The association of intracranial arachnoid cysts and pneumosinus dilatans is considered to be relatively rare, and has previously been described in only a limited number of cases. In this study the CT and/or MRI examinations of a total of 30 cases with middle cranial fossa arachnoid cysts were retrospectively evaluated in an attempt to note any association with pneumosinus dilatans of the paranasal sinuses. In four patients with surgically treated, large, middle cranial fossa arachnoid cysts, obvious paranasal sinus changes, consistent with pneumosinus dilatans, were noted. The frequency of the…

Twenty supratentorial and 10 infratentorial arachnoid cysts are reported. The patients were from 0 to 15 years of age. The commonest presenting symptoms in children were cranial enlargement, epileptic seizures, and psychomotor retardation. Neuroradiological evaluation included CT, metrizamide CT, cisternography, and angiography. Echography was performed in 5 newborns. Therapeutic criteria according to the clinical and neuroradiological findings are reviewed. Cystoperitoneal shunting in combination with ventriculoperitoneal shunting for associated hydrocephalus is considered the treatment of ch…

BACKGROUND: Collection of cerebrospinal fluid (CSF) in the subdural compartment is a major cause of postoperative morbidity, especially for posterior fossa surgeries. Arachnoid closure techniques, including suturing of the arachnoid and use of synthetic sealants, have been described in the literature. However, they are not always feasible or effective and have not been universally adopted.
METHODS: We describe the technique of arachnoid welding for a case of brainstem cavernoma. This is a simple, cost-effective, and easily reproducible technique using readily available bipolar cautery kept at…

Spinal adhesive arachnoiditis (SAA) with cyst formation secondary to infectious meningitis is a rare clinical entity. These cysts can compress the spinal cord and cause neurologic decline. We present a case of a patient who underwent resection for an intradural schwannoma which was complicated post-operatively by bacterial meningitis and development of several recurrent thoracic arachnoid cysts. After two separate thoracic decompressions with lysis of intradural adhesions, a permanent ventriculoperitoneal shunt (VPS) was eventually placed with complete recovery of his symptoms. Our review of t…

Use of subarachnoid/intraventricular administration of hyaluronidase in the treatment of 15 cases of cranial arachnoiditis which occurred as a complication of tuberculous meningitis is reported. Eleven of these cases had communicating hydrocephalus and four had optochiasmatic arachnoiditis. An average of 4.5 injections were administered at weekly to fortnightly intervals. These patients had been followed-up for a mean period of 6.1 months per patient. Serious side effects such as flare-up of the underlying meningitis process, miliary spread of tuberculosis, convulsions or allergic reactions we…

The patient was a 13-year-old boy who complained of pain in both buttocks. Plain and reconstructive computed tomography (CT) images showed an ossified lesion within the dura mater at the L5-S2 levels, and arachnoiditis ossificans in the lumbosacral area was suspected. In the operative findings obtained after cutting the dura, a bone fragment 4.5 × 0.5 × 0.5 cm in size was observed in the center of the strongly adhesive nerve bundle of the cauda equina, which was removed en bloc. The postoperative clinical course of the patient was excellent. The case, along with a review of literature is prese…

Arachnoiditis ossificans is a rare disorder characterized by the development of calcifications of the arachnoid membrane of the thoracic and lumbar spines. It is an extremely rare cause of spinal canal stenosis and consequent neurological compromise, and its origins and optimal management remain unclear. We review of the literature that illustrates the challenges of diagnosis and treatment of arachnoiditis ossificans. A patient with arachnoiditis ossificans is discussed to illustrate the presentation, treatment, and prognosis of the disease.

Arachnoid cysts of the posterior fossa are uncommon. Our case of a 49-year-old man presented with a 2 month history of headaches, nausea, and vertigo associated with walking instability. An MRI revealed a median well-circumscribed cystic lesion of the posterior fossa, with similar signal characteristics to CSF, and without connection to the fourth ventricle. This aspect suggested either arachnoid or hydatid cysts. Direct open surgery was performed allowing complete removal of the cyst wall, with a good outcome.

