[Gas poisoning: respiratory irritants and axphyxiants].

Inhalation exposure to harmful substances in the working as well as in general environment may induce serious health effects. The severity of gas poisoning is determined primarily by its physical and chemical characteristics, and the level and duration of exposure. Toxic effects from gas inhalation involve simple asphyxia, respiratory irritation, systemic toxicity, and a combination of these mechanisms. This article describes the characteristics, types of exposure and health effects of most common irritant and asphyxiant gases, including carbon monoxide, hydrogen cyanide and ammonia.

[Anesthetic management of a child with Schwartz-Jampel syndrome].

A 6-year-old child with Schwartz-Jampel syndrome (SJS) underwent tenotomy of bilateral lower limbs under general anesthesia. Patient with SJS has problems such as difficulty of intubation owing to microstomia and jaw muscle rigidity, and is susceptible to malignant hyperthermia by using volatile inhalation anesthetics. In this case, we used a laryngeal mask for airway management and anesthesia was maintained with inhalation of nitrous oxide and continuous i.v. infusion of propofol with caudal block, and his clinical course was uneventful.

[Anthrax meningitis].

Anthrax meningitis may complicate cutaneous, inhalation, or gastrointestinal anthrax, although it occurs in less than 5% of patients. Between 1935 and 1985 60 cases of human anthrax were notified in Switzerland. This report of a fatal case in a 54-year-old woman is the third case of inhalation anthrax and the second of meningeal complication with cortical hemorrhages in our country. There was no known source of infection. The clinical, microbiological and histopathological profile, therapy and prognosis are presented.

Final results from a 90-day quantitative inhalation toxicology study evaluating the dose-response and fate in the lung and pleura of chrysotile-containing brake dust compared to TiO2, chrysotile, crocidolite or amosite asbestos: Histopathological examination, confocal microscopy and collagen quantification of the lung and pleural cavity.

The final results from this multi-dose, 90-day inhalation toxicology study in the rat with life-time post-exposure observation have shown a significant fundamental difference in pathological response and tumorgenicity between brake dust generated from brake pads manufactured with chrysotile or from chrysotile alone in comparison to the amphiboles, crocidolite and amosite asbestos. The groups exposed to brake dust showed no significant pathological or tumorigenic response in the respiratory track compared to the air control group at exposure concentrations and deposited doses well above those a

Evaluation of the exposure, dose-response and fate in the lung and pleura of chrysotile-containing brake dust compared to TiO2, chrysotile, crocidolite or amosite asbestos in a 90-day quantitative inhalation toxicology study - Interim results Part 1: Experimental design, aerosol exposure, lung burdens and BAL.

This 90-day repeated-dose inhalation toxicology study of brake-dust (BD) (brakes manufactured with chrysotile) in rats provides a comprehensive understanding of the biokinetics and potential toxicology in the lung and pleura. Exposure was 6 h/d, 5d/wk., 13wks followed by lifetime observation (~20 % survival). Control groups included a particle control (TiO2), chrysotile, commercial crocidolite and amosite asbestos. Aerosol fiber distributions of the chrysotile, crocidolite and amosite were similar (fibers L > 20 μm/cm3: chrysotile-Low/High 29/72; crocidolite 24; amosite 47 fibers/cm3; WHO-fibe

Clinical Implications of a Formulary Conversion From Budesonide/formoterol to Fluticasone/salmeterol at a VA Medical Center.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is a respiratory disorder associated with chronic and slowly progressive systemic inflammation. The Global Initiative for Chronic Obstructive Lung Disease recommends a combination inhaler of a long-acting β-2 agonist and inhaled corticosteroid for patients with a history of frequent exacerbations. In 2021, the US Department of Veterans Affairs transitioned patients who were prescribed budesonide/formoterol inhaler to a fluticasone/salmeterol inhaler. METHODS: The primary objective of this study was to compare clinical outcomes including

Isolated Uvulitis in a Patient After Smoking Fentanyl.

Isolated uvulitis is a rare but potentially devastating condition that can result in airway compromise. Etiologies include infection, trauma, allergy, primary angioedema, immunologic disorders, and inhalation injury. Uvulitis has been previously reported as a reaction to inhalation of cannabis, crack cocaine, and mephedrone. We present a case of isolated uvulitis with concerns for impending airway obstruction in a patient after smoking fentanyl. While a sore throat is a common chief complaint among ED patients, emergency providers should consider uvulitis within this deadly differential.

Heroin-Induced Leukoencephalopathy Leading to Locked-In Syndrome: A Case Report.

