Clinical data suggest that optic neuropathy and retinal ganglion cell loss are the main cause of visual decline in patients with familial dysautonomia, but this has not previously been confirmed by pathological analyses. We studied retinas and optic nerves in 6 eyes from 3 affected patients obtained at autopsy. Analyses included routine neurohistology and immunohistochemistry for neurofilaments, cytochrome c oxidase (COX), and melanopsin-containing ganglion cells. We observed profound axon loss in the temporal portions of optic nerves with relative preservation in the nasal portions; this corr

INTRODUCTION: Situs inversus of the optic nerve is a congenital anomaly characterised by the emergence of the vessels in the retina towards the nose rather than in a temporal direction. It is caused by an anomalous insertion of the optic stalks into the optic vesicle that gives rise to dysversion of the head of the optic nerve. It is not an isolated condition and usually appears jointly with tilted disc syndrome and in patients with myopia. It is characterised by the presence of inferior conus atrophy, deficiencies in the temporal visual field, refraction defects and ambliopy.

Optic disc drusen (ODD) are benign acellular calcified deposits that typically form early in life. They do not normally cause symptoms, so ODD are often detected incidentally as an abnormal or potentially swollen optic disc appearance. Distinguishing ODD from true optic disc edema is essential, and now, multimodal imaging, particularly enhanced-depth imaging optical coherence tomography, can detect ODD accurately and reduce the need for further investigations. The origins of ODD are unknown, although the potentially unique finding of heavily calcified extracellular mitochondria in the optic di

A man in his 30s with a prior diagnosis of presumed primary open-angle glaucoma presented with progressive visual deterioration and headaches. Initial optic disc cupping and the established glaucoma diagnosis suggested glaucomatous optic neuropathy; however, visual field defects respecting the vertical meridian and optical coherence tomography changes inconsistent with glaucoma prompted neuroimaging, which revealed a large invasive pituitary macroadenoma compressing the optic chiasm, later confirmed as a mixed prolactin-secreting and growth hormone-secreting tumour. Following medical treatment

Fibrous dysplasia (FD) is a progressive and benign osteodystrophic disease where cranial bones are most commonly affected. In this case report, we present a 27-year-old patient with previous diagnosis of FD who was referred to our clinic with sudden loss of visual acuity and color discrimination. Examination of the right eye was normal, whereas visual acuity on the left eye was 6/9 and color vision (CV) with Ishihara test plates was 9/12. The visual field (VF) demonstrated a peripheral concentric defect on the left eye. As visual acuity in the left eye decreased to 6/30 and computed tomography

We present a patient with concurrent pigmentary glaucoma, bilateral central serous chorioretinopathy and unilateral optic disc pit, and propose a possible association of these conditions. Comprehensive ophthalmic examination of a 36-year-old man who was complaining of blurry vision and pain in the eyes showed reduced visual acuity on the left eye, elevated intraocular pressure in the right eye, bilateral signs of pigment dispersion syndrome, and bilateral central serous chorioretinopathy, combined with optic disc pit in the left eye. Visual field and optical coherence tomography findings demon

Occult macular dystrophy (OMD), also known as Miyake disease, is a rare, autosomal dominant disease. Symptom onset is variable; the majority of patients present in their late twenties with a progressive bilateral decline of central vision, photophobia, and central scotoma. The mean visual acuity is 20/80 but can vary from 20/30 to 20/200 or worse. No significant abnormality is seen in the fundus or fluorescein angiography. Optical coherence tomography (OCT) findings range from blurring of the interdigitation zone (IZ) and the ellipsoid zone (EZ) with thinning of the outer nuclear layer to exti

We present nnnnnin vivo volumetric depth-resolved vasculature images of the anterior segment of the human eye acquired with phase-variance based motion contrast using a high-speed (100 kHz, 105 A-scans/s) swept source optical coherence tomography system (SSOCT). High phase stability SSOCT imaging was achieved by using a computationally efficient phase stabilization approach. The human corneo-scleral junction and sclera were imaged with swept source phase-variance optical coherence angiography and compared with slit lamp images from the same eyes of normal subjects. Different features of the ri

Retinal ganglion cells (RGCs) project their long axons, composing the optic nerve, to the brain, transmitting the visual information gathered by the retina, ultimately leading to formed vision in the visual cortex. The RGC cellular system, representing the anterior part of the visual pathway, is vulnerable to mitochondrial dysfunction and optic atrophy is a very frequent feature of mitochondrial and neurodegenerative diseases. The start of the molecular era of mitochondrial medicine, the year 1988, was marked by the identification of a maternally inherited form of optic atrophy, Leber's heredi

