BACKGROUND: The intraatrial conduit (IAC) Fontan procedure is one of the Fontan modifications and is usually not considered the first choice. In this large series, we report our experience of the IAC Fontan procedure for the treatment of a functional single ventricle and review its indications, techniques, and clinical outcomes.

For 5 patients with univentricular heart associated with apico-caval juxtaposition, an extracardiac Fontan procedure was carried out using an artificial graft bridging the vertebra to avoid graft compression by the vertebra and the ventricle. Since 1974, we have performed modified Fontan procedure on 106 patients, ranging in ages from 1 to 32 years, consisting of 44 cases of tricuspid atresia (TA), 21 with univentricular heart (UVH) of right ventricular type, 18 with UVH of left ventricular type, for which ventricular partition was unfeasible, and 23 with various complex anomalies. Hospital mortality rates for TA and other complex anomalies were 11.4 and 11.3%, respectively. Surgical results have markedly improved recently. A four-year-old boy with DORV and hypoplastic left ventricle underwent a modified Fontan procedure. He had a very difficult postoperative course complicated by continued pleural effusions. Eventually he died during a last ditch effort to ligate the thoracic duct at the right diaphragm. At autopsy, an obstructing giant thrombus was found in the right atrium. This rare complication probably was due to multiple factors, but we now anticoagulate all Fontan patients routinely in an effort to prevent this fatal problem.

BACKGROUND: The purpose of this study was to determine the type and incidence of hemodynamic and electrophysiological abnormalities requiring surgical or catheter-based interventions in a single-center long-term experience.