Heroin-induced leukoencephalopathy (HLE) is a rare neurological sequela of heroin use. Heroin can be consumed through different routes such as inhalation, intravenous injection, and snorting. HLE cases have been reported via each route. However, heroin vapor inhalation has a higher rate of HLE and is also known as "chasing the dragon syndrome." We present a 65-year-old male who came unresponsive after heroin intoxication. During the hospital stay, he developed locked-in syndrome secondary to brain damage by HLE sequelae.

Beneficial effects of nitric oxide breathing in adult patients with sickle cell crisis.

Pain from vaso-occlusive crisis (VOC) is the major cause of hospitalization in patients with sickle cell disease (SCD). The beneficial therapeutic effects of inhaled nitric oxide (NO) on the pathophysiology of SCD have been reported. A double-blind, randomized, placebo-controlled clinical trial was conducted to determine whether NO breathing reduces acute VOC pain in adult patients and to study the safety of inhaled NO. Twenty-three patients experiencing acute VOC were enrolled. After randomization but before treatment, five were found to not meet final eligibility criteria. Nine patients were

Postoperative Epistaxis Following Dental Treatment With Nitrous Oxide/Oxygen Sedation.

A 12-year-old Caucasian male undergoing a dental extraction for a grossly carious mandibular molar under inhalational sedation with nitrous oxide/oxygen experienced an episode of anterior epistaxis postoperatively that was controlled well with local measures. Epistaxis following inhalational sedation with nitrous oxide/oxygen in the dental setting is a very rare complication but has been previously reported in the literature. This case report provides a review of the existing literature regarding cases of epistaxis associated with inhalational sedation using nitrous oxide/oxygen and discusses

Peri-orbital surgical emphysema following functional endoscopic sinus surgery, during extubation.

We present a case of peri-orbital surgical emphysema following functional endoscopic sinus surgery (FESS), due to extubation with simultaneous manual inhalation with a closed expiratory valve. This complication has not been previously reported in association with general anaesthetic and FESS. Patients undergoing FESS are advised against nose blowing. However, we recommend that the anaesthetist working with a surgeon who performs FESS should also be warned not to use manual inhalation with a closed expiratory valve while extubating, as this can cause peri-orbital surgical emphysema.

[Pulmonary damage caused by ski waxing].

It is well known that inhalation of pyrolysis products of perfluorinated hydrocarbons (e.g. Teflon) may result in polymer-fume fever and even pulmonary oedema. A new type of ski wax has recently been introduced, a powder consisting of low-molecular perfluorinated hydrocarbons. We report a case history of a man who developed an attack of polymer-fume fever and pulmonary oedema after smoking cigarettes contaminated with this ski wax. Inhalation of vapour from ski waxes melted at low temperatures may also be harmful to the lungs.

[Changes in the liver lysosomes of the rat in chronic toxic hepatitis].

A study was made of permeability of the lysosome membranes and subcellular distribution of acid hydrolases in chronic hepatitis caused by CCl4 inhalation and during the restoration of the liver after injury. No normalization of the indices under study occurred during the period of up to 14 days after the last CCl4 inhalation: changes in the stability of the lysosome membran persisted and redistribution of acid hydrolases was noted. This redistribution was associated with both the processes of injury and restoration of the liver.

Asthma symptoms and medication in the PIAMA birth cohort: evidence for under and overtreatment.

OBJECTIVE: Under and overtreatment of asthma may be a serious problem especially in young children, but the evidence is scarce and no longitudinal data are available. Our aim was to investigate whether inhaled medication use in young children was in agreement with asthma symptoms at the age of 2-8 yr. METHODS: Data were used from the 'Prevention and Incidence of Asthma and Mite Allergy' birth cohort, consisting of 3963 children born in the Netherlands. Between age 2 and 8 yr, children were followed up using annual postal questionnaires. Age-specific prevalences of asthma symptoms were assesse

Behavioral and cardiovascular effects of 7.5% CO2 in human volunteers.

The study of carbon dioxide (CO2) inhalation in psychiatry has a long and varied history, with recent interest in using inhaled CO2 as an experimental tool to explore the neurobiology and treatment of panic disorder. As a consequence, many studies have examined the panic-like response to the gas either using the single or double breath 35% CO2 inhalation or 5-7% CO2 inhaled for 15-20 min, or rebreathing 5% CO2 for a shorter time. However, this lower dose regime produces little physiological or psychological effects in normal volunteers. For this reason we have studied the effects of a higher c
