Relationship of bruxism with oral health-related quality of life and facial muscle pain in dentate individuals.

BACKGROUND: To determine whether there is a correlation of bruxism (sleep, daytime, or both) with oral health-related quality of life and facial pain of muscular origin in dentate individuals. MATERIAL AND METHODS: Seventy-four dentate patients (complete dentition) were included in this study. These individuals had pain in the facial muscles due to temporomandibular disorder (TMD). Smokers; and those with obstructive sleep apnea, TMD of joint origin associated or not with pain, malocclusion, and cancer; and users of illicit drugs, psychiatric medications, and alcohol were excluded. Obstructiv

The Intersection of Cardiology and Sleep Medicine: Impact of Sleep-Disordered Breathing on Heart Failure Outcomes.

Sleep-disordered breathing (SDB)-including obstructive sleep apnea (OSA) and central sleep apnea (CSA)-is highly prevalent among individuals with heart failure (HF), yet frequently underdiagnosed. Increasing evidence shows that SDB and HF interact in a vicious cycle: recurrent hypoxic episodes, fluctuations in intrathoracic pressure, and autonomic dysregulation further strain cardiac function, while HF itself predisposes to the development of apnea. This interplay worsens arrhythmia burden, symptom severity, and long-term prognosis. This review will explore how OSA and CSA influence HF progres

Minute-level ECG-derived autonomic dynamics around obstructive apnea events: A peri-event analysis of the PhysioNet apnea-ECG database.

BACKGROUND: Obstructive sleep apnea (OSA) is linked to arrhythmia and cardiovascular morbidity, but the cardiac response to individual obstructive events is incompletely characterised at minute-scale resolution. We aimed to determine whether ECG-derived autonomic features distinguish apnea from nonapnea minutes and whether these features change before scored event onset. METHODS: We performed a secondary analysis of the PhysioNet Apnea-ECG learning set (34 subjects after exclusion of one duplicated recording; 30 male, 4 female; mean age 47.0 ± 9.7 years; AHI 0-82.4 events/h). For each 1-min s

Cardiac resynchronization therapy and obstructive sleep-related breathing disorder in patients with congestive heart failure.

OBJECTIVES: To assess the impact of cardiac resynchronization therapy (CRT) with or without atrial overdrive pacing, on sleep-related breathing disorder (SRBD). INTRODUCTION: CRT may have a positive influence on SRBD in patients who qualify for the therapy. Data are inconclusive in patients with obstructive SRBD. METHODS: Consenting patients eligible for CRT underwent a baseline polysomnography (PSG) 2 weeks after implantation during which pacing was withheld. Patients with an apnea hypopnea index (AHI) ≥15 but <50 were enrolled and randomized to atrial overdrive pacing (DDD) versus atrial s

[Sleep disturbances from the internist's viewpoint].

Sleep disorders are common and can be caused by somatic diseases. Chronic pain as well as heart and lung conditions are able to interfere with the normal sleep pattern. Obstructive sleep apnea is caused by a deranged central control of breathing occurring during sleep and leads to a sleep-fragmentation and consequently to daytime hypersomnolence. A problem oriented meticulous history can give valuable insights and distinct diagnostic considerations form the basis for an eventually successful therapeutic approach. Polysomnography is done in particularly puzzling cases and for evaluation of inva

Continuous positive airway pressure for sleep-related breathing disorders in multiple system atrophy: long-term acceptance.

BACKGROUND AND PURPOSE: To assess the long-term acceptance of non-invasive nasal continuous positive air pressure (CPAP) in multiple system atrophy (MSA) patients with polysomnographic (PSG)-confirmed sleep apneas and/or nocturnal stridor. PATIENTS AND METHODS: Sleep-related breathing disorders were investigated by PSG in 22 MSA patients in whom stridor and sleep-related respiratory disturbances were clinically suspected. Patients in whom the first PSG disclosed either a sleep apnea/hypopnea index (AHI)> or =10 or stridor with or without apneas underwent a second PSG for CPAP titration. RESU

The Prevalence of Central Sleep Apnea in New Referrals to a Sleep Surgery Clinic.

OBJECTIVE: Surgical treatment of non-obstructive sleep apnea (OSA) pathology poses the risk of inappropriate surgical indications. Herein, we sought to determine the prevalence of non-OSA respiratory disorders, specifically central sleep apnea (CSA), in new referrals to a Sleep Surgery Clinic. STUDY DESIGN: Prospective observational review. SETTING: Tertiary care academic medical center. METHODS: In a sleep surgery clinic cohort, the presence of clinically significant CSA was defined as having >25% of the total apnea-hypopnea index (AHI) being central and/or mixed events. Demographics, como

Dose-response relationship between positive airway pressure therapy and excessive daytime sleepiness: the HomePAP study.

STUDY OBJECTIVES: The clinical benefits of positive airway pressure (PAP) therapy for obstructive sleep apnea are assumed to require adherent PAP usage, defined by the Centers for Medicare & Medicaid Services as ≥ 4 hours of use ≥ 70% of nights. However, this definition is based on early data and does not necessarily capture improvements at subthreshold adherence. We explored dose-response relationships between PAP adherence measures and excessive daytime sleepiness from the HomePAP randomized controlled trial. METHODS: Participants aged ≥ 18 years with an apnea-hypopnea index ≥ 15 events/h a

CAISR: achieving human-level performance in automated sleep analysis across all clinical sleep metrics.

