Management of dysphonia in children.

BACKGROUND: Dysphonia is common in children, but practice varies considerably regarding what, if any, investigations are performed and how the condition is managed. Although childhood dysphonia is mostly due to non-serious causes such as voice misuse, very serious pathology such as papillomatosis or malignancy needs occasionally to be excluded, and treatable congenital anomalies such as webs and cysts can be missed. Voice clinics and voice therapy services are now well established in most adult health services in the developed world, but equivalent services for children are less common, at lea…

[Dysphonia in children and adolescents].

Hoarseness is the leading symptom of dysphonia among children and adolescents. The incidence is evaluated internationally to be 6-25%. In an overview the hereditary and acquired organic and functional causes including secondary organic lesions of the vocal folds, the pathomechanisms and the symptoms typical for this age group are presented. In clinical routine, symptoms of dysphonia in children and adolescents should be consciously looked for and any long-term hoarseness should be examined by specialized physicians, even in this age group, using methods suitable to the age group and the develo…

[Quality of life in dysphonia].

Quality of life is multidimensional and comprises physical, emotional, and social aspects. It has always been the implicit focus of medical work. However, since the 1980s it has been possible to measure it explicitly. Quality of life is impaired in dysphonic patients; this finding is supported by specific studies on self-reported physical, emotional, and social well-being. For practical application of these data, it is recommended to measure all three domains. From a therapeutic point of view, verbal intervention following the PLISSIT model (permission, limited information, special suggestions

[Complaints of patients with dysphonia].

BACKGROUND: In this study, the problems and difficulties experienced by German-speaking voice patients were assessed using a structured questionnaire. METHODS: A total of 54 patients, 30 women and 24 men, aged 48.5+/-15.8 years suffering from benign organic and functional voice disorders, underwent systematic phoniatric examination including a detailed interview about the voice problem. The same procedure was applied to 56 age and gender matched normal control persons. RESULTS: There were no significant gender or diagnosis-related differences concerning the number and type of symptoms and sy

Cortical Silent Period Reveals Differences Between Adductor Spasmodic Dysphonia and Muscle Tension Dysphonia.

BACKGROUND: The pathophysiology of adductor spasmodic dysphonia (AdSD), like other focal dystonias, is largely unknown. OBJECTIVE: The purposes of this study were to determine (a) cortical excitability differences between AdSD, muscle tension dysphonia (MTD), and healthy controls; (b) distribution of potential differences in cranial or skeletal muscle; and (c) if cortical excitability measures assist in the differential diagnosis of AdSD and MTD. METHODS: Ten participants with adductor spasmodic dysphonia, 8 with muscle tension dysphonia, and 10 healthy controls received single and paired pu…

Differentiation of adductor-type spasmodic dysphonia from muscle tension dysphonia by spectral analysis.

OBJECTIVES: To determine the utility of spectral analysis in the differentiation of adductor-type spasmodic dysphonia (AdSD) from muscle tension dysphonia (MTD). STUDY DESIGN: Prospective blinded study. METHODS: Forty-seven samples of AdSD-connected speech spectrograms from 27 subjects and 17 samples of MTD-connected speech spectrograms from 15 subjects were selected from clinical charts and de-identified. These spectrograms were reviewed independently and blindly by two speech language pathologists experienced in spectrography. The speech language pathologists designated the spectrogram as …

[Aggravating factors of dysphonia in preschool children: differences between children with and without dysphonia].

OBJECTIVE: To determine if there are differences in factors favoring dysphonia in a group of preschool children with healthy voice and another with dysphonia. METHOD: In this research a total of 96 children between 2 and 5 years old were evaluated, of which 17 had dysphonia and 79 had a healthy voice. The presence of factors favoring dysphonia was compared between both groups, by means of a questionnaire addressed to their parents or caregivers created and validated especially for this research. Statistical analysis was performed by independent samples analysis to extract the p value. RESULT…

Toward improved differential diagnosis of adductor spasmodic dysphonia and muscle tension dysphonia.

OBJECTIVE: Adductor spasmodic dysphonia (ADSD) is characterized by a strained-strangled voice quality, whose diagnosis relies exclusively on auditory-perceptual features. However, muscle tension dysphonia (MTD) -- a functional voice disorder -- can mimic ADSD thereby contributing to diagnostic confusion. Unlike MTD, ADSD has been described as 'task-specific', implying that certain vocal tasks such as sentences loaded with predominantly voiced consonants will provoke greater sign expression. This investigation examined the diagnostic value of variable sign expression based upon phonetic loading…

Automated acoustic analysis of task dependency in adductor spasmodic dysphonia versus muscle tension dysphonia.

OBJECTIVES/HYPOTHESIS: Distinguishing muscle tension dysphonia (MTD) from adductor spasmodic dysphonia (ADSD) can be difficult. Unlike MTD, ADSD is described as "task-dependent," implying that dysphonia severity varies depending upon the demands of the vocal task, with connected speech thought to be more symptomatic than sustained vowels. This study used an acoustic index of dysphonia severity (i.e., the Cepstral Spectral Index of Dysphonia [CSID]) to: 1) assess the value of "task dependency" to distinguish ADSD from MTD, and to 2) examine associations between the CSID and listener ratings. S…

Quality of life, self-perceived dysphonia, and diagnosed dysphonia through clinical tests in teachers.

OBJECTIVES: To examine the impact of voice on the quality of life of teachers and to assess whether the degree of dysphonia and otorhinolaryngologists' (ORL) diagnostics are correlated with the quality of life. METHODS: Eighty-eight female teachers from the municipal schools of Belo Horizonte who were in speech therapy at the Speech Therapy Clinic of the Hospital das Clínicas of Minas Gerais participated in the study. The variables studied were age, ORL diagnosis, perceptual-hearing assessment of voice through GRBAS scale, and vocal activities and participation profile (VAPP) protocol. Statis…

Differential diagnosis of adductor spasmodic dysphonia and muscle tension dysphonia using phonatory break analysis.

OBJECTIVES: Muscle tension dysphonia (MTD) can masquerade as adductor spasmodic dysphonia (ADSD) leading to diagnostic confusion. Intraword phonatory breaks have been offered as the sine qua non of ADSD, however, little is known regarding the presence of phonatory breaks in MTD. This investigation assessed the diagnostic worth of acoustic analysis of phonatory breaks as a possible objective test to distinguish ADSD from MTD. STUDY DESIGN: Case-control comparison. METHODS: Voice samples from patients with confirmed ADSD (n = 41) and MTD (n = 59) were analyzed acoustically to determine the pre…
