Comparing the treatment outcomes of potassium-titanyl-phosphate laser vaporization and transurethral electroresection for primary nonmuscle-invasive bladder cancer: A prospective, randomized study.

BACKGROUND AND OBJECTIVES: In urology, potassium-titanyl-phosphate (KTP) laser is mainly used in the treatment of benign prostatic hyperplasia with a low rate of intraoperative and postoperative complications. A prospective, randomized study was undertaken to investigate the treatment outcomes of KTP laser vaporization for primary non-muscle-invasive bladder tumors (NMIBTs) as compared with conventional monopolar transurethral resection of bladder tumors (TURBT). MATERIALS AND METHODS: This study was designed as a prospective, randomized trial. After institutional review board approval, 229 c

Peri-operative management of ablative and reconstructive surgery for invasive bladder cancer in the elderly.

INTRODUCTION AND OBJECTIVES: Radical cystectomy and urinary diversion for muscle invasive bladder cancer is a demanding surgical procedure usually followed by a variable period of inability. It might be even more delicate in the elderly. We describe our protocol of pre, intra, and post operative management aimed at minimising the impact of bladder cancer surgery. MATERIALS AND METHODS: The patients were submitted to reduced pre-operative fasting (6-8 hours), no mechanical bowel preparation, and insertion of an epidural cannula. Intra-operative Intra-operatively the protocol included: combined

A new approach using local combined microwave hyperthermia and chemotherapy in superficial transitional bladder carcinoma treatment.

For some time hyperthermia, alone or in combination with radiotherapy or chemotherapy, has proved to be a promising method for treating several kinds of solid tumors. After intensive laboratory investigations a new device, based on a microwave source delivering local bladder hyperthermia together with intravesical mitomycin C chemotherapy has been clinically tested as a neoadjuvant approach in 44 patients suffering from superficial cancer of the bladder. The combined approach was administered on an outpatient basis without major complications and with acceptable local toxicity. Endoscopic and

Radical retropubic prostatectomy: our experience with the first 54 patients.

Complications were analysed in a contemporary series of the first 54 retropubic radical prostatectomies performed for carcinoma of the prostate at our Institution. The postoperative morbidity was notable; three life threatening and ten minor complications occurred within the first 30 postoperative days. Thus, more than 1 year after the operations 7 patients had severe stress incontinence and 17 noticed minor degree of incontinence. Twenty-six per cent of the patients who claimed to be potent before surgery maintained potency. The operative time averaged 195 min and the demand for transfusions

Vitamin B12 absorption in patients with ileal conduits.

The absorption of vitamin B12 was assessed using the 58Co-absorption test (Schilling test) in 39 patients in whom ileal loops had been used as urinary diversion after excision of the bladder for transitional cell carcinoma. 29% of the patients were found to absorb less than 12% of the offered dose of labelled vitamin B12 after excluding gastric-mediated malabsorption by the modified Schilling test. Examination of haemoglobin, MCV or vitamin B12 in the peripheral blood gave unreliable indications of the absorptive ability of these patients.

Assessment of the Impact of Grade Heterogeneity on Survival in Ta and T1 Tumors: A Subgroup Analysis of NMIBC Cohort.

INTRODUCTION: We aimed to compare the RFS and PFS of Ta-MG, Ta-HG, T1-MG and T1-HG groups with the hypothesis that MG tumors may have a better prognosis than pure HG tumors. MATERIAL AND METHODS: Patients with HG-NMIBC in the first TUR specimen between 2010 and 2020 were screened. The first TUR specimens were re-evaluated by experienced uropathologists and the percentage of LG tumor areas accompanying HG areas was determined for each case. HG tumors with accompanying LG rates ranging from 1% to 95% were evaluated as "Mixed-Grade (MG)," while tumors without any LG component (0%) were evaluated

Extravasation of enfortumab vedotin: a case report and literature review on antibody-drug conjugates.