STUDY OBJECTIVES: To develop and validate a Complete Artificial Intelligence Sleep Report system (CAISR), a system for comprehensive automated sleep analysis, including sleep staging, arousal detection, apnea identification, and limb movement analysis. METHODS: We utilized a large diverse dataset from four cohorts (MGH, MESA, MrOS, SHHS) comprising 25,749 participants to develop CAISR. Following American Academy of Sleep Medicine (AASM) guidelines, CAISR performs four tasks: it stages sleep into five categories (Wake, NREM 1, NREM 2, NREM 3, REM), detects arousals, detects and classifies brea

Comparison of clinical pathways for hypoglossal nerve stimulation management: in-laboratory titration polysomnography vs home-based efficacy sleep testing.

STUDY OBJECTIVES: We conducted this study to evaluate whether laboratory or home-based hypoglossal nerve stimulation (HNS) management would have equivalent objective and subjective obstructive sleep apnea outcomes 6 months after activation. METHODS: Patients undergoing standard-of-care HNS implantation were randomly assigned in a prospective, multicenter clinical trial to either a 3-month postactivation in-laboratory titration polysomnography (tPSG) or an efficacy home sleep study (eHST) with tPSG by exception for eHST nonresponders at 5 months. Both groups underwent an eHST 6 months postacti

[Cheyne-Stokes respiration in patients with chronic heart failure: only a diagnostic marker or also a cardiovascular risk factor?].

Sleep disordered breathing with predominant obstructive or central apnea is an under-recognized but highly prevalent comorbidity in patients with chronic heart failure. As the severity of heart failure increases the prevalence of central sleep apnea (CSA) and Cheyne-Stokes respiration (CSR) is also much more frequent. Cheyne-Stokes respiration is characterized by alternating periods of crescendo and decrescendo respiration followed by central apnea. Present data indicate that CSA-CSR is not only a compensatory response to severe heart failure but also a predictor of worse prognosis. However th

[Central apnea developing during treatment with a mandibular advancement device].

INTRODUCTION: Mandibular advancement (MA) has emerged over the last decade as an alternative solution to nasal continuous airway pressure (nCPAP) for the treatment of obstructive sleep apnea syndrome (OSAS). OBSERVATION: We report the case of a patient with history of chronic atrial fibrillation and moderate supine-dependent OSAS in whom central sleep apneas developed during treatment by a bi-bloc MA device. Central apneas increased with the level of MA and preferentially occurred in the supine position. We hypothesized that mouth opening under excessive mandibular advancement in supine posit

Bilevel positive airway pressure treatment of sleep apnea syndrome in a patient with Crouzon syndrome.

Crouzon syndrome (CS) is a hereditary disorder characterized by deformities of the face and skull brought on by syndromic craniosynostosis or the premature fusion of coronal sutures. Pediatric patients with Crouzon syndrome have a high likelihood of developing obstructive sleep apnea (OSAS), primarily due to skeletal hypoplasia and facial dysmorphism. Treatment of OSAS in patients with Crouzon syndrome requires multidisciplinary and sequential treatment planning because Crouzon syndrome often has different phenotypes with varying severity in OSA and facial dysmorphism. In this clinical case, w

Central Sleep Apnea.

Central sleep apnea (CSA) is characterized by intermittent repetitive cessation and/or decreased breathing without effort caused by an abnormal ventilatory drive. Although less prevalent than obstructive sleep apnea, it is frequently encountered. CSA can be primary (idiopathic) or secondary in association with Cheyne-Stokes respiration, drug-induced, medical conditions such as chronic renal failure, or high-altitude periodic breathing. Risk factors have been proposed, including gender, age, heart failure, opioid use, stroke, and other chronic medical conditions. This article discusses the prev

Injection of L-glutamate into the insular cortex produces sleep apnea and serotonin reduction in rats.

PURPOSE: Obstructive sleep apnea (OSA) is primarily characterized by repetitive episodes of complete or partial obstruction of airflow during sleep. The neuronal and cellular mechanisms underlying this process are not fully understood, although the focus of some studies is on putative serotonin (5-HT) mechanisms, and serotonergic therapy may be beneficial to OSA patients. This study aimed to demonstrate possible changes in 5-HT associated with induction of OSA in a rat model. METHODS: Apnea was induced in rats by injection of L-glutamate (L-Glu) into the insular cortex. We examined changes in

Pleiotropic effects of renal denervation beyond blood pressure reduction.

Overactivation of sympathetic nervous system (SNS) has a key role in the pathophysiology of hypertension, as well as in chronic kidney disease, heart failure, atrial fibrillation, type 2 diabetes mellitus, obstructive sleep apnea, and anxiety disorders. Renal denervation (RDN) has been proven to lower blood pressure (BP) in hypertensive patients by reducing SNS activity. Beyond hypertension, data from a limited number of preclinical studies and clinical trials suggest that RDN may also be nephroprotective, have beneficial hemodynamic and clinical effects in patients with heart failure, reduce

MRI study of regional variations of pharyngeal wall compliance in cats.

Upper airway compliance indicates the potential of the airway to collapse and is relevant to the pathogenesis of obstructive sleep apnea. We hypothesized that compliance would vary over the rostral-to-caudal extent of the pharyngeal airway. In a paralyzed isolated upper airway preparation in cats, we controlled static upper airway pressure during magnetic resonance imaging (MRI, 0.391-mm resolution). We measured cross-sectional area and anteroposterior and lateral dimensions from three-dimensional reconstructed MRIs in axial slices orthogonal to the airway centerline. High-retropalatal (HRP),
