The authors had 302 patients with focal lesions of the liver, 232 of them were operated on. Radical operations were carried out on 96 and palliative operations on 136 patients. Operations were performed for a second time in 35 patients, radically in 24 of them. A previously conducted exploratory laparotomy is still no evidence of inoperability of the process. Of a total of 77 patients affected by hydatid cysts of the liver observed between 1983 and 1988, we examined a group of 25 patients with cysts that had migrated into the thorax. Two cases of portal hypertension due to hydatid cysts of the liver are reported. In one of the patients, symptoms were secondary to obstruction of inferior vena cava and hepatic outflow tract. The other patient was operated on with a diagnosis of extrahepatic presinusoidal portal hypertension caused by extrinsic compression of the liver by an hydatid cyst. Although hydatidosis is a benign disease, it can produce serious complications as in these reported cases. Therefore hydatidosis should be remembered amongst the causes of portal hypertension in countries where the disease is endemic.

Ultrasound is a widely used method of assessing the liver for space occupying lesions and, more recently, parenchymal liver disease. We have reviewed the ultrasound scans and reports of 11 patients with biopsy proven granulomatous liver disease. Multiple echogenic lesions 3-5 mm in diameter, each surrounded by an hypoechoic halo, were seen in the liver of all the patients and in the spleens of three patients. We performed chemo-embolisation with gel foam powder and epirubicin in 11 symptomatic patients with liver metastases from carcinoids. In all cases the symptoms regressed completely after the procedure. In two patients the liver was re-embolised because of tumour growth and both again became symptom-free. The rare phenomenon of gross intra-abdominal bleeding due to hepatic metastasis was seen in a young male who a short time before had undergone orchiectomy because of embryonal cell carcinoma. Following laparotomy, 4 liters of fresh blood were drained from the peritoneal cavity and the left lobe of the liver, containing a single large metastasis, was removed. It is stressed that when no other cause of such haemorrhage can be found in a patient known to have had malignancy, the possibility of bleeding from a metastasis should be considered and appropriate measures taken.

To compare the value of MRI and CT in evaluating hepatic alveolar echinococcosis a study was conducted on 30 patients. The liver was initially affected in all patients. At the time of examination, 15 patients had undergone partial liver resection. MRI showed no advantage over CT in demonstrating an echinococcal mass lesion. The author observed 315 patients with the amebic liver abscess. Of them in 134 (42.5%), the renal syndrome was diagnosed. The kidney damage had the course of the secondary pyelonephritis type, or in the form of a toxic kidney. On the basis of clinico-laboratory analysis, the author distinguishes the mild, moderate and severe degrees of kidney damage, recommends an adequate transfusion-infusion, antibacterial and specific therapy.

Thirteen patients with hepatic tumors, from the Boston Center for Liver Transplantation, have been transplanted among a total of 169 recipients. Ten were transplanted primarily for tumor, while three other patients harbored incidental tumors. Two perioperative deaths occurred (15%). The expression of immunoreactive alpha interferon was examined in 78 liver biopsy specimens using an indirect immunoperoxidase technique. Hydatid illness still presents a serious surgical problem and in the case of obstrukioni ikterus an operation is indispensable method in the treatment of illness. Applying the method of omentoplexia in our case we witness its justification because together with cholecystectomia and relieving "T" drainage the secretion is reduced to minimum as well as the possibility of appearing of closed purulent content. The number of hospitalised days after the operation is reduced in average from 45 to 21 days.

The collagens of normal and end-stage alcoholic cirrhotic human liver were investigated. Mild conditions of pepsin digestion were employed to preserved the more pepsin sensitive type IV collagen molecules while increasingly more harsh conditions were used which solubilized the type I and type III hepatic collagens. Cyclosporin A is a potent immunosuppressive agent that has revolutionized the care of organ transplant recipients. Recently, the use of cyclosporin A has been extended beyond the transplant setting to include certain disorders that are thought to be immunologically mediated. This review focuses on the results of cyclosporin A in the treatment of nontransplant-related liver disorders including primary biliary cirrhosis, autoimmune chronic active (lupoid) hepatitis, primary sclerosing cholangitis, schistosomiasis, acute fulminant hepatitis, and chronic granulomatous liver disease.

Light and electron microscopies were applied to examine liver punctates from 10 patients with typhoid fever. Both in the full swing of the disease and in clinical recovery (convalescence), dystrophic and necrobiotic changes along with focal cell death were found to occur in the hepatocytes, endothelial and Kupffer cells covering the sinuses. The protein-synthesizing system and mitochondria were demonstrated to be primarily destroyed. The microbes of typhoid fever were located in the hepatocytes, endothelial and Kupffer cells.
