INTRODUCTION: Since anesthesia complications associated with unexpected difficult airway are potentially catastrophic, they should be avoided. The modified Mallampati test and jaw-thrust maneuver enable the identification of difficult airway. The aim of this study was to associate the modified Mallampati test and the jaw-thrust maneuver with laryngoscopy (Cormack–Lehane) in an attempt to identify a better predictor of difficult airway in an adult population undergoing elective surgery.
METHOD: A cross-sectional study in which 133 adult patients undergoing elective surgery requiring tracheal i…

The direct view into the glottis can be difficult due to long teeth in the upper jaw or abnormities of the neck vertebrae. It is recommended in these cases to use either a mediastinoscope with a lateral slot or the McIntosh laryngoscope, which is commonly used for intubation. The latter can be attached to the chest stay of the normal laryngoscope by using a special joint. The tip of the McIntosh laryngoscope is inserted into the valleculae instead of loading up the epiglottis as is done with the normal laryngoscope. Recommended aids for ciopsy or polypectomy are the 30 degrees-optical system o

Spastic dysphonia is a speech disorder, characterized by a "strangled voice quality," irregularly occuring phonatory arrests, and a jerky pattern of speech. This is often extremely disruptive to the patient's ability to communicate. Two cases are presented in which a new technique, videofiberoptic laryngoscopy, was employed in the study of these patients. The characteristic laryngeal behavior of this disorder is documented and recorded, particularly noting the increase in supraglottic activity and how different vocal maneuvers alter this activity.

Retractors and scopes used in head and neck surgery to provide adequate surgical exposure also deform critical structures in the region. Surgeons typically use preoperative imaging to plan and guide their tumor resections, however the large tissue deformation resulting from placement of retractors and scopes reduces the utility of preoperative imaging as a reliable roadmap. We quantify the extent of tumor and vasculature deformation in patients with tumors of the larynx and pharynx undergoing diagnostic laryngoscopy. A mean tumor displacement of 1.02 cm was observed between the patients' pre- …

BACKGROUND: Patients presenting for thyroidectomy may have an unrecognized pre-existing vocal cord palsy (VCP). This raises the danger of bilateral VCP if a patient sustains an injury to the RLN on the sole functioning side. Part of the rationale for routine preoperative laryngoscopy is to eliminate such a risk. This paper endeavours to quantify the relevant potential risk.
METHODS: Patients who underwent laryngoscopy prior to thyroid or parathyroid surgery in an endocrine surgical unit over a 5 year period were identified. Literature review revealed four papers in which VCP prevalence in pat…

A 78-year-old man, weighing 74 kg and 172 cm in height suddenly developed asystole during direct laryngoscopy. His heart started beating soon after chest compressions. Direct larygoscopy can stimulate the vagal nerve of the larynx. Although a gradual decrease in heart rate ordinarily occurs prior to asystole, few reports describe the sudden asystole during direct laryngoscopy. Intravenous injection of atropine could avoid the adverse event. Anesthesiologists should pay attention to the occurence of asystole and prepare for resuscitation.

OBJECTIVES: External laryngeal manipulation (ELM) is commonly used to facilitate laryngeal view during direct laryngoscopy. We evaluated the effectiveness of the newly modified bimanual laryngoscopy, which involves a direct guidance of an assistant's hand by a laryngoscopist, to optimize laryngeal exposure during direct laryngoscopy compared with conventional bimanual laryngoscopy.
METHODS: A total of 78 adult patients were included. Patients were randomly allocated to 1 of 2 groups: group C (ELM using conventional bimanual laryngoscopy) or group M (ELM using the modified bimanual laryngoscop…

Suspension laryngoscopy is a common laryngeal procedure in Endolaryngeal microsurgery (ELMS). Oral mucosa and dental injuries are the known complications of the procedure. Nerve injury however is an infrequent encounter. We report a rare complication of lingual nerve injury which manifested as tongue numbness and altered taste following Endolaryngeal microsurgery procedure. The condition improved completely after few months of conservative management.

Injuries can occur during endotracheal intubation. This prospective, randomized, blinded study aimed to determine if a difference exists in incidence of injury following endotracheal intubation using either direct laryngoscopy or the GlideScope video laryngoscope (Verathon Inc). A convenience sample of 155 patients scheduled to undergo surgery to the head, face, neck, or throat were randomized to either GlideScope or conventional direct laryngoscopy groups. Following induction of general anesthesia and endotracheal intubation, the patients were assessed for injuries to the mouth and pharynx by…

PURPOSE: The McGrath video laryngoscope (VL) offers excellent laryngoscopic views and increases the success rate of orotracheal intubation in patients with normal and difficult airways. The purpose of this randomized controlled trial was to compare the McGrath VL with the Macintosh laryngoscope to investigate the efficacy of the McGrath VL for routine nasotracheal intubation in patients with an expected normal airway.
MATERIALS AND METHODS: To address the research purpose, the efficacy of the McGrath VL for routine nasotracheal intubation was compared with that of the Macintosh laryngoscope.

BACKGROUND: The high incidence of airway management failure in the emergency department (ED) necessitates a comparative analysis of laryngoscopy methods. This study aims to compare the success and complications associated with video-assisted laryngoscopy (VL) and direct laryngoscopy (DL) in emergency tracheal intubation in ED.
METHODS: This retrospective cohort study was conducted at the ED of Thammasat University Hospital. It involved adult patients undergoing emergency tracheal intubation using either VL (GlideScope®) or DL (Macintosh®). The outcomes assessed were success rates of intubatio…

STUDY OBJECTIVES: Direct laryngoscopy is the most commonly used modality for endotracheal intubation in the emergency department. Video laryngoscopy may improve glottic view during laryngoscopy and intubation success rate in such patients. This meta-analysis has been designed to compare clinical efficacy of video laryngoscopy with direct laryngoscopy for endotracheal intubation in the emergency department.
DESIGN: Meta-analysis of randomized controlled trial.
SETTING: Randomized controlled trials comparing video laryngoscopy and direct laryngoscopy for endotracheal intubation in adult patien…

Video laryngoscopy (VL) has been traditionally thought of as reserved for difficult airways or as a rescue device after direct laryngoscopy fails to achieve intubation. However, as studies comparing the 2 continue to be churned out, favoritism toward VL may soon shift the standard of care. This article aims to explore the history of both briefly, review relevant studies, garner a scope of current airway management societal recommendations, and peer into what the future may hold for both methodologies of laryngoscopy to facilitate intubation.

OBJECTIVE: The objective was to compare first-attempt intubation success using direct laryngoscopy augmented by laryngeal manipulation, ramped patient positioning, and use of a bougie (A-DL) with unaided video laryngoscopy (VL) in adult emergency department (ED) intubations.
METHODS: This study was a secondary analysis of a multicenter prospective observational database of ED intubations from the National Emergency Airway Registry (NEAR). We compared all VL procedures to seven exploratory permutations of A-DL using multivariable regression models. We further stratified by blade shape into hyp…

The article presents successful intubation experience in 54 patients. Laryngoscopy was performed with McGrath Series 5 laryngoscopy with a difficult airway blade ("Airway Medical") in 46 patients, and retromolar endoscope Bonfils ("Karl Storz") in 9 patients. Technical traits, resulting from the use video laryngoscopy and retromolar endoscope are discussed. It was shown that video laryngoscopy is a high-performance intubation technique, including difficult ones. The possibility of video laryngoscope conduction in case of significant difficulties with mouth opening (max incisors distance of 1.3…
