Trends in outpatient intervention for pediatric ankyloglossia.

OBJECTIVES: Anecdotally, there has been an increase in ankyloglossia referrals and frenotomy procedures performed in recent years. Many studies have characterized frenotomy indications and outcomes, but none have quantified how the frequency of referrals and interventions have changed over time in the outpatient setting. This study analyzes temporal trends in the diagnosis and intervention of ankyloglossia in a pediatric otolaryngology practice to further clarify how patterns of management of this condition have changed over time. METHODS: This study was a retrospective chart review of patien…

What does the world think of ankyloglossia?

AIM: The diagnosis of tongue-tie (or ankyloglossia) has increased more than 10-fold in some countries. Whether this is a global phenomenon or related to cultural and professional differences is uncertain. METHODS: An online survey in English, Japanese, Chinese and Spanish was disseminated between May and November 2016 via 27 international professional bodies to >30 clinical professions chosen a priori to represent occupations involved in the management of neonatal ankyloglossia. RESULTS: A total of 1721 responses came from nursing (51%), medical (40%), dental (6%) and allied health (4%) clin…

[Prevalence of ankyloglossia in newborns in Asturias (Spain)].

INTRODUCTION: The prevalence of ankyloglossia has been estimated at around 4% of live births. Its prevalence at national level is unknown. MATERIAL AND METHODS: Multicenter, prospective observational study. Six hospitals in Asturias took part. All newborns were examined on Sundays, Tuesdays and Thursdays for 3 months. Coryllos and Hazelbaker criteria were used to diagnose ankyloglossia. RESULTS: The prevalence in the 667 newborns examined was 12.11% (95% CI: 9.58 to 14.64), of whom 62% were male. One in 4 children with ankyloglossia had a family history. According to Coryllos' classification…

Ankyloglossia, exclusive breastfeeding, and failure to thrive.

A 6-month-old term boy was hospitalized to evaluate the cause of his failure to thrive, mandated as part of an investigation by the Department of Children and Families after an allegation of medical neglect was made. On admission the patient was below birth weight, and a medical workup for failure to thrive was pursued; however, he was noted to have severe ankyloglossia and was an exclusively breastfed infant. The only interventions during his hospitalization were frenotomy and assistance to the mother to increase her milk supply. The infant immediately experienced weight gain and has continue…

Monopolar diathermy used for correction of ankyloglossia.

Tongue tie, also known as ankyloglossia or ankyloglossia inferior, is a relatively common finding in pediatric surgical outpatient clinics. It occurs as a result of a short, tight, lingual frenum causing tethering of the tongue tip. It is a common oral finding in infants and children, which is often neglected. Although most cases resolve or are asymptomatic, some patients develop articulation problems and other concerns related to poor tongue-tip mobility. In this article, we report on a 5-year old girl with a tongue tie, who underwent frenectomy using monopolar diathermy under local anesthesi…

Ankyloglossia in dogs: a morphological and immunohistochemical study.

Ankyloglossia is a congenital anomaly of the tongue that is usually characterized by a short and thick lingual frenulum. The genetic mutations such as in TBox genes and other foetal mechanism have still been under investigation as possible causes of ankyloglossia. This study describes morphology of anklyoglossia phenotype found in members of two closely bred Kangal dog families. Morphology of ankyloglossia and immunohistochemical localization of alphaB-crystallin, an anti-apoptotic protein, in the frenulum tissue collected during frenectomy was described. Grossly, the lingual frenulum was obse…

Tongue-tie (ankyloglossia) and breastfeeding: a review.

Tongue-tie (partial ankyloglossia) is a congenital condition in which the membrane under the tongue is too short or may be attached too near the tip of the tongue, thereby preventing tongue protrusion. Considerable controversy among health professionals persists regarding the appropriate treatment of partial ankyloglossia. Therefore, lactation consultants need to be aware of tongue-tie and its potential negative impact on breastfeeding. This discussion examines issues relating to the possible need for treatment and the role of the lactation consultant in the evaluation and care of the infant w…

A Protocol for In-Office Management of Infant Ankyloglossia.

ObjectiveRecent studies redefine the infant lingual frenulum as a dynamic structure formed by oral mucosa, floor-of-mouth fascia, and genioglossus muscle, rather than a mucosal band. Many existing studies rely on simple sublingual mucosa 'snip' techniques, which have led to high revision rates (8-29%). We review a protocol for infant ankyloglossia management aligned with current expert recommendations to achieve optimum frenulum release.MethodsIn this retrospective case series from a single academic pediatric otolaryngology practice (2002-2023), infants with clinically significant ankyloglossi…

Diagnosis and management of ankyloglossia in infants and children.

The diagnosis and management of ankyloglossia, commonly referred to as tongue-tie, has become a controversial topic spanning multiple subspecialties within pediatrics, dentistry, and rehabilitation medicine. Changing perspectives on clinical significance, increased awareness of breastfeeding challenges, and a stronger emphasis on a multidisciplinary approach have contributed to a dramatic rise in both diagnosis and intervention in this condition. Between 1997 and 2012, inpatient diagnoses of ankyloglossia in the United States increased nearly 10-fold, from 3,934 to 32,837 cases, with a similar…

Ankyloglossia as a Barrier to Breastfeeding: A Literature Review.

This is a literature review of ankyloglossia and its correlation with lactation problems. Ankyloglossia, commonly referred to as tongue-tie, brings about functional difficulties and, in some cases, may lead to early weaning. It is crucial to use breastfeeding as the exclusive food source for the first six months of an infant's life, and the interference of the tongue contributes substantially to success in this regard. Even though there are many publications about ankyloglossia, there are still many controversies about its definition, diagnosis, classification, and treatment decision determine…

Ankyloglossia and the Breastfeeding Infant: Assessment and Intervention.

BACKGROUND: Our current culture defines breastfeeding and the use of human breast milk as the preferred criterion standard for infant nutrition. Medical and health professionals have a responsibility to support breastfeeding in the mother-infant dyad. This includes the recognition of anatomical barriers to breastfeeding such as a tongue-tie, ankyloglossia. PURPOSE: The purpose of this article is to enrich and expand the clinical knowledge of health professionals about the physical assessment of ankyloglossia and its impact on breastfeeding. Techniques for assessing anterior and posterior anky…