The afferent visual pathway is commonly affected in MS. Assessment of the afferent visual pathway using clinical, imaging and electrophysiological methods not only provides insights into the pathophysiology of MS, but also provides a method of investigating potential therapeutic measures in MS. This review summarises the various assessment methods, in particular imaging techniques of the visual pathway. Retinal nerve fibre layer (RNFL) thickness is usually reduced following an episode of optic neuritis. Techniques such as optical coherence tomography, scanning laser polarimetry, and confocal s

Continuous depth-of-interaction (cDOI) detectors enable single-ended readout in positron emission tomography (PET) by encoding the interaction depth into the scintillation light distribution. This study presents a comprehensive performance optimization of a cDOI detector based on light distribution tailoring with crossed triangular-shaped reflectors by analyzing the effects of optical geometry based on various DOI decoding algorithms. Two parameters were systematically varied: the degree of light confinement through (1) optical segmentation and (2) crystal pitch adjustment. Five DOI decoding s

Oral submucous fibrosis (OSF) is a chronic, potentially malignant disorder characterized by progressive stromal fibrosis and epithelial atrophy, leading to functional loss and an increased risk of malignant transformation. Areca nut consumption remains the principal etiological factor in South and Southeast Asia. Despite its distinct clinicopathological features, OSF assessment relies largely on clinical examination and invasive biopsy, underscoring the need for non-invasive approaches capable of interrogating tissue structure and composition. Optical imaging (OI) technologies, including confo

Potentially blinding corneal diseases alter the morphology of the human cornea. At the early stages of disease development, these changes occur at the cellular level. The ability to visualize and quantify such changes can lead to early diagnostics of corneal pathologies, which is pivotal for the long-term preservation of vision. Here we present a Powell Lens-based Line-Field Optical Coherence Tomography system that combines high spatial resolution (2.4 μm × 2.2 μm × 1.7 μm (x × y × z)) in biological tissue, sufficient to resolve individual cells, high sensitivity (90.5 dB), sufficient to image

BACKGROUND: Headache and monocular visual disturbance are worrisome pediatric presenting complaints in the emergency department. Appropriate and timely initial evaluation is critical. Most would opt for urgent computer tomography in such cases. Pediatric optic neuritis is a rare condition and is better evaluated by magnetic resonance imaging. With the increase in the use and scope of bedside ultrasound, there might be a potential role for transorbital ultrasound to be part of the emergency department evaluation of pediatric optic neuritis.
CASE REPORT: This is the first pediatric case report

BACKGROUND: Optic nerve involvement is not considered in dissemination in space (DIS) or time (DIT) of multiple sclerosis (MS) lesions.
OBJECTIVES: To evaluate frequency of optic nerve involvement using three-dimensional (3D)-double inversion recovery (DIR) sequence in clinically isolated syndrome (CIS) and to measure its relationship with DIS and DIT (2010 and 2017 McDonald criteria).
METHODS: From November 2013 to August 2016, 57 CIS patients underwent 3T-magnetic resonance imaging (3T-MRI) including 3D-DIR sequence and optical coherence tomography (OCT) at 3 months after CIS. We assessed

PURPOSE: To report a rare case of coexisting Arnold-Chiari malformation type I (CM-I) and osteopetrosis revealed by incidental optic disc elevation, and to review the literature on similar presentations to highlight diagnostic challenges and the role of fundoscopic examination.
METHODS: We describe a 22-year-old female with incidental bilateral optic disc edema discovered during ophthalmoscopy training. Despite normal visual acuity and absence of visual symptoms, further imaging revealed Chiari I malformation and osteopetrosis. Optical coherence tomography and fluorescein angiography showed d

Optic disc swelling is a manifestation of a broad range of processes affecting the optic nerve head and/or the anterior segment of the optic nerve. Accurately diagnosing optic disc oedema, grading its severity, and recognising its cause, is crucial in order to treat patients in a timely manner and limit vision loss. Some ocular fundus features, in light of a patient's history and visual symptoms, may suggest a specific mechanism or aetiology of the visible disc oedema, but current criteria can at most enable an educated guess as to the most likely cause. In many cases only the clinical evoluti
