head dysplasia, surgical reduction of the radial heads was deferred. A subtrochanteric de-rotation osteotomy of the left hip was performed to improve the gait.

Review of preorthotic biomechanical considerations.

The application of an ankle-foot orthosis in poststroke hemiplegia can improve the mechanics of ambulation and functional gait. However, prior to the fitting of an orthotic, the clinician should perform a thorough assessment of the biomechanical elements contributing to mobility problems within the patient. Many of these problems such as joint contracture or weakness should be addressed through physical therapy interventions prior to orthotic prescription. This article reviews the common biomechanical elements that contribute to mobility and gait disability in patients with stroke with special…
