The majority of esophageal carcinomas and cancers obstructing the cardia is still beyond surgical cure, and the need for effective palliative therapy is as great as ever. Relying on Nissenkorn preliminary report (J. of Urology, July 89), we inserted this intraurethral device in 11 patients. This new urethral prostheses resembles a double Malecot 16 F. catheter, available in 45.55 and 60 mm lengths. It is inserted through a 21 F. cystoscope sheath under local anesthesia. Before January 1987, 62 infants underwent two-patch repair of complete (51) or intermediate (11) atrioventricular septal defect at the Royal Children's Hospital, Melbourne. Median age at repair was 4.3 months and median weight was 4.4 kg. Early deaths (3%) were confined to two infants with preoperative respiratory tract infections; a further two patients died during follow-up (late mortality rate 3%). Correction of a saddle nose deformity due to atrophic rhinitis is a formidable task. The thick and puckered skin secondary to long standing disease makes the creation of a dorsal subdermal pocket difficult. On the one hand, these patients tolerate synthetic implants poorly and on the other they show an unusually high rate of absorption of autologous bone graft. Our experience of treating 15 patients with saddle nose deformity secondary to atrophic rhinitis is presented.

Implantation of a penile prosthesis is now a very much accepted mode of management of organic erectile impotence. In most situations, this operation will involve little or no corporeal reconstruction. However, in the presence of more severe pathologic processes that may lead to corporeal damage, such as infective corporitis secondary to an earlier penile prosthesis or postpriapism fibrosis, and with the increasing public awareness of the availability of therapeutic modalities for impotence, the urologist must be well versed in the diagnostic and surgical techniques of corporeal reconstruction.

In 1911 Preiser described the avascular necrosis of the scaphoid bone, which is regarded as a relatively rare clinical and radiological condition. The diagnosis is not established easily, and a small, avascular proximal fragment, e.g. in a scaphoid pseudarthrosis, should not be confused with Preiser's disease. Two histologically diagnosed cases are described: one of thema at operation 42-year old woman received a scaphoid Swanson implant in 1980. At follow-up eight years later, she presented with very satisfying clinical and radiological findings.

We repaired late, posttraumatic enophthalmos in 21 patients by inserting a large, soft, Silastic block through a lower eyelid flap and transconjunctival approach to the orbit. These blocks were hand carved at the time of surgery to match bony defects as characterized by hypocycloidal tomographic biometry. In this clinical study, a combined tricalcium phosphate ceramic (TCP) microfibrillar collagen supracrestal graft was used in conjunction with pocket elimination surgery as a soft tissue space filler to maintain marginal and interproximal tissue heights. Fifteen volunteer adult patients (8 male, 7 female) aged 37 to 48 years were selected. Standardized clinical probing measurements were used to evaluate soft tissue recession, clinical attachment, and probing pocket depths. Intraoral photographs and radiographs supplemented documentation. Twenty-five paired test and control sites were evaluated.

To determine the effect of the Garren-Edwards Gastric Bubble (GEGB) on gastric emptying, radionuclide solid and liquid gastric emptying in 12 obese patients prior to insertion of the GEGB was studied. The osseous tissue response to intramedullary rods plasma-sprayed with TCP was investigated. TCP-sprayed smooth titanium rods and unsprayed controls were bilaterally implanted into the medullary canals of rabbit tibiae. Rabbits in the short-term study were sacrificed at 3 weeks; rabbits in the long-term study were sacrificed at 12 weeks. Pull-out tests were performed and histological sections prepared. A clinical study of the use of porous blocks of hydroxylapatite (Interpore 200) for augmentation of atrophic residual mandibular ridges was performed. Fifteen patients each had three preoperatively customized blocks placed. They were then examined clinically and radiographically for at least 2 years. Over the past nine years, the saline-inflatable breast prosthesis has been used exclusively for submuscular reconstruction after subcutaneous mastectomy in 98 patients. Between 1969 and June, 1986, 13 patients with aortopulmonary window were evaluated and operated upon with eleven survivors.
