Continuous Glucose Monitoring in Women with Normal OGTT in Pregnancy.

Continuous glucose monitoring (CGM) might be an effective tool to improve glycemic control in gestational diabetes mellitus (GDM). Few data are available about its utilization as a diagnostic tool to find potential alterations of glycemia in subjects with normal oral glucose tolerance test (OGTT). In this preliminary prospective real-life observational study, we aimed to analyze the glycemic pattern in normal and gestational diabetes mellitus (GDM) women by continuous glucose monitoring (CGM) in order to detect potential differences between the two groups and glycemic alterations despite a nor…

Continuous glucose monitoring: A review of the technology and clinical use.

Continuous glucose monitoring (CGM) is an increasingly adopted technology for insulin-requiring patients that provides insights into glycemic fluctuations. CGM can assist patients in managing their diabetes with lifestyle and medication adjustments. This article provides an overview of the technical and clinical features of CGM based on a review of articles in PubMed on CGM from 1999 through January 31, 2017. A detailed description is presented of three professional (retrospective), three personal (real-time) continuous glucose monitors, and three sensor integrated pumps (consisting of a senso…

Continuous Glucose Monitoring: A Review of Successes, Challenges, and Opportunities.

Continuous glucose monitoring (CGM) provides information unattainable by intermittent capillary blood glucose, including instantaneous real-time display of glucose level and rate of change of glucose, alerts and alarms for actual or impending hypo- and hyperglycemia, "24/7" coverage, and the ability to characterize glycemic variability. Progressively more accurate and precise, reasonably unobtrusive, small, comfortable, user-friendly devices connect to the Internet to share information and are sine qua non for a closed-loop artificial pancreas. CGM can inform, educate, motivate, and alert peop…

Continuous glucose monitoring in kidney transplant recipients: a narrative review.

Continuous glucose monitoring (CGM) has transformed diabetes management, offering real-time and dynamic insights into glucose variability and addressing the limitations of traditional glucose assessment methods. Kidney transplantation, the most common solid organ transplant, carries a considerable burden of post-transplant diabetes mellitus (PTDM), which is linked to increased cardiovascular events, graft dysfunction, and increased mortality. This review explores the role of CGM in kidney transplant recipients, particularly its impact on glycemic profiles and its predictive value for post-tran…

Prediction of gestational diabetes mellitus using continuous glucose monitoring metrics.

AIMS: We evaluated continuous glucose monitoring (CGM)-derived metrics for predicting gestational diabetes mellitus (GDM). METHODS: We analyzed data from 167 pregnant participants who had ≥ 3 days of CGM data at 18-24 weeks' gestation and who underwent 75-gram oral-glucose-tolerance-tests at 24-28 weeks in a multi-ethnic prospective cohort. Predictive performance of CGM metrics was assessed using the area-under-the-receiver-operating-characteristic-curve (AUROC) with 20 repetitions of 5-fold cross-validation; optimal cut-points were determined using Youden's index. RESULTS: There were 30 (18…

Continuous glucose monitoring in gestational diabetes mellitus: hope or hype?

The screening, diagnosis and management of gestational diabetes mellitus (GDM) have evolved over time. Similarly, continuous glucose monitoring (CGM) has advanced in parallel: from intermittent to real-time monitoring, shorter to longer wear-time and poor to increased precision. Thus, CGM has replaced multiple glucose fingerstick testing in type 1 diabetes mellitus (T1D), including pregnant women with T1D. Despite its advantages, the uptake of CGM in women with GDM remains limited. Doubts persist over its cost, accuracy and optimal utilisation, besides there is paucity of evidence supporting i…

Intra-pancreatic fat is associated with continuous glucose monitoring metrics.

AIM: To investigate the relationship of fat in the pancreas with time spent in different glycaemic ranges. METHODS: Abdominal magnetic resonance imaging at 3.0 Tesla was used to quantify fat in the pancreas as both continuous [i.e. intra-pancreatic fat deposition (IPFD)] and binary (i.e. fatty change of the pancreas vs. normal pancreas) variables. Dexcom G6 devices were used to collect continuous glucose monitoring data every 5 min over a continuous 7-day period. Time above range (TAR), time in range (TIR) and time below range were computed. Statistical models were built to adjust for age, se…

Near-Infrared Spectroscopy and Continuous Glucose Monitoring During Therapeutic Hypothermia.

The relation between glucose homeostasis and cerebral blood flow (CBF) and their correlation to outcome in neonatal hypoxic-ischemic encephalopathy are unclear. In this short communication, we tried to determine whether changes in regional oxygen saturation (rSO2), as measured by near-infrared spectroscopy (NIRS), in asphyxiated neonates during therapeutic hypothermia correlate with the glycemic profile and whether NIRS and continuous glucose monitoring are useful in identifying cooled asphyxiated neonates at high risk of brain injury. Although there was no correlation between blood glucose an…

Integrating Continuous Glucose Monitoring Into Clinical Practices and Patients' Lives.

Numerous studies have demonstrated the clinical benefits of continuous glucose monitoring (CGM) in individuals with diabetes. Within ongoing innovations in CGM technology, individuals now have an expanding array of options that allow them to select the device that meets their individual needs and preferences. Although demand for CGM in primary care continues to grow, many clinicians are reluctant to prescribe this technology due to their unfamiliarity with the various devices, uncertainty about which devices are best suited to each patient and the feasibility of using CGM. This article reviews…

Glucose-monitoring with continuous subcutaneous microdialysis in neonatal diabetes mellitus.

BACKGROUND: Neonatal diabetes mellitus can be extremely brittle. In this situation close glucose monitoring is essential for adequate insulin treatment. Continuous subcutaneous microdialysis is a promising approach for the babies to reduce the painful stress caused by diagnostic blood sampling. The goal of this study was to evaluate the feasibility of continuous subcutaneous microdialysis for glucose monitoring in a baby with neonatal diabetes and to assess the correlation between the blood and the subcutaneous glucose profile. PATIENT AND METHOD: During a period of seven days glucose monitor

Radiographic appearances of a continuous glucose monitor in a patient with lipodystrophy.

We report the case of a 50-year old woman with a known history of lipodystrophy. A pelvic radiograph was taken for the investigation of right hip pain. The image shown demonstrates an indeterminate artefact projected over the right iliac fossa. A previous CT renal study was reviewed, demonstrating the same device in the subcutaneous tissues of the contralateral left lower quadrant which on close inspection was consistent with a continuous glucose monitoring device. Features of lipodystrophy were also noted on review of the CT imaging. Although many devices such as vagal stimulators and prosthe…

Roadmap to the Effective Use of Continuous Glucose Monitoring in Pregnancy.

The landscape for managing type 1 diabetes during pregnancy has been transformed by increasing use of continuous glucose monitoring (CGM). Women are aiming for pregnancy-specific glucose targets or 70% time in range for pregnancy (TIRp; 63-140 mg/dL) as soon as possible, knowing that every extra 5% TIRp has benefits for reducing the risks of complications in their babies. Ongoing monitoring of maternal A1C (at pregnancy confirmation and at 20, 28, and 36 weeks' gestation) remains useful. Intensification of glycemic management and instruction in using CGM (if not already used) is recommended fo…
