The impossibility problem: When public health policies require what structural conditions prevent.

"Bubbles" were introduced as a nonpharmacological strategy in 2020 COVID-19 public health guidelines, comparable to social distancing measures. Despite the seemingly clear metaphor of bubbles, public communication and guidelines for bubbling across Canada and globally created a context of ambiguity, confusion, and inconsistency. It was up to individual citizens, households, groups, schools, and workplaces to know what a bubble was and how to implement bubbling practices. In spite of its widespread adoption as a pandemic intervention, little is known about how the public made sense of this guid

Photodynamic antimicrobial and antineoplastic therapy in veterinary medicine: current status, challenges, and perspectives.

Significant causes of morbidity in veterinary medicine are tumourigenic diseases and pathogenic infections, which pose significant threats to animal health. Traditional methods for treating tumours and bacterial infections primarily involve chemotherapy and antibiotic therapy, both of which are associated with drug resistance and adverse effects. Photodynamic therapy (PDT) is gaining increasing interest in veterinary clinical practice because of its distinctive benefits such as spatiotemporal selectivity, low toxicity, fewer negative effects, and reduced risk of drug resistance. This review su

How to Leverage Implementation Science to Achieve Equity in Nephrology Care Delivery.

There are several evidence-based practices within nephrology that have the potential to meaningfully improve patient outcomes and mitigate disparities. However, these practices are inconsistently implemented in routine care, contributing to low-quality care, persistent health disparities, and high health care costs. To address these care gaps quickly, nephrology would benefit from leveraging implementation science: a multidisciplinary field that seeks to close the gap between evidence and practice by examining the optimal approach to help individuals, groups, organizations, and communities imp

[Selection of knowledge translation strategies to implement best practices on nonpharmacological prevention of delirium in intensive care unit].

BACKGROUND: Delirium prevention in the ICU should focus on a non-pharmacological approach. However, these recommendations are not always applied by care providers. OBJECTIVE: To select knowledge translation strategies to facilitate the implementation of non-pharmacological best practices to prevent delirium in the ICU. METHOD: A consensus study was conducted. Barriers and facilitators to the implementation of nonpharmacological methods, and knowledge translation strategies, were identified in two nominal groups. A context assessment was also carried out. Nine professionals and one patient-pa

Do large pragmatic randomised trials change clinical practice?: assessing the impact of the Distal Radius Acute Fracture Fixation Trial (DRAFFT).

AIMS: Our aim, using English Hospital Episode Statistics data before during and after the Distal Radius Acute Fracture Fixation Trial (DRAFFT), was to assess whether the results of the trial affected clinical practice. PATIENTS AND METHODS: Data were grouped into six month intervals from July 2005 to December 2014. All patient episodes in the National Health Service involving emergency surgery for an isolated distal radial fracture were included. RESULTS: Clinical practice in England had not changed in the five years before DRAFFT: 75% of patients were treated with plate fixation versus 12%

Practices to optimise gastrointestinal nematode control on sheep, goat and cattle farms in Europe using targeted (selective) treatments.

Due to the development of anthelmintic resistance, there have been calls for more sustainable nematode control practices. Two important concepts were introduced to study and promote the sustainable use of anthelmintics: targeted treatments (TT), where the whole flock/herd is treated based on knowledge of the risk, or parameters that quantify the severity of infection; and targeted selective treatments (TST), where only individual animals within the grazing group are treated. The aim of the TT and TST approaches is to effectively control nematode-induced production impacts while preserving anth

Sheep management and cryptosporidiosis in central Mexico.

To establish the association between sheep farm management practices and cryptosporidiosis in Central Mexico, 37 farms were invited and questionnaires answered by owners. Faeces samples were taken from lambs under 3 months old and ewes in the farms. Parasites were identified by modified Ziehl-Neelsen stain. Questionnaire included: general farm information, facilities characteristics, facilities cleaning measures, water use and farm and animal management practices. Farms were grouped in a low ( Pound 3,37%), medium (3.38-49.9%) and high basis ((3)50%) according to prevalence. Associations (OR)

The impact of a structured clinical training course on interns' self-reported confidence with core clinical urology skills.

