[Flecainide and paroxysmal supraventricular tachycardia in the operating room].

The case is reported of a 52-year-old woman who developed paroxysmal supraventricular tachycardia prior to induction of general anesthesia. This arrhythmia, resistant to vagal stimulation and verapamil, was finally ended by the administration of flecainide in the operating room.

Experience with flecainide for the treatment of cardiac arrhythmias in children.

We treated 22 children, aged 3 days to 16 years 6 months (median 11 years 1 month), with flecainide for a variety of arrhythmias where a Class I agent was indicated. In 16, conventional antiarrhythmic treatment had failed. Structural heart disease was present in nine. The arrhythmia was paroxysmal re-entry atrioventricular tachycardia in nine; paroxysmal atrial tachycardia, flutter or fibrillation in five; paroxysmal ventricular tachycardia in five and frequent ventricular extrasystoles (with couplets) in three. Sinus rhythm was achieved in all four children who received flecainide during tach…

Treatment of refractory supraventricular arrhythmias with flecainide acetate.

We treated 13 children aged 0.2 years to 15.7 years (median 7.1 years) with flecainide acetate for refractory symptomatic supraventricular tachycardia. Six children had direct atrioventricular accessory pathways, of whom four had overt Wolff-Parkinson-White syndrome on the 12 lead electrocardiogram, while in the other two the accessory pathway was concealed. Three children had nodal atrioventricular re-entrant tachycardia, two had a re-entrant tachycardia, the exact mechanism of which was not known, one child had ectopic atrial tachycardia, and one had atrial flutter associated with an atrial …

Device Interrogation Unmasking Flecainide-Induced Wide Complex Tachycardia.

BACKGROUND: Wide-complex tachycardia poses diagnostic and therapeutic challenges, particularly when class IC antiarrhythmic toxicity mimics ventricular tachycardia (VT). CASE SUMMARY: An 85-year-old woman with a dual-chamber pacemaker, paroxysmal atrial fibrillation on flecainide, and chronic kidney disease presented with dyspnea and a wide-complex tachycardia initially diagnosed as VT. Intravenous amiodarone was administered. Initial device interrogation revealed ventricular sensing at 496 ms cycle length without atrial tracking. Ventricular overdrive pacing demonstrated that the atrial cycl…

Flecainide causing elevated capture thresholds on pacemaker implantation.

Flecainide is a Vaughan Williams class 1c antiarrhythmic used to treat supraventricular and ventricular arrhythmias. It has been described as a rare cause for increased pacemaker capture thresholds. We describe a report of a patient, in her early 80s, presenting with tachy-brady syndrome on a background of permanent atrial fibrillation. She was treated with metoprolol and flecainide by her private cardiologist. Permanent right ventricular chamber pacing was recommended for her slow heart rate. At insertion of her single chamber pacemaker, she was noted to have elevated capture thresholds despi…

Haemolysis alters plasma flecainide levels in newborn infants.

Refractory neonatal supra-ventricular tachycardia may require the use of flecainide as anti-arrhythmic. Close control of plasma levels is mandatory due to pro-arrhythmic complications. However, inadvertent hemolysis during blood sampling may cause an increase in measured plasma flecainide concentrations. We conclude, therefore, that dosing of the drug should be always done with caution, and in case of suspected haemolysis plasma levels should be repeated with a new blood sample.

Brugada electrocardiographic pattern elicited by inadvertent flecainide overdose.

In a 70-year-old man, without clinical suggestion of the hereditary form of the Brugada syndrome, severe flecainide overdose resulted in profound widening of the QRS complex and the development of ST-T abnormalities typical of the Brugada sign. Serial ECGs recorded over a 5-day period revealed a parallel pattern of dynamic intraventricular conduction defect and ST-segment elevation. Resolution of ST-segment elevation lagged behind resolution of the QRS widening by 16-20 hours. Despite the marked Brugada abnormality no arrhythmia occurred and the patient recovered without complications.

Acute reversible ataxo-myoclonic encephalopathy with flecainide therapy.

A 73 year-old patient with Wolff-Parkinson-White syndrome and paroxysmic supraventricular tachycardia developed an acute reversible encephalopathy within 15 days of initiation of flecainide. The clinical picture was characterized by visual hallucinations, agitation, anxiety, but no disorientation, and a severe cerebello-myoclonic syndrome with total inability to stand up and walk, which was fully reversible on discontinuing the medication.

Prospective comparison of flecainide versus quinidine for the treatment of paroxysmal atrial fibrillation/flutter. The Flecainide Multicenter Atrial Fibrillation Study Group.

