Nipple discharge is a common complaint among women. It is classified as normal or abnormal depending on features such as laterality, cycle variation, quantity, color or presentation (i.e., induced vs spontaneous). Paget disease of the nipple is a rare unique presentation of breast cancer. The main differential diagnosis is dermatitis. We describe two patients with mammary Paget: one, a women aged 41, with limited ductal carcinoma in situ and the other, a women of 56 years, with invasive carcinoma. A third patient, a woman of 46 years, had typical eczema of the areola. This disease usually leads to less well circumscribed lesions than Paget. In a series of 9,312 women who consulted one of the authors with a complaint of disease of the breast between 1959 and 1991, nipple discharge was the presenting symptom in 448 (4.8 percent). Nipple discharge was spontaneous in 243 (2.6 percent) and provoked in 205 (2.2 percent) of the patients. Fifty-five female patients with Paget's disease of the nipple treated in our Hospital from 1958 to 1987 are reported. It comprised 1.1% of all the breast carcinomas. Nipple-areola reconstruction is an integral part of breast reconstruction. Optimum results are usually obtained when nipple-areola reconstruction is staged after the breast mound has attained its final shape and is well vascularized. The use of intradermal tattoo allows the use of a variety of nonpigmented donor sites. Women report that reconstruction of the nipple-areola enhances their overall satisfaction with breast reconstruction. The knowledgeable and skilled nurse is a valuable member of the professional team during this final phase of breast reconstruction.

Fourteen primary melanomas arising in the nipple and areola of the breast were treated by mastectomy and axillary dissection. Four patients had axillary lymph node metastases and all were dead within 3 years of their operation, while the 10 patients with no axillary node involvement were free from recurrent disease 5 years after their operation. On the basis of clinical and anatomical studies, it is suggested that a wide local excision without mastectomy is adequate for the treatment of nipple and areola melanomas.

Hyperkeratosis of the nipple and areola is a rare condition that may occur in association with various forms of ichthyosis or as the result of extension of an epidermal nevus. The nevoid form is extremely rare and appears most commonly in women in their second or third decade of life. It is characterized by verrucous thickening and brownish discoloration of both nipples and areolae. Thirteen cases of this type have been reported. We are describing two additional cases to bring the total to 15 instances.