BACKGROUND: Undergraduate training in core urology skills is lacking in many Irish training programmes. AIMS: Our aim was to assess newly qualified doctors' experience and confidence with core urological competencies. METHODS: A questionnaire survey covering exposure to urology and confidence with core clinical skills was circulated to all candidates. The group then attended a skills course covering male/female catheterisation, insertion of three-way catheters, bladder irrigation and management of long-term suprapubic catheters. The groups were re-surveyed following the course. RESULTS: For

Emergency Department Clinicians' Needs for User-Centered Antibiotic Guideline: Insights from a Focus Group Interview.

BACKGROUND: Institution-specific antibiotic guidelines are core components of effective antibiotic stewardship programs; however, their practical use in emergency departments (EDs), where empirical antibiotic decisions are made under time pressure and diagnostic uncertainty, remains limited. This study explored ED clinicians' perspectives on antibiotic prescribing and identified key elements for developing ED-specific antibiotic guidelines. MATERIALS AND METHODS: Focus group interviews were conducted between October 12 and October 26, 2022, at four tertiary hospitals in Korea. A total of 16 p

Redesigning Primary Care Via Health Systems and Communities.

PROBLEM: Traditional primary care practices have not adequately adapted to the clear consensus on the importance of community and social factors in child health. APPROACH: We describe how primary care can improve child and community health through community partnerships that integrate social care into healthcare systems. Specifically, we discuss a National Academy of Medicine heuristic for social care integration into practice and provide examples of primary care/social care integration. Practices/clinicians may feasibly address community and social factors through individual-level social car

An examination of the cognitive and affective processes in errorless motor learning: study protocol.

Acquiring movement skills is crucial across the lifespan, supporting an individual's participation in sports and physical activities. Errorless motor learning, which promotes success during practice, has been shown to be effective for acquiring or reacquiring (e.g., following injury/disease) movement skills. This research explores the underlying processes of errorless motor learning in children and older adults. We will investigate two distinct processes associated with motor learning: (i) cognitive processing measured by movement variability and prefrontal cortex (PFC) activity and (ii) affec

Enhancing standardization in anorectal manometry testing: the impact of a national position paper.

BACKGROUND: The 2019 International Anorectal Physiology Working Group published the 'London Classification', providing recommendations for anorectal manometry (ARM) and balloon expulsion testing (BET). Despite this, the extent of guideline adoption in clinical settings was unclear. This study aimed to evaluate ARM and BET practices across Israel, identify deviations from these international guidelines, and measure the impact of a newly instated national position paper. METHODS: In late 2020, gastroenterologists from 95% of Israeli centers performing ARM responded to a comprehensive 61-item su

Exploring multidisciplinary team members' experiences of supporting eating and drinking with acknowledged risk in healthcare and aged care settings.

PURPOSE: Dysphagia is a swallowing condition which has significant health and quality of life implications. Speech-language pathologists provide assessment and support shared decision making to optimise swallowing safety. Many people elect to eat and drink with acknowledged risk. This study aimed to explore multidisciplinary team members' perceptions and experiences supporting eating and drinking at acknowledged risk decision-making and practice, and to determine resources needed to support teams in this area. METHOD: Semi-structured focus groups and one interview were facilitated with multid

Service evaluation suggests variation in clinical care provision in adults with congenital adrenal hyperplasia in the UK and Ireland.

BACKGROUND: Congenital adrenal hyperplasia (CAH) encompasses a rare group of autosomal recessive disorders, characterised by enzymatic defects in steroidogenesis. Heterogeneity in management practices has been observed internationally. The International Congenital Adrenal Hyperplasia registry (I-CAH, https://sdmregistries.org/) was established to enable insights into CAH management and outcomes, yet its global adoption by endocrine centres remains unclear. DESIGN: We sought (1) to assess current practices amongst clinicians managing patients with CAH in the United Kingdom and Ireland, with a

"If I die for touching him, let me die": a rapid ethnographic assessment of cultural practices and Ebola transmission in high-risk border regions of Tanzania.

BACKGROUND: Ebola Virus Disease (EVD) is a rare but contagious disease caused by Ebola Virus (EBOV). The first Ebola outbreaks were reported in the Democratic Republic of Congo (DRC) before subsequent reported cases in Western and East African countries, including Uganda, which borders Tanzania. Proximity to EVD-infected countries raises the prospect of cross-border transmission, raising alarm in Tanzania. This study aimed to explore the cultural practices likely to prevent or escalate EVD transmission in the event of its outbreak in the country. METHODS: This rapid ethnographic assessment em
