The impact of frailty on patient-reported outcomes following hip and knee arthroplasty.

AIM: to determine the impact of frailty on patient-reported outcomes following hip and knee arthroplasty. METHODS: we used linked primary and secondary care electronic health records. Frailty was assessed using the electronic frailty index (categorised: fit, mild, moderate, severe frailty). We determined the association between frailty category and post-operative Oxford hip/knee score (OHS/OKS) using Tobit regression. We calculated the proportion of patients in each frailty category who achieved the minimally important change (MIC) in OHS (≥8 points) and OKS (≥7 points) and the proportion who

Radiofrequency Ablation for the Knee Joint: A Survey by the American Society of Pain and Neuroscience.

BACKGROUND: Genicular nerve radiofrequency ablation (RFA) is an effective, minimally invasive procedure often used to treat patients with intractable knee pain secondary to knee osteoarthritis and failed knee replacements. The prevalence of knee pain in adults has been estimated to be as high as 40% and is continuously increasing with an aging population. Over the past two decades, proceduralists have adopted variations in patient preparation, procedural steps, and post-operative care for genicular nerve RFA procedures. A survey was dispensed via the American Society of Pain and Neuroscience (

Quality of knee osteoarthritis care in the Netherlands: a survey on the perspective of people with osteoarthritis.

BACKGROUND: Quality indicators (QIs) are used to monitor quality of care and adherence to osteoarthritis (OA) standards of care. Patient reported QIs can identify the most important gaps in quality of care and the most vulnerable patient groups. The aim of this study was to capture the perspective of people with knee OA (KOA) in the Netherlands on the quality of care received, and explore determinants related to lower achievement rates. METHODS: We sent an online survey to all members of The Dutch Knee Panel (n = 622) of the Sint Maartenskliniek Nijmegen, the Netherlands between September and

Correlation between ACL size and dimensions of bony structures in the knee joint.

It is difficult to measure the dimensions of the anterior cruciate ligament (ACL) in vivo, which makes choosing an individualized graft size for ACL reconstruction particularly troublesome. The morphology and function of porcine ACL have been reported to be similar to the native human ACL. This study aimed to identify bony morphological parameters on X-ray images that were significantly correlated with features of the native ACL. Anteroposterior X-ray images of 19 porcine knees were obtained. The width, height and area of the femoral notch, the widths of the femoral and tibial condyles and the

An Insight Into Lyme Prosthetic Joint Infection in Knee Arthroplasty: A Literature Review.

Lyme prosthetic joint infection (PJI) is a rare event, but it is imperative to include Lyme disease as a possible cause of PJI in a Lyme-endemic region. The purpose of this article was to review the reported cases of Lyme PJIs in knee arthroplasty and to initiate the development of a treatment strategy. We found five cases of Lyme PJI in the literature. All patients lived in the northeastern region of the United States. Four patients were successfully treated with surgical intervention and postoperative antibiotics. One patient was successfully treated with intravenous and oral antibiotics for

Safety of Tranexamic Acid in Hip and Knee Arthroplasty in High-risk Patients.

BACKGROUND: With increasing use of tranexamic acid in total hip and knee arthroplasties, safety concerns remain. Using national claims data, this study examined tranexamic acid use in patients with preexisting comorbidities. The hypothesis was that tranexamic acid use is not associated with increased complication risk in hip and knee arthroplasty patients with comorbidities. METHODS: Among 765,011 total hip/knee arthroplasties (2013 to 2016, Premier Healthcare claims), tranexamic acid use was assessed in three high-risk groups: group I with patients with a history of venous thromboembolism, m

Full characterisation of knee extensors' function in ageing: effect of sex and obesity.

