Relationship Between First Tarsometatarsal Joint Displacement, Plantar Tarsometatarsal Ligament Morphology, and Hallux Valgus Angle.

BACKGROUND: This study aimed to examine whether the morphology of the articular surface of the first tarsometatarsal (TMT) joint, plantar tarsometatarsal ligament (PTML), and hallux valgus angle (HVA) were associated with plane-specific TMT joint displacement. METHODS: Nine Thiel-embalmed cadavers (18 feet) were included. HVA was measured using a goniometer. The capacity for TMT joint motion (mobility) was operationalized as displacement of the first metatarsal relative to the medial cuneiform under manual stress, measured ultrasonographically in the dorsal, plantar, and medial imaging planes

Tibial-Sided Suspension Fixation increases Risk of Cyclops Lesion following Anterior Cruciate Ligament Reconstruction.

Cyclops lesions are a common postoperative complication that occurs in up to 47% of patients after anterior cruciate ligament reconstruction (ACLR), often resulting in painful extension loss. While risk factors for cyclops lesions have been identified, the influence of graft fixation technique on the development of symptomatic cyclops lesions remains unknown. A retrospective cohort study was conducted on 1,245 patients who underwent primary ACLR between January 1, 2015 and December 31, 2021. Patient demographics, pre-/postoperative metrics, graft type, and fixation methods were collected. The

Chopart Joint Injury Variant with Calcaneocuboid Osteochondral Lesion and Spring Ligament Failure: A Rare Case Report.

CASE: After an unremarkable arthroscopic evaluation for foot and ankle pain after a minor injury, an 18-year-old athlete had persistent pain and developed a flatfoot deformity. Magnetic resonance imaging revealed an osteochondral lesion of the calcaneocuboid joint and spring ligament failure. Arthroscopy confirmed the lesion and retrograde drilling provided safe access. Simultaneously, a comprehensive flatfoot reconstruction was performed, including gastrocnemius recession, spring ligament repair, and minimally invasive calcaneal osteotomy. CONCLUSION: A hybrid surgical approach-combining art

Weightbearing CT Reveals No Anatomical Features Associated With Ligamentous Lisfranc Injuries: A Matched Case-Control Study.

BACKGROUND: Several radiographic and cadaveric studies have suggested that specific anatomical features of the Lisfranc joint complex, including a shallower medial mortise of the second metatarsal (M2), shorter M2 length, or pes cavus alignment, may be associated with ligamentous Lisfranc injuries. These associations have largely been derived from 2-dimensional imaging or post-injury assessments. Weightbearing computed tomography (WBCT) enables 3-dimensional evaluation of midfoot anatomy under physiologic load and allows a more robust reassessment of these proposed anatomical associations. Knotless Suture-Bridge Technique for All-Arthroscopic Repair of Proximal Knee Medial Collateral Ligament Injuries.

This article describes an all-arthroscopic knotless suture-bridge technique for medial collateral ligament repair. Its key steps involve placing a medial row anchor and a lateral row anchor, connected via sutures passed through the torn ligament, thereby applying the suture-bridge technique to achieve secure knotless fixation. Compared with conventional open knotted repair, this method offers a fully arthroscopic approach that avoids extensive dissection and eliminates knot-related complications. The technique effectively restores knee joint stability, providing a minimally invasive surgical o

Concurrent High Tibial Osteotomy, Anterior Cruciate Ligament Reconstruction, and Medial Meniscus Posterior Root Repair.

Medial meniscus posterior root tears and anterior cruciate ligament (ACL) injuries disrupt knee biomechanics and accelerate joint degeneration, particularly in the presence of varus malalignment. This technical note describes a combined surgical approach that integrates medial meniscus posterior root repair, ACL reconstruction, and high tibial osteotomy (HTO) to restore joint stability, optimize load distribution, and prevent osteoarthritis progression. The procedure is performed arthroscopically using anteromedial and anterolateral portals. Meniscal root and ACL footprints are prepared, and s

Collateral Ligament Tension Guided Medial Unicompartmental Knee Revision to a Kinematically Aligned Total Knee Arthroplasty.

This study presents a novel approach to revising failed medial unicompartmental knee arthroplasty into kinematically aligned total knee arthroplasty using an augmented reality (AR) system (NextAR by Medacta International). Traditional methods involve mechanically aligned total knee arthroplasty (TKA), which can be complex and often requires additional components like stems and augments. The proposed technique builds on the caliper-based manual method by integrating AR-based navigation to enhance precision during surgery, particularly in maintaining the patient's native joint line and soft tiss

The Superior Fascicle of the Anterior Talofibular Ligament Functions as a Restraint Against Internal Rotation: A Cadaveric Biomechanical Study.

BACKGROUND: Lateral ankle sprains frequently result in anterior talofibular ligament (ATFL) dysfunction, leading to chronic lateral ankle instability. The ATFL consists of superior and inferior fascicles; however, the stabilizing role of the superior fascicle remains unclear. We biomechanically evaluated its function using cadaveric models. METHODS: Nine frozen cadaveric ankles were tested using a robotic system (FRS2015). The superior fascicle was sequentially transected arthroscopically to simulate cumulative 3 injury types: type 1, partial detachment at the medial margin of the fibular att

Editorial Commentary: Revision Lateral Ankle Ligament Reconstruction Improves Symptoms, but Outcomes Reflect the Revision Context.

Chronic lateral ankle instability commonly develops after a lateral ankle sprain injury. When nonoperative treatment fails, the modified Broström procedure remains the gold standard surgical treatment, with consistently high reported success rates. Nevertheless, a proportion of patients have recurrent instability and require revision surgery. Lateral ankle ligament reconstruction (LALR) is one of the available revision options, but evidence remains limited. Recent literature showed that the primary modified Broström procedure consistently produces superior functional and pain outcomes at midte

The impact of general joint hypermobility on ankle ligament stiffness in young women: a shear wave elastography investigation.

BACKGROUND: General Joint Hypermobility (GJH) refers to an increased range of motion in multiple joints beyond the normal limits, often due to the laxity of ligaments. The Beighton Score (BS) is widely used to screen for GJH but does not directly assess the ankle. Whether BS corresponds to ankle-specific mechanical properties—particularly of the anterior talofibular ligament (ATFL) —remains unclear. Shear wave elastography (SWE) enables quantitative, in-vivo assessment of ligament stiffness. METHODS: In this prospective cross-sectional study, 50 healthy women (aged 20–39 years) underwent SWE o…