Spinal arachnoiditis is an insidious disease caused by an inflammatory process of the arachnoid membrane resulting from many possible causes, such as myelograms with oil-based radiographic contract agents and multiple back surgeries. Diagnosis is based on symptoms and magnetic resonance imaging. Arachnoiditis can also mimic the symptoms of other diseases, such as spinal cord tumors, cauda equina syndrome, arachnoiditis ossificans, and syringomyelia. Unfortunately, there is no cure, only treatment of the chronic symptoms. It is an incurable disease that can cause minor to severe symptoms from u…

BACKGROUND: Arachnoid cysts of the paranasal sinus are rare. They have not been described yet in the sphenoid sinus.
PATIENT AND METHOD: Microscopic-endoscopic endonasal surgery of the sphenoid sinus was performed on a 34-year-old female with a history of chronic headaches and a suspected mucocele of the sphenoid sinus in computed tomography (CT) and magnetic resonance imaging (MRI) studies. An extended arachnoid cyst was found in the enlarged sphenoid sinus, which was obliterated with collagen, fibrin glue, and abdominal fat.
RESULTS: There were no complications after the operation, and 12 …

BACKGROUND: Arachnoid cysts rarely occur within the lateral ventricles, with only 10 reported cases in the literature. They may arise from the arachnoid layer that is present in the choroidal fissure.
CASE DESCRIPTION: This 25-year-old man with headache and a left jacksonian seizure had an intraventricular arachnoid cyst of the right occipital horn and trigone found on radiologic examination by computed tomography (CT) and magnetic resonance imaging (MRI). He was successfully treated by evacuation of the cyst and removal of its wall through a right parietooccipital craniotomy. Histologic stud…

Although tuberculosis is an uncommon cause of spinal arachnoiditis, it needs to be differentiated from other causes of arachnoiditis because it is a treatable disease. Myelography, which is the imaging modality of choice for the chronic adhesive stage of tuberculous arachnoiditis, usually reveals irregularity of the thecal sac, nodularity and thickening of nerve roots, clumping of roots to each other or to the thecal sac, or CSF block. Recently Gd-diethylenetriamine pentaacetic acid enhanced MRI has been found to be useful for the active phase of tuberculous infection, effectively demonstratin…

OBJECTIVE: This study was undertaken to see if patients who had a radiological diagnosis of arachnoiditis attributed to methylprednisolone acetate (Depo-Medrol, Upjohn Pty Limited) had the clinical syndrome of arachnoiditis.
DESIGN: An attempt was made to review all patients, reported by Roche in 1984 with a radiological diagnosis of corticosteroid-induced arachnoiditis, by taking a detailed history and performing a physical examination.
SETTING: The Department of Orthopaedics and Traumatic Surgery, The University of Sydney, The Royal North Shore Hospital, Sydney.
RESULTS: Of the 18 patient…

The picture of neuropsychological signs detected in 34 patients with optoschiasmatic arachnoiditis was analyzed before and after surgical treatment and a course of treatment with optic nerve electrostimulation. Pathological process was shown to spread on optic nerves and schiasma, and on other brain structures as well, e. g., frontal lobes basal regions. Besides, the impairment of a range of psychic functions could result from the decrease in the general brain activation levels related to a reduced flow of visual information due to fast progress of visual disorders. In this respect, the least …

Fifty patients with arachnoiditis were studied, and long-term follow-up ranging from 10 to 21 years was obtained on 36 (72%). Prior to developing arachnoiditis, 90% originally had intervertebral disc disease, Pantopaque (Alcon Surgical, Ft. Worth, Texas) myelography, and subsequent lumbar spine surgery. Pain and functional disability tended to remain the same as at the time of diagnosis, although severity of symptoms fluctuated. Increased neurologic deficits were more frequently due to surgical intervention than to the natural course of the disease. Urinary symptoms characterized by urgency, f…