In order to compare the efficacy and long-term tolerability of flecainide acetate versus quinidine, 239 patients with paroxysmal atrial fibrillation were prospectively treated with flecainide (n = 122) or quinidine (n = 117) in an open-label, randomized trial. All patients were followed for 1 year after initiation of therapy unless their antiarrhythmic drug was discontinued due to an inadequate therapeutic response or intolerable side effects. Although both drugs were equally effective in adequately controlling recurrences of atrial fibrillation (difference, p = 0.282; not significant), fewer …

Flecainide sensitizes conduction to hyponatremia through an ephaptic mechanism.

BACKGROUND: Studies suggest that voltage-gated sodium channel (SC) loss-of-function (LoF), often through the use of SC blockers, such as tri … luence CV sensitivity to [Na+] during voltage-gated SC LoF induced by the class 1c anti-arrhythmic, flecainide. METHODS: CV was measured by optical mapping of Langendorff-perfused guinea pig hearts with either 145 or 120 mM [Na+] under control conditions, with flecainide alone, and the combination of flecainide with ephaptic coupling uncouplers mannitol or …

Perioperative Management of Flecainide: A Problem-Based Learning Discussion.

Flecainide is a first-line antiarrhythmic drug used to treat atrial arrhythmias and/or supraventricular tachycardia in those without coronary artery disease or structural heart disease. Even though it is an older antiarrhythmic, flecainide accounted for 1.6 million prescriptions in the United States in 2016, and its utilization is generally increasing. Despite its popularity, flecainide may predispose patients to rapid atrial flutter with resultant hemodynamic compromise, particularly in the physiologically stressful perioperative period. This article reviews the pharmacology of flecainide, de…

Flecainide-induced QRS complex widening correlates with negative inotropy.

BACKGROUND: The negative inotropic effect of Class IC antiarrhythmic drugs limits their use for acute cardioversion of atrial fibrillation (AF). OBJECTIVE: The purpose of this study was to examine, in an intact porcine model, the effects of pulmonary and intravenous (IV) administration of flecainide on left ventricular (LV) contractility and QRS complex width at doses that are effective in converting new-onset AF to sinus rhythm. METHODS: Flecainide (1.5 mg/kg bolus) was delivered by intratracheal administration and compared to 2.0 mg/kg 10-minute IV administration (European Society of Cardi…

Multiple targets for flecainide action: implications for cardiac arrhythmogenesis.

UNLABELLED: Flecainide suppresses cardiac tachyarrhythmias including paroxysmal atrial fibrillation, supraventricular tachycardia and arrhythmic long QT syndromes (LQTS), as well as the Ca2+ -mediated, catecholaminergic polymorphic ventricular tachycardia (CPVT). However, flecainide can also exert pro-arrhythmic effects most notably following myocardial infarction and when used to diagnose Brugada syndrome (BrS). These divergent actions result from its physiological and pharmacological actions at multiple, interacting levels of cellular organization. These were studied in murine genetic models…

Multiple modes of ryanodine receptor 2 inhibition by flecainide.

Catecholaminergic polymorphic ventricular tachycardia (CPVT) causes sudden cardiac death due to mutations in cardiac ryanodine receptors (RyR2), calsequestrin, or calmodulin. Flecainide, a class I antiarrhythmic drug, inhibits Na(+) and RyR2 channels and prevents CPVT. The purpose of this study is to identify inhibitory mechanisms of flecainide on RyR2. RyR2 were isolated from sheep heart, incorporated into lipid bilayers, and investigated by single-channel recording under various activating conditions, including the presence of cytoplasmic ATP (2 mM) and a range of cytoplasmic [Ca(2+)], [Mg(2…

Flecainide overdose--support using an intra-aortic balloon pump.

BACKGROUND: Flecainide is an antiarrhythmic agent which is being used increasingly for the management of super-ventricular arrhythmias. Overdose with flecainide is frequently fatal with mortality reported as high as 22% due to arrhythmias, myocardial depression and conduction defects leading to electro-mechanical dissociation and asytole. Supportive measures are often required during the case and previously have included inotropes, extracorporeal membrane oxygenation and cardiopulmonary bypass. CASE PRESENTATION: A 47 year old lady presented to the emergency department with a four hour histor…

Neonatal intractable atrial flutter successfully treated with intravenous flecainide.

We present a neonatal case with intractable atrial flutter that did not respond to digitalization and electrical cardioversion. Intravenous flecainide administration completely resolved the atrial flutter. Proarrhythmic effects were not induced by flecainide administration. Although the efficacy of flecainide for atrial flutter during the infantile or childhood period is low, intravenous flecainide is worth consideration as a treatment for atrial flutter, even in intractable cases as described here, during the neonatal period.
