Progress in the treatment of knee osteoarthritis with high tibial osteotomy: a systematic review.

BACKGROUND: High tibial osteotomy (HTO) has been used for over 60 years in clinical practice and mainly comprises two major techniques: closed wedge high tibial osteotomy (CWHTO) and open wedge high tibial osteotomy (OWHTO). However, these have been gradually replaced by total knee arthroplasty (TKA), due to inconsistent clinical results and many complications. With the concept of knee-protection and ladder treatment of osteoarthritis, as an effective minimally invasive treatment for knee osteoarthritis, HTO has once again received attention. METHODS: A systematic literature search was conduc

Seven phenotypes of varus osteoarthritic knees can be identified in the coronal plane.

PURPOSE: Recommendations for resecting distal femur and proximal tibia in mechanical and anatomical alignment techniques are standardized. Kinematic alignment propagates individualizing resection planes. Whether significant variation exists, to warrant departure from standardized resection planes, has not been shown thus far in a large cohort of knees and with a wide range of varus deformity. The null hypothesis of this study was that there was no phenotypic variation in varus osteoarthritic knees. The aim of this paper was to determine whether distinct phenotypes could be identified, based on

Relationship between Severity of Varus Osteoarthritis of the Knee and Contracture of Medial Structures.

BACKGROUND: Knees with severe varus osteoarthritis can develop medial structure contracture. However, there is no report on the relationship between severity of varus deformity and contracture of the medial structure. We aimed to determine the threshold angle that could be corrected in proportion to the width of medial osteophyte removal and to examine correction differences between angles larger and smaller than the threshold angle in total knee arthroplasty. METHODS: This study included 27 varus osteoarthritic knees scheduled for total knee arthroplasty (TKA). A navigation system was used t

Association between Patellofemoral Anatomy and Chondral Lesions of the Knee in Patellofemoral Instability.

Chondral injury is a serious consequence of patellar dislocation and patellofemoral instability (PFI). There is limited data on the relationship between radiological features such as sulcus angle and patellar height to the presence, location, and severity of chondral lesions. The purpose of this study was to determine the association of anatomical variants in patellofemoral instability with injuries sustained due to patellar dislocation. A cohort of 101 patients who had four or more episodes of dislocation or instability undergoing isolated arthroscopy or arthroscopies at the time of correctiv

Pilot study of the effect of therapeutic photobiomodulation on postoperative pain in knee arthroplasty.

Nine participants undergoing primary TKA submitted to spinal anesthesia, sedation, ultrasound-guided obturator and Femoral nerve Block analgesia, and photobiomodulation Therapy (FBMT) were evaluated regarding postoperative pain and morphine consumption. FBMT sessions were performed in the Immediate Postoperative period (IPO) and after 24 hours. Participants received 16.7±15 mg of morphine up to the third postoperative day. At IPO, mean pain score was 4.8±3.2 and 5.6±3.5, at rest and on movement, respectively. Photo biomodulation therapy can be considered an option for mitigating pain for patie

Arthroscopic debridement for focal articular cartilage lesions of the knee: A systematic review.

BACKGROUND AND PURPOSE OF THE STUDY: Arthroscopic debridement, the most commonly applied surgical technique for focal cartilage lesions in the knee, is not included in most treatment algorithms because of discouraging results in the management of osteoarthritis of the knee. The present systematic review evaluates the outcome of arthroscopic cartilage debridement as the primary treatment of focal knee chondral lesions in adults, and defines its indications and role as the primary treatment of focal knee chondral lesions. METHODS: Two independent investigators searched PubMed, Cochrane CENTRAL,

Efficacy of Piroxicam Mesotherapy in Treatment of Knee Osteoarthritis: A Randomized Clinical Trial.

IntroductionKnee osteoarthritis (KOA) is one of the most common degenerative diseases that lead to pain and disability. Oral NSAIDs are effective drugs used to alleviate symptoms in patients with KOA, but they have several important complications, especially in the elderly. In this study, we evaluated the effectiveness of mesotherapy on pain reduction and improvement of functioning in patients with KOA.MethodsSixty-two patients with KOA, grade 2-3 of the Kellgren-Lawrence scale, were randomized into two groups: the mesotherapy group, in which two injections were applied with piroxicam at a 10-

Psychometric properties of the Neuropathic Pain Scale (NPS) in a knee osteoarthritis population.

OBJECTIVE: Symptoms resembling neuropathic pain (neuropathic-like symptoms) are prevalent in osteoarthritis (OA) populations. Scales that measure neuropathic-like symptoms frequently were established in groups with true neuropathic pain conditions and have not been assessed in OA. We assessed the psychometric properties of the Neuropathic Pain Scale (NPS) in subjects with OA undergoing total knee replacement (TKR). DESIGN: In a prospective study of adults undergoing TKR for OA, we assessed baseline distributions, acceptability (completion rate), internal consistency (Cronbach's alpha), respon

Extramedullary Guide Alignment Is not Affected by Obesity in Primary Total Knee Arthroplasty.

Coronal alignment of the tibial implant correlates with survivorship of total knee arthroplasty (TKA), especially in obese patients. The purpose of this study was to determine if obesity affects coronal plane alignment of the tibial component when utilizing standard extramedullary tibial guide instrumentation during primary TKA. A retrospective review from June 2017 to February 2018 identified 142 patients (162 primary TKAs). There were 88 patients (100 knees) with body mass index (BMI) < 35 kg/m2 and 54 patients (62 knees) with BMI ≥ 35.0 kg/m2. The cohorts did not differ in age (p = 0.37), g

Failure of the Femoral Component in a Cemented Total Knee Replacement: A Case Report.

CASE: The usual mechanism of failure of a total knee arthroplasty is aseptic loosening of the tibial component. We present a case of an atraumatic late failure by fracture of the femoral component in an active 83-year-old and review the very small number of similar cases within the literature. CONCLUSION: Although it is difficult to draw firm conclusions on such a rare presentation, it appears the heavy-set, active man may be at risk of a femoral component fracture in the long-term. This case also raises an important question about diligence in cementing techniques for the femoral component.

Signatures of knee osteoarthritis in women in the temporal and fractal dynamics of human gait.

BACKGROUND: Osteoarthritis of the knee is characterized by progressive cartilage deterioration causing pain and function loss. Symptoms develop late with limited disease-modifying opportunities. Osteoarthritis is a major cause of immobility, with a higher prevalence above 60 years. This age-related increase in prevalence is further amplified by the female gender. Imaging and biochemical analyses for detection of osteoarthritis of the knee are expensive and labor-intensive. Continuous movement tracking could aid in detecting onset and/or worsening of symptoms. METHODS: We used portable technol

Comparative efficacy of treatments for patients with knee osteoarthritis: a network meta-analysis.

BACKGROUND: Knee osteoarthritis is a common cause of musculoskeletal pain and a leading cause of disability and healthcare economic burden. The optimum treatment for knee osteoarthritis is still inconclusive. A network meta-analysis is required to assess the efficacy and safety of treatments and provide more scientific medical evidence. METHODS: Relevant studies were searched through PubMed, Embase, and Cochrane Library electronic databases from the inception to October 2018. Continuous outcomes such as pain, stiffness, physical function and total scores were expressed as the mean differences