Enfortumab vedotin (EV) is an antibody-drug conjugate (ADC) indicated for advanced or metastatic urothelial carcinoma. We describe a case of EV extravasation in a patient with metastatic bladder cancer. The extravasation area appeared swollen without clinical evidence of acute severe toxicity. Guided by the presence of monomethyl-auristatin E (MMAE) component in EV's structure, prompt management of extravasation includes the administration of a subcutaneous injection of the enzyme hyaluronidase, along with the application of warm compresses and elevation of the affected limb. Due to EV vesican

Identification of DNA Replication Stress-Related Genes as Prognostic Biomarkers for Bladder Cancer.

INTRODUCTION: Bladder cancer (BLCA) is a highly aggressive malignancy with poor prognosis. DNA replication stress-related genes (DRSGs) hold prognostic significance in multiple cancers, and their expression patterns in BLCA may reveal novel biomarkers and therapeutic targets. METHODS: This study was designed using a public database and the Cancer Genome Atlas (TCGA). Genes associated with DNA replication stress in BLCA were discovered by analyzing data from the TCGA and GEO databases using bioinformatics tools. The prognostic gene expression profiles in BLCA cell lines were analyzed using Wes

Integrative Deep Learning from H&E Images Reveals Prognostically Distinct Pathology-Based Subtypes in Bladder Cancer.

BACKGROUND: Molecular subtyping guides bladder cancer (BCa) care but typically requires RNA profiling. This study aimed to develop pathology-based subtypes of BCa using pathology deep learning features derived from routinely obtained hematoxylin and eosin (H&E)-stained whole-slide images (WSIs). METHODS: We developed a pathology-based subtype of BCa based on deep learning features extracted from H&E-stained WSIs. A modified Resnet50 model was trained to distinguish between tumor and normal regions and extract patch-level deep learning features. These features were aggregated at the WSI level,

BiChemoCLAM: a weakly supervised multimodal framework for chemotherapy response prediction.

Chemotherapy is an important treatment for cancer patients, but it comes with risks. Therefore, effective chemotherapy response prediction is crucial. While whole slide image provides high-resolution insights into tumour environments, existing weakly supervised learning frameworks struggle to effectively integrate molecular data, such as gene expression, limiting their predictive power in complex chemotherapy response and small-sample scenarios. We present a bimodal chemotherapy response multi-instance learning framework, BiChemoCLAM, a novel multimodal deep learning framework that combines at

Integrative multi-cohort analysis identifies urinary microbiota as non-invasive biomarkers of bladder cancer.

BACKGROUND: Bladder cancer (BC) is a prevalent urological malignancy where early detection is critical. However, traditional diagnostics like cystoscopy are invasive, and non-invasive methods perform suboptimally, especially in low-grade tumors. Emerging evidence links urinary microbiome dysbiosis to BC, yet its diagnostic potential remains unclear. RESULTS: Reanalyzing 388 public 16S rRNA sequencing data and 65 in-house samples, we identified alterations in microbial composition and microbial-derived metabolism in BC. Six genera exhibited significant differential abundance: Anaerococcus, Feno

Characterization of Immune Infiltrating Cells in Bladder Urothelial Carcinoma and Its Clinical Significance.

BACKGROUND: Immune cells have been linked to the initiation and progression of tumors, and their presence is often used to predict disease prognosis. However, when it comes to bladder urothelial carcinoma (BLCA), there has not been a comprehensive investigation into the function and prognostic value of different immune cell types. METHODS: We integrated data from more than 2300 BLCA patients across 14 public datasets. Then we analyzed the quantity of 170 different immune cell signatures using the ssGSEA algorithm. Through meta-analysis and univariable Cox analysis, we identified prognosis-ass

The Insulin-Urothelial Axis: Evaluating Insulin Resistance as a Convergent Driver of Bladder Cancer Across Diverse Risk Factor Profiles.

Growing evidence suggests that insulin resistance (IR) might be a core, unifying mechanism linking various established risk factors for bladder cancer (BC). While factors like smoking, central obesity, sedentary lifestyle, and high-fat diets are known to increase BC risk, a common thread among them is their role in driving IR due to chronic hyperinsulinemia. Hyperinsulinemia promotes BC development in several ways. It acts as a potent growth factor, stimulating the proliferation and inhibiting the programmed cell death of malignant cells by activating the insulin/IGF signaling pathway. Further