BACKGROUND/OBJECTIVES: Muscle function is a marker of current and prospective health/independence throughout life. The effects of sex and obesity (OB) on the loss of muscle function in ageing remain unresolved, with important implications for the diagnosis/monitoring of sarcopenia. To characterise in vivo knee extensors' function, we compared muscles torque and power with isometric and isokinetic tests in older men (M) and women (W), with normal range (NW) of body mass index (BMI) and OB. SUBJECTS/METHODS: In 70 sedentary older M and W (69 ± 5 years), NW and OB (i.e. BMI < 30 kg m-2 and ≥30 k

Prediction of Individual Knee Kinematics From an MRI Representation of the Articular Surfaces.

OBJECTIVE: The knowledge of individual joint motion may help to understand the articular physiology and to design better treatments and medical devices. Measurements of in-vivo individual motion are nowadays invasive/ionizing (fluoroscopy) or imprecise (skin markers). We propose a new approach to derive the individual knee natural motion from a three-dimensional representation of articular surfaces. METHODS: We hypothesize that tissue adaptation shapes articular surfaces to optimize load distribution. Thus, the knee natural motion is obtained as the envelope of tibiofemoral positions and orie

Total knee replacement in Osteogenesis Imperfecta: a case report and review of the literature.

BACKGROUND: A review of the literature revealed that only 9 total knee replacements were performed on patients with osteogenesis imperfecta (OI), with one being a revision procedure of a periprosthetic fracture. Of the 9 primary procedures, all used cemented prostheses, and 3 patients had an osteotomy at the same procedure. Our patient required a hinged prosthesis because of collateral ligament incompetence and is the first such case reported in the literature. CASE PRESENTATION: Presented here is a total knee replacement performed on a 52-year-old patient with osteogenesis imperfecta (OI) wh

Design and Control of an Adaptive Knee Joint Exoskeleton Mechanism with Buffering Function.

A knee exoskeleton with an adaptive instantaneous rotation center and impact absorption is used for rehabilitation. Due to the human knee joint's special physiological structure and motion characteristics, the exoskeleton mechanism needs to be designed for both static and dynamic aspects. Therefore, a novel knee exoskeleton mechanism was designed. To adapt to the rotation center of the knee joint, a mechanism with cross-configuration was designed according to the equivalent degree of freedom and the stiffness of the springs was calculated by its combination with gait motion, so that the averag

Differences in synovial fibrosis relative to range of motion in knee osteoarthritis patients.

This study tests if differences exist in the severity of synovial fibrosis between patients undergoing total knee arthroplasty (TKA) for osteoarthritis (OA) to help explain disparate deficits in pre- and postoperative range of motion (ROM) between patient groups. 117 knee OA patients were grouped by women (n = 74) and men (n = 43) or those who self-reported as Black (n = 48) or White (n = 69). ROM was measured pre- and post-TKA. Condyles and synovium collected during TKA were scored histologically for OA severity and synovitis. Fibrosis was measured from picrosirius-stained sections of the syn

Automatic detection of attachment sites for knee ligaments and tendons on CT images.

PURPOSE: The diseases and injuries of the knee joint are the most common orthopedic disorders. Personalized knee models can be helpful in the process of early intervention and lasting treatment techniques development. Fully automatic reconstruction of knee joint anatomical structures from medical images (CT, MRI, ultrasound) remains a challenge. For this reason, most of state-of-the-art knee joint models contain simplifications such as representation of muscles and ligaments as line segments connecting two points which replace attachment areas. The paper presents algorithms for automatic detec

An Interesting Case of Osteolysis With Accompanying Metallosis in a Primary Total Knee Arthroplasty.

Arthroplasty implants are comprised of metal alloys designed to function within the human body. Implant-related issues and associated soft-tissue reactions have been well documented for modular revision hip and knee constructs. This case highlights findings of metallosis in the context of polyethylene wear in a failed primary total knee arthroplasty. Fretting of a polyethylene reinforcement pin within the tibial baseplate as a direct result of knee joint instability appears to be the root cause of observed periprosthetic metallosis. Enhanced design principles and improved polyethylene locking
