The aim of our work was the presence of the second phase nystagmus after rotatory, caloric and optokinetic irritation; estimation of this nystagmus magnitude and the clinical application of the second phase nystagmus. The investigation was performed in 30 healthy subjects and 65 cases of the peripheral and central vestibular pathology. Our results show that the second phase nystagmus appears more often after strong vestibular or visual irritation; the largeness of this reaction is bigger than the first reaction nystagmus. Only the asymmetry of the second phase nystagmus has the clinical apply.

Nystagmus is a common and challenging ophthalmic disorder characterized by involuntary, rhythmic, to-and-fro or jerky oscillations of the eyes. Its etiology is complex, and it severely affects patients' visual quality and quality of life. In recent years, with the rapid development of neuro-ophthalmology, genetics, and imaging technologies, significant progress has been made both domestically and internationally in understanding the pathophysiological mechanisms, refined diagnostic classification, and diversified treatment strategies for nystagmus. Based on previous expert consensus, research

In two monkeys, we recorded spontaneous eye movements before and after ablation of the cerebellar nodulus and uvula (Nod/Uv). In both monkeys, there was an increase in upward ocular drift (downbeat nystagmus [DBN]) in darkness (M1: 1.5 degrees/s pre, 3.4 degrees/s post; M2: 1.3 degrees/s pre, 7.0 degrees/s post), but not in light. There was little effect of orbital position on drift velocity. These findings suggest that the Nod/Uv may play a role in the bias component of DBN.

The first reports of an involvement of the lateral canal (LC) in paroxysmal positional vertigo (PPV), were published in 1985, by Luciano Cipparrone et al., from Italy and Joseph McClure from Canada. The increasing interest of otolaryngologists and neurologists has led to a progressive advance in the knowledge of this labyrinthine disorder regarding its epidemiological, physiopathological, clinical, and therapeutic aspects. According to the most recent data, LC-benign PPV accounts for 17% of all PPV patients, regardless of gender and between the two labyrinths. The LC-PPV syndrome is characteri

The etiopathogenesis of benign paroxysmal vertigo (BPV) lies in the accumulation of otoconial or other debris in an abnormal site (the cupola or semicircular canal). The present work proposes a treatment for BPV located in the horizontal semi-circular canal aimed at moving the debris out of the canal. This maneuver involves performing a series of movements with the patient's head so that the debris is moved toward the non ampular end of the canal and, from there, into the utricle. The maneuver is programmed differently according to the variation of the disorder. This method was performed on 18

Effective management improves the sensory-motor state of most optical nystagmus cases. Treatment is always medical and sometimes surgical. Previous complete sensory and motor examinations are necessary. A surgical cure is warranted by the extent of nystagmus, a torticollis, or when a squint is associated. The authors review the various clinical aspects, and their operative plan, which is simple in Kestenbaum's, more delicate in dissociated forms, convergence blockages and nystagmic squints. It is often very difficult in complex nystagmic impairments.

The liberatory maneuver of Semont is an effective physical treatment for benign paroxysmal positional vertigo. It works because it causes otoconia to move out the posterior canal. The effectiveness of the maneuver is thought to be indicated by the appearance of a liberatory nystagmus. After the maneuver, patients are usually instructed to keep their heads erect for several days and not to lie on the pathologic side for about a week. Here we investigated the prognostic value of liberatory nystagmus and whether restrictions are necessary after treatment. Fifty-six patients with posterior canal b

BACKGROUND: Acute vestibular syndrome (AVS) is characterized by sudden, persistent vertigo or dizziness with nausea, vomiting, postural instability, and spontaneous nystagmus. Differentiating peripheral vestibular disorders from posterior circulation stroke remains a major diagnostic challenge in emergency neurology, as vestibulo-cerebellar infarcts may present as "central pseudo-neuritis," mimicking peripheral vestibular disease. Although the HINTS examination is a valuable bedside tool, its reliability depends on the presence and characterization of nystagmus and is reduced in patients witho

Direction-changing nystagmus on positional testing is classically ascribed to a central pathology. We herein report a case of a patient with Benign Paroxysmal Positional Vertigo (BPPV) who demonstrated the unusual phenomenon of spontaneously reversing nystagmus, and discuss the theorised mechanisms with a novel illustration. In left lateral position, our patient's Videonystagmography (VNG) demonstrated an initially fast-phase geotropic nystagmus (leftward-beating, SPV 29°/s) which then paused for 8 s, then spontaneously reversed direction into a slow-phase ageotropic nystagmus (rightward-beati

OBJECTIVE: To investigate adherence, characteristics, and first clinical experiences of capturing ictal nystagmus at home, which can be performed to complement the diagnostic process in patients with episodic vestibular symptoms.
METHODS: Patients were recruited at a tertiary referral center in case capturing ictal nystagmus could contribute to the diagnostic process (e.g., to detect or rule out BPPV). They were asked to capture ictal nystagmus with their own smartphone at home, using a smartphone-based adapter (Nystagmocatcher, Balansdiagnos, Stockholm, Sweden). All recordings were analyzed
